Rollover motor vehicle crashes and occupant ejection: comparative analysis from a Middle Eastern trauma center.

OBJECTIVES: Rollover motor vehicle crashes (ROCs) and occupant ejection (OE) are associated with worse outcomes. We studied the incidence, predictors, injury patterns, and outcomes of ROCs and OE in a rapidly developing country. METHODS: This was a retrospective analysis of the Qatar National Trauma Registry (2013-2024). Patients were grouped as ROCs with ejection (Group I), ROCs without ejection (Group II), and ejection without ROC (Group III). We applied the Haddon Phase-Factor Matrix. RESULTS: 2,764 patients were analyzed into Group I (n = 908; 32.9%), Group II (n = 1,626; 58.8%), and Gro

The necessity of CT/CTA scans on pediatric cerebrovascular injury after blunt trauma - a single center study.

BACKGROUND: Blunt cerebrovascular injury (BCVI) is a rare but potentially devastating complication of pediatric blunt trauma. Delayed or missed diagnosis may result in ischemic stroke, permanent neurological deficits, or death. To avoid missed injuries, many trauma centers apply liberal computed tomography angiography (CTA) screening strategies, although this may expose children to substantial and potentially unnecessary radiation. Several adult- and pediatric-derived screening tools have been proposed, but their applicability in pediatric trauma populations remains controversial. This study a…

Outcome of distal femoral replacement: Influence of trauma vs. elective indications - A prospective single-center study.

BACKGROUND: Distal femoral replacement (DFR) is often the last leg-saving option in advanced femoral bone defects after total knee arthroplasty (TKA) and periprosthetic distal femoral fractures. The aim of this study was to compare the midterm outcome of DFR following periprosthetic fracture or elective TKA revision. METHODS: This prospective cohort study included 93 DFR patients (49 trauma, 44 elective) from 2010 to 2021. DFR for revision in periprosthetic joint infection (PJI) or tumor resections were not included. Data on demographics, surgical history and reason of failure until DFR, revi

Enteral protein supplementation in critically ill trauma and surgical patients: A single-center randomized clinical trial.

BACKGROUND: Critically ill trauma and surgical patients are highly catabolic, with expected high protein needs. However, there is uncertainty regarding the amount of protein required to optimize their outcomes. We conducted a single-center, randomized clinical trial to test the hypothesis that supplementing enteral protein intake would improve outcomes. METHODS: Between November 15, 2016, and November 26, 2021, critically ill trauma and surgical patients were randomized either to a treatment arm aimed to deliver 2 g/kg/d of enteral protein or to standard nutritional care. Data were collected

Persistent disparities between trauma center types in the management of children with high-grade blunt splenic injuries.

BACKGROUND: In the early 2000s, substantial variations were reported in the management of pediatric patients with blunt splenic injury (BSI). The purpose of this study was to assess the recent trends and disparities between different types of trauma centers. We hypothesized that there would be persistent disparities despite decreased trends in the rate of splenectomy. METHODS: This is a retrospective cohort study using the American College of Surgeons Trauma Quality Improvement Program database. We included patients (age ≤18 years) with high-grade BSI (Abbreviated Injury Scale 3-5) between 20…
