BACKGROUND: Refractory and stable defects of vitiligo and piebaldism may be unresponsive to medical therapy. Melanocyte transplantation can restore the normal pigmentation in some selected patients.
OBJECTIVES: To evaluate the efficacy of additional mini-grafting with 1.0-1.2-mm punch grafts to complete the restoration of achromic defects when performing surgical correction of leukoderma.
METHODS: Eight patients with refractory stable leukoderma were treated with melanocyte transplantation; three with segmental vitiligo had epidermal shave, by removing the hyperpigmented macules at the perip…

During the topical treatment of 45 patients, who had extensive forms of alopecia areata, with the allergen diphencyprone, 3 of them (6.7%) developed vitiligo. Two were females and 1 male aged 53, 19 and 28 years respectively. None of these patients had a personal or family history of vitiligo. Vitiligo appeared 3-5 months after the onset of treatment and was localized only to the areas of topical application in the younger woman and the man. In the older woman, vitiligo extended to several areas apart from those where the medicament was applied. After the end of diphencyprone treatment, vitili…

A clinical analysis of L50 patients having vitiligo revealed the incidence among new patients to be 1.25%. The male /female ratio was 1.3:1. Patients below the age of 30 years constituted 85%. Family history of vitiligo was available in 240/o. The different morphologic, Patterns consisted of vitiligo vulgaris (90 cases), vitiligo areata (22 cases), vitiligo acro-facialis (11 cases), vitiligo univers (11 cases , vitiligo mucoaae (9 cases),and vitiligo zosteriformis (7 cases). Association with diabetes meffitus (5 cases), alopecia areata (4 cases), Hashimoto's thyroiditis (1 case) and pemphigus …

The administration of phenylalanine (Phe) combined with UVA exposure was found to be effective in vitiligo. Phe is an amino acid which constitutes part of the daily dietary protein, and when orally administered in a dose of 50 mg/kg body weight, it results in an elevated plasma level. Since peak concentrations of Phe in the blood are reached between 30 and 45 min after ingestion, UVA exposure was administered at this time. After 4 months (32 treatments) reasonable repigmentation preferentially occurred in the skin area of subcutaneous fat (adipose tissue). Apart from the repigmentation of hypo…

A case of tuberculoid leprosy with complete depigmentation on its active margins, in a predisposed individual to vitiligo is presented. The postulated mechanism of hypopigmentation vis-a-vis leprosy lesions is discussed in brief in the context of the presenting unusual association.

The association of vitiligo with inflammation of the uveal tract is well established. The relationship between vitiligo and hypopigmentation and/or degeneration of the retinal pigment epithelium (RPE) not secondary to ocular inflammation has not been adequately investigated. Sixty (27%) of 223 consecutive patients with vitiligo were found to have some evidence of RPE hypopigmentation ranging from mild, focal areas of involvement in most cases to extensive RPE degeneration with a retinitis pigmentosa-like syndrome in one patient. Fifteen (25%) patients complained of night blindness. Only 6 (4%)…

The efficacy of topical 8-methoxypsoralen (8-MOP) in varying concentrations and vehicles was assessed in 73 vitiligo patients. The response rates in different anatomic sites were also assessed. Seven patients (9%) had 100% repigmentation; 26 (36%) had 50% or greater repigmentation, 29 (40%) had some degree of pigment return, but less than 50%; 11 (15%) had no repigmentation. Results suggest that neither concentration of drug nor vehicle is a crucial factor for inducing repigmentation of vitiliginous patches on the face, trunk, and extremities. Low-dose 8-MOP (0.1%) was as effective as high dos…

Vitiligo is a disease of cutaneous pigment cells. Recently we reported that one third of patients with vitiligo had destructive lesions in the retinal pigment epithelium and/or the choroid which were visible by ophthalmologic examination. To determine whether abnormalities of the pigment system of the eye and skin are in fact associated, we examined for vitiligo the skin and hair of 107 patients with active uveitis. Thirty-four patients had an infectious or a known cause for their uveitis and none of these had vitiligo. In contrast, six of seventy-three patients (8%) with idiopathic uveitis ha…
