Advances in Functional Vascular Stents for Cardiovascular Therapy with Drug Delivery and Computational Design.

Vascular stents are crucial devices in the treatment of cardiovascular diseases, and their structural design and function critically affect therapeutic efficacy and patient prognosis. Conventional stents can effectively restore vascular patency by providing mechanical support to blood vessels. However, they still face significant challenges including restenosis, thrombosis, and limited adaptability to complex patient-specific lesion characteristics. To address these limitations, drug delivery offers an important strategy to modulate the pathological microenvironment, enhance long-term vascular…

[Multiple sequence revascularization of infrapopliteal arteries in the management of critically ischemic extremity].

AIM OF THE STUDY: Most cases of distal bypasses closures are caused by reduced blood flow within the affected peripheral vasculature, resulting in corresponding reduction in the graft's blood supply. The authors use multiple, sequence, Y grafting and bridge grafting procedures on crural and pedal arteries, in order to improve the above hemodynamic features. Furthermore, the techniques facilitate better circulation within larger extremity regions. METHODS: From April 2007 to January 2009, the authors completed a total of 38 sequence bypass procedures. The procedures included 30 Y graft procedu

Stability and function of adult vasculature is sustained by Akt/Jagged1 signalling axis in endothelium.

The signalling pathways operational in quiescent, post-development vasculature remain enigmatic. Here we show that unlike neovascularization … ts, this causes a loss of arteries and arterioles and, despite a high capillary density, diminished vascular patency and severe cardiac dysfunction. Similarly, endothelial Akt deletion induces retinal VSMC loss and basement membrane deterioration resulting in vascular regression and retinal atrophy. Mechanistically, the Akt/mTOR axis controls endothelial Jagged1 e …

Restoring Access to Kidney Transplantation: Midterm Outcomes of Pre-Emptive Aortobifemoral Bypass in Candidates with Severe Aortoiliac Disease.

BACKGROUND: Severe aortoiliac calcification and occlusive disease may preclude safe arterial clamping and anastomosis during kidney transplantation (KT), particularly as KT candidates age and accumulate comorbidities such as diabetes and peripheral arterial disease (PAD). Pre-emptive aortobifemoral bypass (ABFB) can restore durable inflow; however, multicenter data describing subsequent transplant access and outcomes stratified by symptom status remain limited. METHODS: This retrospective multicenter cohort study was conducted across 12 French and 1 Belgian center between 1984 and 2017. Patie

Outcomes following ipsilateral great saphenous vein bypass for lower extremity arterial injuries.

OBJECTIVE: Use of autologous great saphenous vein (GSV) grafts for repair of extremity arterial injuries is well established. Contralateral great saphenous vein (cGSV) is traditionally used in the setting of lower extremity vascular injury given the risk of occult ipsilateral superficial and deep venous injury. We evaluated outcomes of ipsilateral GSV (iGSV) bypass in patients with lower extremity vascular trauma. METHODS: Patient records at an ACS verified Level I urban trauma center between 2001 and 2019 were retrospectively reviewed. Patients who sustained lower extremity arterial injuries

Keeping NO in No-Touch Saphenous Vein Bypass Grafts.

The saphenous vein is the most commonly used conduit in patients undergoing coronary artery bypass surgery. Graft patency is improved using the no-touch technique where the vein is harvested with minimal trauma, avoiding high pressure distension and maintaining normal vessel architecture. Various cells that are damaged when using conventional harvesting are preserved using the no-touch technique and are a source of nitric oxide. The no-touch technique is becoming accepted more widely as is the role of nitric oxide in improved saphenous vein graft patency. However, there are conflicting views r

Use of the 11-Item Modified Frailty Index to Determine Accurate Prognosis and Revascularization Strategy in Patients Undergoing Vascular Bypass Surgery for Intermittent Claudication.

BACKGROUND: In cases of intermittent claudication (IC) where traditionally noninvasive management yields unsatisfactory results, revascularization strategy in IC patients is generally decided based on anatomical considerations and the availability of a saphenous vein graft. Life expectancy should also be considered. This study aimed to investigate the relationship between the 11-item modified frailty index (mFI-11) and the overall survival (OS) in patients with IC who underwent vascular bypass surgery to facilitate revascularization strategy selection. METHODS: We reviewed the records of 144

An Infected Popliteal Aneurysm after Plain Old Balloon Angioplasty.

The case was a 76-year-old man with chronic limb-threatening ischemia. Plain old balloon angioplasty (POBA) was performed on the popliteal artery. Subsequently, he suffered from cellulitis around the POBA site, followed by reocclusion. Staphylococcus aureus was detected in a blood culture. After re-revascularization with POBA, both purulent gonitis and an infected popliteal aneurysm were observed to occur. We performed aneurysmectomy and bypass grafting with the saphenous vein and then continued antibiotic therapy. Although treatment consisted of endovascular therapy (EVT) with nothing left be

[Stenting of calcified iliofemoral venous segment in post-thrombotic syndrome].

Stenting of the iliac veins in patients with post-thrombotic syndrome is an intervention associated with a low risk of complications, high primary and secondary patency rates, low incidence of restenosis, leading to a significant decrease in the severity of symptoms of chronic venous disease and a high rate of trophic ulcer healing, as compared with conservative therapy. Unlike subcutaneous veins, the formation of calcinates in deep veins after endured thrombosis is of considerably less frequent occurrence. Described in the article is a clinical case report concerning successful stenting of th

[Antithrombotic therapy after venous stenting].

Presented herein is a literature review considering the problems of using antithrombotic therapy after venous stenting. Described herein are the literature data according to which the authors give preference to anticoagulant therapy (low-molecular-weight heparins, vitamin K antagonists, direct oral anticoagulants). This is followed by considering the problems of duration of treatment depending on various clinical situations. According to the presented data, the problem of prescribing disaggregants in a combination with anticoagulants after stenting of veins remains disputable, finding however

Adjuncts to improve patency of infrainguinal prosthetic bypass grafts.

The superiority of autologous venous conduit for infrainguinal arterial bypass has been well documented, especially when the bypass extends below the knee or to the tibial arteries. Nevertheless, when adequate autologous vein is not available, prosthetic bypasses (eg, polytetrafluoroethylene [PTFE] or Dacron) are often used in limb-salvage situations. The inferior long-term patency of these infrainguinal prosthetic bypasses has been documented by a number of studies. One such study reported a prospective multicenter randomized trial with only a 12% primary patency for infrapopliteal prosthetic
