Rectal tuberculosis after infliximab therapy despite negative screening for latent tuberculosis in a patient with ulcerative colitis.

Tumor necrosis factor-α inhibitors are now considered as standard therapy for patients with severe inflammatory bowel disease who do not respond to corticosteroids, but they carry a definite risk of reactivation of tuberculosis. We present a case in which a patient with inflammatory bowel disease developed a de novo tuberculosis infection after the start of anti-tumor necrosis factor-α treatment despite showing negative results in tuberculosis screening. Although there are many case reports of pleural, lymph nodal and disseminated tuberculosis following infliximab therapy, we present the first

[Extrapulmonary infections with Mycobacterium tuberculosis complex].

In a retrospective study we reviewed all records of patients with newly diagnosed Mycobacterium tuberculosis complex infection during the years 1990-1994. Of the 45 patients identified, 27 (60%) had only pulmonary tuberculosis and 18 (40%) had extrapulmonary tuberculosis with (5 patients, 11%) or without (13 patients, 18%) concomitant pulmonary infection. The extrapulmonary manifestations were as follows: miliary 5, joint 3, meningeal 2, pleural 2, kidney 1, peritoneal 1, bone 1, psoas 1, lymphatic 1, and anal 1. In the 18 patients with extrapulmonary tuberculosis the infection was due to Myco

Radiological features of asbestos-related visceral pleural changes.

The radiological manifestations of asbestos-related visceral pleural changes are described. Generally, visceral pleural reactions follow the mesothelial cells response to various injurious substances, including asbestos, and even saline. The changes are nonspecific. They may occur subsequent to pleural reactions associated with many conditions, which include tuberculosis, viral pleurisy, malignancy and lymphoma, lupus, or rheumatoid-induced effusions, cardiac failure, and pulmonary embolism, among other etiologies. The failure to absorb the fibrinous exudate on the visceral pleural surface can

Adenosine deaminase activity, not diagnostic for tuberculous pleurisy.

We have measured adenosine deaminase (ADA) in pleural effusions of 95 patients, using a method optimalised for rapid determination on a Hitachi 705 analyzer. High ADA activity was found in four of the five patients with tuberculous pleurisy, in four of the seven with empyema and in three of the seven patients with mesothelioma. One patient with very high serum ADA activity due to liver disease also had a high activity in the pleural effusion. Low activity was found in all patients with other neoplastic pleural effusions, parapneumonic pleural effusions, transudates, and in pleural effusions du

Image-guided pleural biopsy.

PURPOSE OF REVIEW: The most efficient and cost-effective approach to pleural exudates not diagnosed by means of thoracocentesis remains uncertain. Both closed pleural biopsy and thoracoscopy may be utilized for the acquisition of pleural tissue. This review will focus on the developments in image guidance of closed pleural biopsy. RECENT FINDINGS: Recent studies suggest that computed tomography and ultrasound guidance improve the yield and safety of closed pleural biopsy. Imaging is best suited to reduce the rate of false-negative biopsy in malignant pleural disease by enhanced targeting of l

Tuberculosis: frequency of unusual radiographic findings.

The changing pattern of the radiologic presentation of tuberculosis was noted in a review of 100 consecutive cases of pulmonary tuberculosis. Active tuberculosis was not suggested as a radiologic diagnosis in 43% of the patients. Of the 100 cases, only 66 had characteristics of reactivation tuberculosis (i.e., minimal upper lobe infiltrate or more advanced cavitary disease). Of the other 34 patients, 18 had less usual presentations, making the radiographic diagnosis difficult. The remaining 16 patients had radiographic patterns quite unusual for tuberculosis, including masslike densities resem

Acute myeloid leukemia with leukemic pleural effusion and high levels of pleural adenosine deaminase: A case report and review of literature.

Pleural effusions are rarely observed in association with acute myeloid leukemia (AML), and their true incidence remains unknown. Given the low diagnostic yield from cytopathologic analysis of malignant pleural effusions and the fact that patients with leukemia are often thrombocytopenic and unable to tolerate invasive procedures, the incidence of leukemic effusions may be underestimated. Here, we report a rare case of pleural effusion in a patient with newly diagnosed AML. Initial analysis revealed an exudative, lymphocyte-predominant effusion. High levels of adenosine deaminase (ADA) were de

Pleural effusion in a patient with allergic bronchopulmonary aspergillosis.

The involvement of pleura in patients with allergic bronchopulmonary aspergillosis (ABPA) is uncommon. In most reported cases the involvement of the pleura is presumed to be due to ABPA per se, due to the observed response to corticosteroids. We report a case of histopathologically proven tubercular pleural effusion in a patient with ABPA at the time of initial diagnosis. This case highlights the importance of intensive evaluation of an exudative pleural effusion in settings where pleural effusion due to the primary underlying disease is uncommon or rare.

Prolidase activity in serum and pleural fluids in patients with tuberculous pleural effusion [correction of effussion].

OBJECTIVES: Pleural tuberculosis, which is present in around 4% of all tuberculosis cases may resolve spontaneously or associated with progressive disease and a high recurrence rate. Recently upon exposed to cytokines and bacterial products, mesothelium has been shown to produce collagen that may be involved in pleural inflammatory responses. Prolidase is involved in the final stage of degradation in collagen catabolism. In this study we aimed to evaluate pleural fluid and serum prolidase activities in patients with tuberculous (TB) pleurisy and compared with those in non-tuberculous (non-TB)

Potential diagnostic value of pleural fluid cytokines levels for tuberculous pleural effusion.

Patients with tuberculous pleural effusion (TPE) or malignant pleural effusions (MPE) frequently have similar pleural fluid profiles. New biomarkers for the differential diagnosis of TPE are required. We determined whether cytokine profiles in the PE of patients could aid the differential diagnosis of TPE. 30 patients with TPE, 30 patients with MPE, 14 patients with empyema (EMP) and 14 patients with parapneumonic effusion (PPE) were enrolled between Dec 2018 and 2019. The levels of interleukin (IL)-6, IL-18, IL-27, CXCL8, CCL-1 and IP-10 were determined in PE by ELISA along with measurements

Tired of the confusion around pleural effusions: Adenosine deaminase detection sets the record straight!

Pleural effusion, characterized by the accumulation of fluid in the pleural space, poses significant challenges in clinical practice, especially in determining whether it belongs to the inflammatory exudates or non-inflammatory transudates. Adenosine deaminase (ADA), an enzyme primarily produced by immune cells, particularly lymphocytes, increase in response to inflammatory conditions, including tuberculosis and malignancies. Elevated ADA levels in pleural have been shown to correlate with inflammatory exudates, making it a valuable biomarker for differentiating between inflammatory and non-in

[Pleurisy and sarcoidosis. Apropos of 2 cases].

Pleural involvement is uncommon in sarcoidosis, observed in less than 5% of cases. We report two cases. Both patients were women. In the first patient, signs of sarcoidosis included pleural, mediastinal node, and multiple joint involvement as well as nasal obstruction, sicca syndrome and subcutaneous nodules. Diagnosis was established on pleural and nasal biopsies. For the second patient, in addition to the pleural involvement, there was interstitial lung disease and cervical, epitrochleal and deep abdominal node enlargement. Diagnosis was established from serial bronchial biopsies, pleural bi

The Clinical Experience of Mycobacterial Culture Yield of Pleural Tissue by Pleuroscopic Pleural Biopsy among Tuberculous Pleurisy Patients.

Background and Objectives: Tuberculous pleurisy is a common extrapulmonary TB that poses a health threat. However, diagnosis of TB pleurisy is challenging because of the low positivity rate of pleural effusion mycobacterial culture and difficulty in retrieval of optimal pleural tissue. This study aimed to investigate the efficacy of mycobacterial culture from pleural tissue, obtained by forceps biopsy through medical pleuroscopy, in the diagnosis of TB pleurisy. Materials and Methods: This study retrospectively enrolled 68 TB pleurisy patients. Among them, 46 patients received semi-rigid pleur

Diagnostic value of adenosine deaminase in tuberculous pleural effusion: a meta-analysis.

BACKGROUND: Many studies have investigated the usefulness of adenosine deaminase activity (ADA) in pleural fluid for the early diagnosis of tuberculous pleurisy. To summarize the diagnostic characteristics of ADA we undertook a meta-analysis using a summary receiver operating characteristic (SROC) curve method. METHODS: Data sources were MEDLINE (1966-1999), the Cochrane Library and bibliographies of review and original articles. Studies were included if the absolute numbers of true positive, false negative, true negative and false positive observations were available or could be derived from
