Identifying spatiotemporal patterns in opioid vulnerability: investigating the links between disability, prescription opioids and opioid-related mortality.

BACKGROUND: The opioid crisis remains one of the most daunting and complex public health problems in the United States. This study investigates the national epidemic by analyzing vulnerability profiles of three key factors: opioid-related mortality rates, opioid prescription dispensing rates, and disability rank ordered rates. METHODS: This study utilizes county level data, spanning the years 2014 through 2020, on the rates of opioid-related mortality, opioid prescription dispensing, and disability. To successfully estimate and predict trends in these opioid-related factors, we augment the Ka

Vulnerability Index Approach to Identify Pharmacy Deserts and Keystone Pharmacies.

IMPORTANCE: Community pharmacies are crucial for public health, providing essential services such as medication dispensing, vaccinations, and point-of-care testing. Addressing disparities in pharmacy access, particularly in underserved rural and low-income areas, is critical for health equity. OBJECTIVE: To identify areas in the US at risk of becoming pharmacy deserts through the development of a novel pharmacy vulnerability index. DESIGN, SETTING, AND PARTICIPANTS: This population-based cross-sectional study in the contiguous 48 states performed geographic information systems analysis of ph

Re: "Severe angiostrongyliasis with neuropsychiatric symptoms in vulnerable adults: Early diagnosis via next-generation sequencing and successful treatment".

Tang et al. (2025) report on two cases of neuroangiostrongyliasis in vulnerable adults, diagnosed via metagenomic next-generation sequencing (mNGS). We highlight the clinical value of mNGS in detecting atypical presentations, particularly in patients without eosinophilia or classic meningitic signs. While mNGS demonstrates diagnostic potential, its limitations and feasibility in low-resource settings must be acknowledged. We propose a contextualized approach integrating mNGS with cost-effective tools like LAMP and rapid antigen tests, especially for underserved populations. The commentary also

"O for KP": an evidence review on recentering HIV prevention and PrEP delivery for key populations and the underserved.

PURPOSE OF REVIEW: Despite the proven efficacy of preexposure prophylaxis (PrEP), global coverage remains far below targets, with the greatest gaps among key and underserved populations. This review is timely as it highlights how inequitable access undermines progress toward ending AIDS by 2030 and introduces the "O for KP" framework - a preferential option for key populations and the underserved . This equity-focused principle redefines program success by how well services reach and sustain those most structurally excluded. RECENT FINDINGS: We synthesized evidence on persistent barriers face

Parental Substance Use Disorder and the Rise in Children Being Raised by Grandparents: Focusing on Children's Needs and Improving Outcomes.

The occurrence of children being raised solely by their grandparents has steadily risen in the past decade prompted by parental substance use disorder (SUD) and associated incarceration and death. These families, especially children, are a vulnerable population with unique needs. Nurses should be cognizant of the needs, risk factors, and protective factors often associated with this population. Meanwhile, the literature to date mostly focuses on grandparents. School and pediatric nurses, specifically, are in an advantageous position to help support these families and help foster better child o

Delayed Diagnostic Paracentesis Is Associated with Increased Preventable Healthcare Utilization in Disadvantaged Patient Populations with Advanced Liver Disease and Elevated INR.

BACKGROUND: Patients hospitalized with cirrhosis, ascites, and elevated INR often experience delays in timely diagnostic paracentesis. AIMS: Identify whether delays in diagnostic paracentesis were associated with adverse outcomes in a hospital system serving a large disadvantaged population. METHODS: Retrospective cohort analysis of patients admitted from January 2017 to October 2019 with cirrhosis, ascites, and INR ≥ 1.5 across a multi-hospital health system in central Texas. We examined demographic and clinical characteristics of patients with diagnostic paracentesis (1) ≤ 24 h; (2) > 24 h

Clinically significant substance use and residential stability among homeless or vulnerably housed persons in Canada: a longitudinal cohort study.

BACKGROUND: We examined clinically significant substance use among homeless or vulnerably housed persons in three Canadian cities and its association with residential stability over time using data from the Health and Housing in Transition study. METHODS: In 2009, 1190 homeless or vulnerably housed individuals were recruited in three Canadian cities and followed for 4 years. We collected information on housing and incarceration history, drug and alcohol use, having a primary care provider at baseline and annually for 4 years. Participants who screened positive for substance use at baseline we

The strain of sons' incarceration on mothers' health.

Research on disadvantage across generations typically focuses on the resources that parents pass on to their children. Yet, social disadvantage might also result from the transmission of adverse experiences from children to their parents. This paper explores one such adverse experience by examining the influence of a son's incarceration on his mother's health. Using panel data from the 1979 National Longitudinal Survey of Youth and its young adult follow up (n = 2651 mothers; 18,390 observations), the paper shows that mothers are more likely to suffer health limitations after a son is incarcer

The Washtenaw ID Project: A Government-Issued ID Coalition Working Toward Social, Economic, and Racial Justice and Health Equity.

Introduction. The policing of identities through policies that restrict access to IDs issued by U.S. governmental entities disparately affects communities of color; communities who identify as low-income, immigrant, older, and/or transgender; and community members who experience chronic mental illness, housing instability, or incarceration. Yet government-issued IDs are increasingly needed to access health-promoting resources such as housing, banking, social services, and health care, and in interactions with law enforcement. Methods. Since 2012, the Washtenaw ID Project's coalition-building p

Understanding Wait Times in Rapid Re-Housing Among Homeless Youth: A Competing Risk Survival Analysis.

Approximately 3.5 million youth experience homelessness in the United States. Assisting youth to exit homelessness as quickly as possible through various housing venues aims to prevent adverse health impacts that prolonged homeless experiences may have on youth. Rapid re-housing (RRH) is a recent, short-term, less costly housing option than permanent supportive housing that provides temporary housing supports and services to counter homelessness. Although previous literature indicates that youth are likely to achieve stable homelessness exits via RRH, the duration of wait times for youth and p

[Softly-softly].

The missions of the mobile psychiatric team in Lyon involve reaching out to vulnerable people to guide them towards the most appropriate care. Establishing a bond and finding the right words to engage with them constitute a balancing act which the caregiver must undertake with the homeless. It is important to try and understand the situation, the person's life story, invent alternatives, while refraining from pushing them too much, in order to maintain their trust.

Substance Use Issues Among the Underserved: United States and International Perspectives.

Substance use affects people of all ages, cultures, and socioeconomic levels. Most underserved populations have lower rates of substance use than the general population in a given society, excluding tobacco use. The impact of substance use is more severe, however, in the underserved, with higher rates of incarceration, job loss, morbidity, and mortality. Innovative solutions are being developed to address these differences. Working together, underserved patients with substance use problems can be helped on their journeys toward health and wholeness.

Respiratory vulnerability to vehicle buffeting.

OBJECTIVE: Buffeting in a jerky ride in a bus or ambulance normally provokes a sustained tachypnoea driven by vibration and sensory mechanisms including vestibular signals. Tachypnoea reinforces the torso against mechanical shocks but results in overbreathing, causing a mild fall in CO(2). However, normal CO(2) is rapidly restored by a reduction in depth of breathing. We test the hypothesis that vulnerable subjects, exemplified by elderly individuals and patients with vestibular disorders, may fail to adapt to buffeting. METHODS: Respiratory and cardiovascular functions were recorded from fiv

A review of disparities in peripheral artery disease and diabetes-related amputations during the COVID-19 pandemic.

The COVID-19 pandemic has profoundly affected health care delivery. In addition to the significant morbidity and mortality associated with acute illness from COVID-19, the indirect impact has been far-reaching, including substantial disruptions in chronic disease care. As a result of pandemic disruptions in health care, vulnerable and minority populations have faced health inequalities. The aim of this review was to investigate how the COVID-19 pandemic has impacted vulnerable populations with limb-threatening peripheral artery disease and diabetic foot infections.

Do Geographic Barriers and Community Vulnerability Impact PrEP Use Among Transgender Latinas in the US South? Findings from the ChiCAS Study.

Transgender (trans) Latinas in the United States face disproportionately higher HIV rates and persistently low use of pre-exposure prophylaxis (PrEP) despite awareness. While geographic access to PrEP has been identified as a barrier in other populations, little is known about how spatial access and community-level vulnerability shape PrEP use among trans Latinas in the US South. Using data from the randomized intervention-waitlist control trial of ChiCAS (N = 144), we examined (1) proximity to Spanish-language PrEP providers, (2) neighborhood vulnerability using the Social Vulnerability Index
