Local Anaesthetic Effect of Methanolic Leaves Extract of Lannea schimperi (Hoschst. Ex Rich) Eng.

BACKGROUND: Local anaesthetics provide relief from pain when applied locally to nerve tissue by blocking conduction of sensory nerve impulse from the receptor to the brain cortex. OBJECTIVE: This study aimed at evaluating local anaesthetic activity of the methanolic leaves extract of Lannea schimperi. METHODS: Six groups of five animals were used; groups I-IV were used for intracutaneous wheal test in guinea pigs for infiltration anaesthesia, while group V and VI were used for guinea pig corneal reflex method of surface anaesthesia. RESULTS: The result indicated a significant ( 0.05) dose d

The application of epidural anesthesıa in pregnant woman with uncorrected tetralogy of Fallot: a case report.

Tetralogy of Fallot is the most common cyanotic congenital cardiac disease. The hemodynamic changes which are present immediately after abdominal delivery may be resulted with serious, life-threatening multiorgan complications. Therefore the choice of anesthesia is very difficult. We describe the case of pregnant, 33 weeks gestation, with uncorrected tetralogy of Fallot who underwent caesarian section. The epidural anesthesia was performed uneventfully. After having stabile vital signs and a good general condition patient was followed up at the intensive care unit for 4 days. She was transferr

Liquid-injection for preperitoneal dissection of transabdominal preperitoneal (TAPP) inguinal [corrected] hernia repair.

BACKGROUND: The creation of an adequate peritoneal flap during laparoscopic transabdominal preperitoneal (TAPP) inguinal hernia repair, while avoiding injuring surrounding structures can be technically challenging. Liquid infiltration of the preperitoneal space can help facilitate dissection and avoid inadvertent injuries. We describe a novel technique for TAPP inguinal hernia repair using liquid-injection for preperitoneal [corrected] dissection and report our initial experience. METHODS: TAPP inguinal hernia repair using a liquid-injection technique during preperitoneal dissection was perfo

Pharmacological agents for preventing morbidity associated with the haemodynamic response to tracheal intubation.

BACKGROUND: Several drugs have been used in attenuating or obliterating the response associated with laryngoscopy and tracheal intubation. These changes are of little concern in relatively healthy patients but can lead to morbidity and mortality in the high risk patient population. OBJECTIVES: The primary objective of this review was to determine the effectiveness of pharmacological agents in preventing the morbidity and mortality resulting from the haemodynamic changes in response to laryngoscopy and tracheal intubation in adult patients aged 18 years and above who were undergoing elective s

Regional anaesthesia and cancer metastases: the implication of local anaesthetics.

Clinical and basic science studies have demonstrated the anti-inflammatory properties of local anaesthetics. Recent studies have begun to unravel molecular pathways linking inflammation and cancer. Regional anaesthesia is associated in some retrospective clinical studies with reduced risk of metastasis and increased long-term survival. The potential beneficial effects of regional anaesthesia have been attributed mainly to the inhibition of the neuroendocrine stress response to surgery and to the reduction in the requirements of volatile anaesthetics and opioids. Because cancer is linked to inf

Continuous infusion of bupivacaine following total knee arthroplasty: a randomized control trial pilot study.

An RCT pilot-study was conducted to assess efficacy of a 48-h continuous local infiltration of intra-articular bupivacaine (0.5% at 2 cc/h) versus placebo (0.5% saline at 2 cc/h) in decreasing PCA morphine consumption following TKA. Secondary outcomes included 48-h VAS pain, opioid side effects, length of stay, and knee function scores up to 1-year postoperatively. Of 67 randomized patients, 49 completed the trial including 24 bupivacaine, and 25 placebo patients. Mean 48-h PCA morphine consumption did not differ significantly between treatment (39 mg ± 27.1) and placebo groups (53 mg ± 30.4)

Efficacy of epidural lidocaine combined with tramadol or neostigmine on perineal analgesia in the horse.

REASONS FOR PERFORMING STUDY: Short duration of analgesia is among the limitations of a single epidural injection with lidocaine in horses. OBJECTIVES: To evaluate the effectiveness and safety of epidural lidocaine in combination with either tramadol or neostigmine for perineal analgesia in horses. METHODS: Epidural catheters were placed in 6 saddle horses that then were given 3 treatments: 2% lidocaine (0.2 mg/kg bwt) alone, 2% lidocaine (0.2 mg/kg bwt) plus tramadol (0.5 mg/kg bwt), and 2% lidocaine (0.2 mg/kg bwt) plus neostigmine (1.0 μg/kg bwt). The order of treatments was randomised. H

Use of a diode laser in an excisional biopsy of two spoonlike neoformations on the tongue tip.

The 810 nm diode laser, due to its high affinity with haemoglobin (elective chromatophore) revealed, by itself, as elective instrument for surgical excision of the tongue, an organ highly vascularized and sensitive to the laser cutting action. The advantages for its use, with respect to the traditional cold blade surgery, are its haemostatic effect (which avoids to close wounds by stitches) and, not less important, the decontaminant effect of the laser ray, due to either the thermal raising created on cutting line and/or the specific selective action exerted on the bacterial membrane by its wa

Midazolam in rabbits terminates dysrhythmias caused by intracerebroventricular ropivacaine.

The current study was designed to investigate the mechanisms by which ropivacaine may act within the central nervous system (CNS) to produce cardiotoxicity. Eighty New Zealand rabbits were divided into four groups randomly. In Group 1, 20 rabbits received intracerebroventricular (icv) saline, and then received icv ropivacaine 30 min later. In Group 2, 20 rabbits received icv ropivacaine. Whenever dysrhythmias continued for more than 5 min, 0.1 ml saline was administered into the left cerebral ventricle. Ten minutes later, 0.1 ml midazolam was given into the left lateral ventricle. In Group 3,

Prevalence of benzocaine and lidocaine patch test sensitivity in Denmark: temporal trends and relevance.

UNLABELLED: BACKGROUND. Allergens included in the European baseline series should result in positive patch test reactions in at least 1% of a patch test population. Inclusion of local anaesthetics other than benzocaine in the baseline series has previously been debated. OBJECTIVES: To investigate temporal trends of benzocaine and lidocaine allergy in dermatitis patients who underwent routine patch testing in a tertiary referral patch test centre, and to clarify and discuss whether lidocaine and benzocaine should be included in routine series. METHODS: Dermatitis patients who underwent routin

In vitro assessment of human chondrocyte viability after treatment with local anaesthetic, magnesium sulphate or normal saline.

PURPOSE: Local anaesthetic agents are often used as an intra-articular analgesic following arthroscopic procedures. However, there is increasing evidence of a potential toxic effect to chondrocytes within the articular cartilage. The aim of this study was to compare the effect on human chondrocyte viability of treatment with bupivacaine, levobupivacaine and ropivacaine. The second aim was to compare the effect on chondrocyte viability of the local anaesthetics with magnesium, a potential alternative analgesic agent. METHODS: Chondrocytes were exposed to one of the local anaesthetic agents (le

Comparison of actions of systemically and locally administrated local anaesthetics in diabetic rats with painful neuropathy.

Nothing is known about actions of levobupivacaine, a long-acting local anaesthetic belonging to the amino amide group, in diabetes-induced neuropathic pain conditions. In this study, we therefore investigated the possible antihyperalgesic and antiallodynic effects of levobupivacaine in diabetic animal model. Actions of systemically (intraperitoneal) or locally (intraplantar) administrated levobupivacaine on streptozotocin-induced diabetic rats with painful neuropathy were examined using a thermal plantar test and a dynamic plantar aesthesiometer. Effects of levobupivacaine were compared with t

Nerve stimulation under ultrasound guidance expedites onset of axillary brachial plexus block.

BACKGROUND: Axillary block is popular technique in upper extremity surgery. Nerve stimulation is used for location of nerves to provide effective blockade. The advent of ultrasound imaging technique in regional anaesthesia has increased the accuracy of needle placement and local anaesthetic deposition around the nerve. The aim of this study is to find out if the nerve stimulation technique with ultrasound guidance has advantages over sole nerve stimulation technique. The outcome measures studied were onset of sensory and motor block, procedure time, number of skin puncture, vascular puncture a

Safety of spinal anesthesia in a patient with achondroplasia for cesarean section.

Controversial reports exist regarding the administration of spinal anesthesia in achondroplastic dwarfs regarding safety, dose, drug choice, in obstetric patients. Bony characteristics such as vertebral anomalies, lordosis and lumbar scoliosis, limited mouth opening and cervical spine instability make the administration of anesthesia to these patients truly a challenge. Here we demonstrate that low dose meperidine and bupivacaine spinal anesthesia in an achondroplastic parturient for cesarean section, was administered successfully after careful evaluation, consideration of risks, and awareness
