People in states worse than dead according to the EQ-5D UK value set: would they rather be dead?

PURPOSE: Quality-adjusted life years (QALYs) measure health by combining length and quality of life. QALYs constitute the effect side of incremental cost-effectiveness ratios, describing the results of health economic evaluations. The objectives of this study were to (1) investigate the prevalence of states worse than dead (SWD) when using the EuroQol-5D UK value set, and (2) to study to what extent SWDs are reasonable with a starting point in experience-based valuations of health states. METHODS: Data from a Swedish cross-sectional population survey were used. The survey was directed to 10,0

Comparison of the Adult Strabismus Quality of Life Questionnaire (AS-20) with the Amblyopia and Strabismus Questionnaire (ASQE) among adults with strabismus who seek medical care in China.

BACKGROUND: The impact of strabismus on visual function, self-image, self-esteem, and social interactions might decrease health-related quality of life (HRQoL). This study aimed to evaluate the psychometric properties and clinical applications of two strabismus-specific HRQoL questionnaires in the cultural context of China. METHODS: The Chinese versions of the Adult Strabismus Quality of Life Questionnaire (AS-20) and the Amblyopia and Strabismus Questionnaire (ASQE) were self-administered to 304 adults with strabismus. The Cronbach's α coefficient was calculated to assess the internal consis

Longitudinal Echocardiographic Evaluation of Pulmonary Hypertension Trajectories in Infants With Bronchopulmonary Dysplasia.

PURPOSE: Describe longitudinal echocardiographic changes during the first year of life in infants with bronchopulmonary dysplasia (BPD)-associated pulmonary hypertension (PH) and assess the prognostic value of early echocardiographic parameters. METHODS: This study was a prospective longitudinal cohort study. Preterm infants with BPD underwent echocardiographic screening at 28 days of life and, if diagnosed with PH, were followed every 3 months until 12 months or death. RESULTS: During the first year of life, 40.9% of infants died, 52.3% survived with complete resolution of PH, and 6.8% surv

DNA methylation under the major depression pathway predicts pediatric quality of life four-month post-pediatric mild traumatic brain injury.

BACKGROUND: Major depression has been recognized as the most commonly diagnosed psychiatric complication of mild traumatic brain injury (mTBI). Moreover, major depression is associated with poor outcomes following mTBI; however, the underlying biological mechanisms of this are largely unknown. Recently, genomic and epigenetic factors have been increasingly implicated in the recovery following TBI. RESULTS: This study leveraged DNA methylation within the major depression pathway, along with demographic and behavior measures (features used in the clinical model) to predict post-concussive sympt

Codon optimality predicts mRNA half-life but does not transfer to lncRNAs.

Codon optimality couples translation elongation to decay and is among the strongest reported sequence determinants of mRNA half-life. Long non-coding RNAs (lncRNAs) are largely untranslated. Is lncRNA stability sequence-encoded in the way mRNA stability is? We compared the classes on half-lives from the same assays, re-estimating the codon stabilization coefficient (CSC) inside each cross-validation fold to remove label leakage. Performance is the cross-validated coefficient of determination R² - 1 for perfect prediction, 0 for the training mean, negative for worse - and the Spearman rank corr

Pre-cachexia and cachexia at diagnosis of stage III non-small-cell lung carcinoma: an exploratory study comparing two consensus-based frameworks.

Despite the development of consensus-based frameworks to define cancer cachexia, the validity and usefulness of these frameworks are relatively unknown. The aim of the present study was to study the presence of pre-cachexia and cachexia in patients with stage III nonsmall-cell lung carcinoma (NSCLC) by using a cancer-specific framework and a general framework for cachexia, and to explore the prognostic value of pre-cachexia and cachexia. In forty patients at diagnosis of stage III NSCLC, weight loss, fat-free mass, handgrip strength, anorexia and serum biochemistry, assessed before the first c

Quantifying the costs of unintentional and intentional drowning in the United Kingdom: A secondary data analysis study.

OBJECTIVES: Drowning is a significant cause of intentional and unintentional fatality in many countries, particularly in young people, but is largely preventable. Measuring Years of Life Lost (YLL) enables the economic Value of a Life Year (VOLY) to be calculated to understand disease burden. We established the YLL and VOLY for drowning (incl. water-related fatalities) in the UK, and the nations comprising the UK (i.e., England, Scotland, Wales and Northern Ireland) to quantify the benefits of prevention and policy formation. STUDY DESIGN: Cross-sectional. METHODS: Data were retrieved from t

Is the systematic use of mapping systems during His Bundle catheter ablation cost-effective? A single-center experience.

AIM: Three-dimensional (3D) nonfluoroscopic mapping systems (NMSs) are generally used during the catheter ablation (CA) of complex arrhythmias. We evaluated the efficacy, safety, and economic advantages of using NMSs during His-Bundle CA (HB-CA). METHODS: A total of 124 consecutive patients underwent HB-CA between 2012 and 2019 in our EP Laboratory. We compared two groups: 63 patients who underwent HB-CA with fluoroscopy alone from 2012 to 2015 (Group I) and 61 patients who underwent HB-CA with the aid of NMSs from 2016 to 2019 (Group II). Two cost-effectiveness analyses were carried out: the

Development of the SAVE study protocol to use lived experience to assess the value of serious health-related suffering and palliative care.

BACKGROUND: Suffering generated by life-limiting or life-threatening conditions is a complex phenomenon inextricably linked with human experience. Most of this suffering can be ameliorated with adequate access to palliative care that is unavailable in much of the world, especially low-resource settings. Exploration of the definitional boundaries of and needs pertaining to suffering due to illness suggests that patients and families assign specific importance to the compassionate and supportive aspects of palliative care. However, the intrinsic and personal value of suffering alleviation has no

Body Composition of Filipino Chronic Obstructive Pulmonary Disease (COPD) Patients in Relation to Their Lung Function, Exercise Capacity and Quality of Life.

BACKGROUND AND OBJECTIVES: The loss of muscle or fat free mass (FFM) as a result of systemic inflammation and poor nutrition in Chronic Obstructive Pulmonary Disease (COPD), is recognized as an important factor that influences symptoms and disease-related outcomes. To date, there are no data on body composition among Filipino COPD patients and how it impacts COPD disease severity. This paper examined the relationship of Fat Free Mass Index (FFMI = FFM/height) and sarcopenia with COPD disease severity variables. METHODS: This was a cross-sectional analytic study comparing low and normal FFMI,

Health state utility estimates for value assessments of novel treatments in Huntington's disease: a systematic literature review.

BACKGROUND: Huntington's disease (HD) is a progressive neurodegenerative disease with a devastating impact on patients and their families. Quantifying how treatments affect patient outcomes is critical for informing reimbursement decisions. Many countries mandate a formal value assessment in which the treatment benefit is measured as quality-adjusted life-years, calculated with the use of utility estimates that reflect respondents' preferences for health states. OBJECTIVE: To summarize published health state utility data in HD and identify gaps and uncertainties in the data available that cou

Severe Junctional Epidermolysis Bullosa: Honoring Parental Values in Pediatric Palliative Decision-Making.

Severe junctional epidermolysis bullosa (JEB) is a rare, incurable, autosomal recessive blistering disorder with a uniformly poor prognosis. We present the case of a female infant diagnosed shortly after birth with genetically confirmed severe JEB, whose family prioritized comfort-focused care and memory-making over life-prolonging interventions. Her interdisciplinary care centered on symptom relief, bonding, and alignment with family values. This case underscores the importance of early diagnosis and values-based decision-making in managing rare, life-limiting conditions like severe JEB.

Person-Centered Care at the End of Life: Challenging Ableism in Conversations About What Matters Most.

Clinicians often struggle with asking patients with progressive, life-limiting illnesses about the point at which pursuing longevity no longer aligns with their values. While person-centered care encourages exploring patients' goals and fears, directly asking individuals to identify conditions "worse than death" can inadvertently reinforce ableist assumptions that disability diminishes quality of life. Yet avoiding these discussions risks neglecting patient autonomy and failing to align treatment decisions with what matters most. This Perspective examines the ethical tensions clinicians face w
