AIMS: To investigate whether there are extractable conclusions for limb fracture management in dependent alcoholics.
METHODS: We discuss four cases of dependent alcoholics who presented in our department over a 12-month period, and who developed significant complications owing to non-compliance with treatment.

In 2000, conjoined twins were born in England. What made this case unique was the fact that if the twins remained unseparated, medical opinion held they would die; if they were separated one twin would live, and one twin would die; the parents refused to consent to separation; and the hospital charged with their care brought the matter to court. We confirmed the diagnosis of multiple sclerosis in a 54-year-old man. The patient rejected this diagnosis and was completely convinced that all his symptoms resulted from a chronic mercury intoxication. We found evidence that this delusional disorder was an independent illness, and not a "symptomatic psychosis in multiple sclerosis." So far, little notice has been taken in psychiatry of delusions of environmental poisoning, which cause difficulties in applying common criteria of delusion.

Technology abandonment may have serious repercussions for individuals with disabilities and for society. The purpose of this study was to determine how technology users decide to accept or reject assistive devices. Two hundred twenty-seven adults with various disabilities responded to a survey on device selection, acquisition, performance, and use. Correctional facilities are obligated to provide psychotropic medications, as part of standard psychiatric care, to inmates with serious mental illness. The right to refuse such medication treatment has become one of the most important and contested areas of legal regulation of correctional mental health. During a combined acupuncture-neural therapy-treatment on account of a gastric ulcer several lesions with vast haemorrhages in the abdominal cavity and in the retroperitoneal space on the right (lesions of the kidney) were produced. At the operation a duodenal ulcer without perforation was found. The competence of the neural therapy in its relation to the complications is discussed, the indication to the application of these forms of therapy in such cases is refused.

Four cases of isolated fourth ventricle in neurocysticerocosis are reported (three after ventriculoperitoneal shunting, and one after hydrocephalus without shunting). Diagnosis was made based on computed tomography (CT) and/or CT cisternogram/ventriculogram. Three patients underwent fourth ventricle drainage (two died and the third remains well). The fourth patient refused surgical treatment and continues to have severe ataxia. Diagnosis, pathophysiology, and treatment of this entity in neurocysticercosis are discussed.

INTRODUCTION: Postoperative nausea and vomiting (PONV) is prevalent among surgical patients, causing hospitalizations, extended stays, and patient dissatisfaction.
