Forty-nine patients with chronic pyloroduodenal ulcer stenosis were treated surgically in our hospitals between 1977 and 1985. Three operations were used: 16 patients had a proximal gastric vagotomy-pyloroplasty (PGV-P); 26 had a vagotomy-antrectomy (V-A); and seven had a truncal vagotomy-pyloroplasty (TV-P). The influence of the proximal selective vagotomy (PSV) on the local blood flow in the gastric wall was investigated. No significant changes of the microcirculation were established in the mucosa of the corpus ventriculi (secretory area). The blood flow in the antrum pylorus was increased temporarily 24 hours after PSV and it was normalized again 4 and 7 days later. In advanced cases of esophagitis with acquired short esophagus and stricture, operative treatment to eliminate gastroesophageal reflux may be difficult or impossible. 24 such patients were treated by transthoracic truncal vagotomy and pyloroplasty. A case of avascular necrosis of the lesser curve of the stomach following a highly selective vagotomy (HSV) for a gastric ulcer is reported. It is fortunately rare, but is a frightening complication. The necrosis, which is presumably ischaemic in origin, is caused by total devascularization of the lesser curvature of the stomach. The complication is usually diagnosed late and has a considerable mortality.

Results of proximal vagotomy in combination of transverse gastroduodenoanastomosis (GDA) at 36 patients with sub- and decompensated ulcer pyloroduodenal stenosis are analyzed. The principles of surgical method choice depending on stenosis localization and motor-evacuatory function of stomach are discussed. Obtained results are compared with ones after selective proximal vagotomy with longitudinal GDA by Jaboule and pyloroplasty by Finney. Clinical, radiological, endoscopic and morphological data demonstrate the advantages of transverse GDA compared with longitudinal GDA and pyloroplasty.

Analysis of the results of operations on 271 patients for duodenal ulcer by four variants of selective proximal vagotomy showed that mobilization of merely the lesser curvature and the anterior surface of the distal end of the esophagus reduces acidity insufficiently (by 36.3%) and leads to a great number of recurrences (20%). Truncal subdiaphragmatic vagotomy with pyloroplasty was used in 28 patients with chronic relapsing pancreatitis, 19 of them were inclined to alcohol abuse. In 27 patients followed-up during 3 years, 18 were delivered from pain attacks resulting from chronic pancreatitis, the attacks became rarer and less intense in 4 patients, 3 patients had single attacks of pancreatic colic.
