Caffeine (5 mg kg-1) was administered orally to two healthy, non-smoking subjects on three separate occasions--before, and during therapy with the xanthine oxidase inhibitor allopurinol at doses of either 300 or 600 mg daily. In a prospective study of 328 high-risk neonates, we used computer-assisted logistic regression of 47 potential risk factors to study acute retinopathy of prematurity. Only four factors were significant in the logistic regression equation: ventilator hours, xanthine administration, birthweight, and maternal bleeding. Xanthine administration was identified as a new independent and significant (P less than .0001) predictor of acute retinopathy of prematurity.

The theory that consumption of homogenized milk containing active xanthine oxidase is a causative factor in development of atherosclerosis is reviewed. Biologically available xanthine oxidase in consumed milk products may be absorbed in the small intestine and enter the blood stream. However, there appears to be no unequivocal evidence that the absorbed enzyme has any pathological effects that may contribute to development of atherosclerotic heart disease.

Chromatographic characteristics of urinary metabolites of theophylline were studied by two-dimensional thin-layer chromatography, high-performance liquid chromatography and gas chromatography--mass spectrometry. Because polymorphonuclear leukocyte (PMN) activation is associated with generation of potent inflammatory mediators, xanthine effects on the PMN respiratory burst were studied.
