Non-muscle invasive bladder cancer(NMIBC)accounts for about 70% of bladder cancers, with most patients treated by TURBT followed by intravesical therapy according to recurrence risk.BCG is effective but BCG-unresponsive cases often require cystectomy.Recently, novel therapies including immune checkpoint inhibitors, drug-delivery devices, and gene therapy have emerged.This chapter reviews recent and ongoing phase Ⅲ trials in BCG-naïve NMIBC.

There are several clinical scenarios that may not adhere to the precise definition of bacillus Calmette-Guérin (BCG)-unresponsive bladder cancer, but nevertheless indicate treatment unresponsiveness. We describe four such scenarios as a frame of reference in order to reach a consensus on the enrollment of such patients to current and future clinical trials in the BCG-unresponsive setting.

Bladder herpes is a very uncommon event related to herpes simplex virus infection. We report a 69-year-old man with 4-year history of bladder cancer who presented with dysuria and urinary frequency. Cystoscopy showed erythematous lesion of the bladder mucosa that was biopsied and revealed multilobed nuclei with eosinophilic intranuclear inclusions, and presence of intracytoplasmic vacuoles that were suggestive of bladder involvement by herpes simplex virus. Different bladder disorders have been associated with herpes virus, but spontaneous resolution generally occurs in 1-2 months.

The carcinogenicity of aflatoxin B1 (AFB1) has been under evaluation in nonhuman primates for the past 13 years. A total of 47 Old World monkeys, chiefly rhesus and cynomolgus, have received AFB1 i.p. (0.125 to 0.25 mg/kg) and/or p.o. We report on a man who had recurrent low grade noninvasive bladder tumors for 8 years and an increasing number of such tumors in both renal pelves and ureters for 3 years. In patients with inoperable cancer of the urinary bladder complicated by uretherohydronephrosis, chronic pyelonephritis, chronic renal failure, profuse bleedings from the tumor and pronounced dysuria, the supravesical derivation of urine is thought to be vitally indicated in spite of the unfavourable prognosis of the basic disease. First, it is necessary to provide the adequate drainage of the upper urinary ducts and kidneys followed by making favourable conditions for their longer and good functioning.

Nephrogenic adenoma of the bladder is a metaplastic lesion of the urothelium which is usually associated with infection or surgery. An additional case of nephrogenic adenoma that recurred one year following endoscopic treatment is described in a patient with a history of recurrent cystitis and hematuria. A review of the literature has shown that numerous cases have been reported in the past years. Thus, this disease entity is not as infrequent as it is considered to be.
