Spontaneous resolution of a postvitrectomy macular hole retinal detachment.

PURPOSE: The purpose of this report was to describe a case of spontaneous resolution of a large postvitrectomy macular hole retinal detachment. METHODS: Case report and optical coherence tomography imaging. RESULTS: A 64-year-old man with history of macula-off retinal detachment and 4 previous vitrectomies in the left eye developed a macular hole and associated retinal detachment 3 months after his last vitreoretinal surgery. Two months later, examination revealed that the macular hole had spontaneously closed, and the retinal detachment had resolved. CONCLUSION: Spontaneous resolution of m…

Pathophysiology and treatment of diabetic retinopathy.

In the past years, the management of diabetic retinopathy (DR) relied primarily on a good systemic control of diabetes mellitus, and as soon as the severity of the vascular lesions required further treatment, laser photocoagulation or vitreoretinal surgery was done to the patient. Currently, even if the intensive metabolic control is still mandatory, a variety of different clinical strategies could be offered to the patient. The recent advances in understanding the complex pathophysiology of DR allowed the physician to identify many cell types involved in the pathogenesis of DR and thus to dev…

Children with blindness due to retinopathy of prematurity: a population-based study. Perinatal data, neurological and ophthalmological outcome.

A population-based group of 27 children with total blindness due to retinopathy of prematurity (ROP), born in Sweden from 1980 to 1990, was examined. They constituted all but two of the total of 29 children with total blindness due to ROP known to the national register of visually impaired children when reviewed from 1980 to 1 January 1995. All children had a gestational age of less than 31 weeks and most had had a complicated perinatal period. The retinal disease was discovered late, most often after it had already progressed to bilateral retinal detachment. Repeated vitreoretinal surgery had…

Advancing perspectives on proliferative vitreoretinopathy: Updates in experimental research and emerging clinical treatments.

Proliferative vitreoretinopathy (PVR) remains a major barrier in vitreoretinal surgery, being the leading cause of recurrent retinal detachment and surgical failure. PVR arises from abnormal wound healing, involving epithelial-mesenchymal transition (EMT) of retinal pigment epithelial cells, hyalocytes trans-differentiation into myofibroblasts, and dysregulated interactions among cytokines, growth factors, and chemokines. These lead to extracellular matrix remodeling and contractile membrane formation. Recent advancements have deepened insight into disease mechanisms and supported preclinical …

Choroidal neovascularization after laser-assisted in situ keratomileusis following penetrating keratoplasty.

PURPOSE: To describe a case of choroidal neovascularization (CNV) after laser in situ keratomileusis (LASIK) following penetrating keratoplasty (PK). METHODS: Case report. RESULTS: A 30-year-old man underwent PK in both eyes for bilateral keratoconus in 1997. Two years later, best-corrected visual acuity (BCVA) was 20/40 (-6=-4.50 x 170 degrees ) in RE and 20/20 (-1.50=-0.50 x 90 degrees ) in LE. To reduce the anisometropic defect, LASIK was performed in RE. After surgery, the refractive defect in RE reduced to -1.75 x 125 degrees and BCVA improved to 20/25. Six months after LASIK the patien

Importance of individualized treatment: Subconjunctival device in silicone oil-induced glaucoma.

Although silicone oil is used as an intraocular buffer in vitreoretinal surgical procedures, its use can cause complications, including glaucoma. This study highlights the importance of individualized approaches for the management of silicone oil-induced glaucoma. A 62-year-old man with a past medical history of retinal detachment and multiple ocular surgical procedurespresented with uncontrolled ocular pressure peaks after retinal detachment surgery using silicone oil as a buffer, which did not resolve after removal of the buffer. We decided to perform anterior chamber lavage followed by the

[Extraocular silicone oil migration to orbit: a case report].

Silicone oil insertion is a common modality in vitreoretinal surgeries after ocular perforating injury. It needs surgical removal after several months. Extraocular silicone oil migration to orbit and eyelid is uncommon complication. We reports a case of silicone oil migration to orbit and eyelid. A 41-year-old male presented with left upper eyelid swelling following silicone oil injection 3 years before admission. Orbital computed tomography scan showed multiple high-density well-defined masses in vitreous, left upper eyelid and orbit. We diagnosed the patient with orbital and eyelid mass, mea

[Ultrasound biomicroscopy in evaluation of conditions for secondary intraocular lens implantation in aphakia].

In order to evaluate conditions for secondary intraocular lens (IOL) implantation detailed analysis of ultrasound biomicroscopy (UBM) images of 26 patients (33 eyes) with aphakia was performed. The etiology of cataract was congenital (12 eyes), traumatic (9 eyes), senile (8 eyes) and uveal (4 eyes). The results showed that UBM provides an opportunity to evaluate the implantation site considering capsule integrity, configuration and parameters of ciliary line. The authors have demonstrated that UBM is scarcely the only visualization method in aphakic eyes that lets find and evaluate structural

Retinal inflammation diagnosed as an idiopathic macular hole with multiple recurrences and spontaneous closures: A case report.

RATIONALE: An idiopathic macular hole that causes substantial reduction in central visual acuity is believed to involve no obvious underlying diseases; thus, it is suspected to form due to the presence of idiopathic tractional forces at the vitreoretinal interface. Importantly, it is effectively treated with pars plana vitrectomy (PPV), which removes the mechanical forces. However, while it is exceedingly rare, a macular hole can develop in eyes after PPV; fresh or postoperative macular holes can close spontaneously without surgical removal of traction. Thus, another mechanism might be involve

In vitro silicone oil emulsification in retinal detachment treatment: Methods, designs, and future strategies.

Silicone oils are widely used as intraocular tamponade agents in vitreoretinal surgery, particularly in the treatment of complex retinal detachments and other severe posterior segment pathologies such as proliferative diabetic retinopathy, macular holes, ocular trauma, or endophthalmitis. While they offer mechanical support for retinal reattachment, their prolonged intraocular presence is associated with the risk of emulsification, i.e., a process in which the oil phase breaks into subvisible droplets that can obstruct aqueous outflow and compromise visual outcomes. Although emulsification is …

Approach to Ophthalmic Surgery Involving the Anterior Vitreous: A Clinical Practice Update from the American Society of Retina Specialists.

When encountering diseases of the anterior vitreous and peripheral retina (ie, the "middle segment" of the eye), vitreoretinal (VR) surgeons are specifically trained in surgical techniques for managing the vitreous and visualizing the peripheral retina during anterior vitrectomy, and therefore, they are well equipped to address the complications of surgery involving the anterior vitreous space. This clinical update reviews and compares the current literature discussing techniques and patient outcomes related to complete pars plana vitrectomy versus incomplete, subtotal pars plana vitrectomy, a
