Sonographic and postmortem findings of left visceral isomerism associated with polysplenia are reviewed. Particular stress is given to pathologic data, which could prospectively suggest some specific ultrasonic features in order to characterize this complex syndrome.

Two patients had amnioinfusion with subsequent improvement in fetal heart rate (FHR) patterns. Ultrasound in case 1 showed a pocket of saline around the umbilical cord and improved FHR. Case 2 likewise had an improvement in the FHR without ultrasonic evidence of a protective pocket of fluid. Case 2 actually had a tight true knot in the cord, which explained the abnormal FHR pattern. Amnioinfusion can form a protective pocket of fluid around the umbilical cord in cases of severe oligohydramnios, although in our experience its success rate is low and unpredictable.

Twenty-two patients with liver abscesses demonstrated by ultrasonography (US) were treated over a nine-year period. The diagnosis was in all cases verified by puncture. The patients were treated by US-guided puncture or catheter drainage. Seventeen patients (77%) were cured without surgical drainage. Four patients were cured after subsequent surgical intervention. One patient died later of pancreatic carcinoma. There were no complications from the US-guided therapy. For the treatment of liver abscesses we recommend US-guided drainage as the first choice. In 29 subjects with Gilbert's syndrome during ultrasonographic examination of the epigastrium in real time a significantly larger anteroposterior dimension of the right liver lobe (by cca 11%] was found. In the other investigated dimensions no significant deviations were found, not even a significantly higher echogeneity of the liver parenchyma. In two instances multiple cholecystolithiasis was detected. Sonography, a non-invasive method with a high yield, is an important link in the examination programme of subjects with Gilbert's syndrome.

The ultrasonographic examination of the fluid-filled stomach in five standardized positions permits the transabdominal visualization of the gastric wall in all sections of the organ. Ultrasonographic methods were used in the diagnosis of the criss-cross heart, a rare congenital rotation abnormality of the heart. As a routine preoperative evaluation for chronic sinusitis and postoperative maxillary cyst (mucocele and pyocele), CT scans and B-mode ultrasonic diagnosis including three methods, linear, sector, and convex scans, were performed. Pyogenic liver abscesses are not infrequent in some developing countries. Amoebic abscesses may also occur in endemic areas and differentiation may be difficult. The ultrasonic data of proven cases of both conditions were compared and we are now more more confident in predicting the aetiology of these lesions.

To minimize the complications arising from the subclavian venipuncture, ultrasonically (2-D Doppler) guided puncture was tried upon ten patients. Venipuncture was successful in nine patients whose subclavian vein was visualized by 2-D Doppler. In one patient, the subclavian vein had shifted cephalad, so the subclavian venipuncture was abandoned. Ultrasonically guided venipuncture is thought to be safer and a more sure method for subclavian venipuncture.
