Cardiovascular and pulmonary disease (CVPD) is common in patients with obstructive sleep apnea syndrome (OSAS). This retrospective study addressed the accumulated in-hospital time during 2 years prior to treatment with nasal continuous positive airway pressure (nCPAP) as compared to 2 years after initiating of nCPAP in patients with OSAS and CVPD. A cohort representing all patients (n = 88) receiving nCPAP during the period 1988-1994 at the Skövde Central Hospital, Skövde, Sweden, was studied. Data collection was based on interviews with patients as well as reviews of clinic charts. All hospit…

We investigated the effects of negative pressure (NP) in the isolated upper airway (UA) in three unanesthetized dogs. The UA was isolated, a … terminated the breath during inspiration, and prolonged TE when applied at end expiration. Central apneas always persisted beyond the termination of the UA closure. TE prolongation in response to NP persisted in the presence of a topical anesthetic nebulized through the UA sufficient to abolish the laryngeal gag reflexes. We conclude that UA closure an …

We have previously shown that hypocapnia triggers Cheyne-Stokes respiration with central sleep apnea (CSR-CSA) in patients with congestive heart failure (CHF). Nasal continuous positive airway pressure (NCPAP) may attenuate CSR-CSA in patients with CHF and CSR-CSA. Accordingly, we hypothesized that attenuation of CSR-CSA by NCPAP would be related to an increase in PCO2. Therefore, we examined the effect of NCPAP on the frequency of apneas and hypopneas, transcutaneous PCO2 (PtcCO2), and minute volume of ventilation (VI) in 12 consecutive patients with CHF and CSR-CSA during stage 2 sleep. A co…

Pressure measurements were made preoperatively at six sites in the pharynx and esophagus in 18 obstructive sleep apnea syndrome (OSAS) patients, 3 social snorers, and 6 healthy normal controls. Repeat measurements were made approximately 2 months after uvulopalatopharyngoglossoplasty (UPPGP) in 10 of the OSAS patients. The pressure transducers were contained in a thin silicone tube inserted through one nostril, and measurements were made both in the awake state and throughout a night's sleep. Oxygen saturation was also continuously recorded and, additionally, nasal and oral airflow measurement…

The area of upper airway (from the nasopharynx to the hypopharynx) was measured by means of computed tomography (CT) scan in 15 confirmed cases of obstructive sleep apnea (OSA) and in 4 normal controls while they were awake. The minimum cross-sectional area (MA) of the upper airway was 14.7 +/- 20.0 mm2 in OSA patients and 80.0 +/- 33.1 mm2 in normal controls and the difference was statistically significant (p less than 0.01). In OSA patients, MA did not correlate with age, body weight, apnea index, desaturation index, mean nadir-SO2 and lowest SO2. MA was also measured with OSA patients while…

The effect of posture on upper airway dimensions was assessed for two reasons. First, some patients with untreated sleep apnea/hypopnea syndrome (SAHS) report they sleep better sitting upright. Second, to allow comparison of the differing techniques used to determine the site of maximal airway narrowing in awake patients with SAHS, as some are carried out in the erect and others in the supine posture. Lateral cephalometry was therefore carried out in 33 nonsnoring normal subjects and in 29 patients with obstructive SAHS (mean apneas plus hypopneas, 46 per hour; range, 17 to 103). In both norma…

We hypothesized that those obstructive sleep apnea (OSA) patients with upper airway collapse during sleep within the transpalatal airway would have a more favorable response to uvulopalatopharyngoplasty (UPP) than those patients with obstruction within the hypopharyngeal airway. We tested this hypothesis in seven OSA patients with transpalatal and seven with hypopharyngeal obstruction undergoing UPP. Preoperatively the apnea/hypopnea index (AHI) was different between palatal and hypopharyngeal obstructors, 37.8 +/- 6.0 (+/- SEM) and 63.9 +/- 6.3, respectively (p less than 0.05), but the apnea-…

Recent studies about renal function and volume regulating hormones in obstructive sleep apnea (oSAS) indicate complex disturbances in volume homeostasis. Increased nocturnal secretion of atrial natriuretic peptide (ANP) and decreased renin secretion during apnea looks similar to a situation seen during hypervolemia or increased cardiac volume load. Increased venous return induced by pathologically high negative intrathoracic pressure during obstructive apnea may be the cause. Since during wakefulness no true hypervolemia is present, a "pseudohypervolemia" or "central hypervolemia" must exist c…

Clinical experience shows that episodes of apnea can occur during regurgitations or vomiting. We questioned whether sleep apneas could be related temporally to documented falls in esophageal pH, when no clinical symptoms of emesis are witnessed. Twenty infants admitted after an apparently life-threatening event ('ALTE') during sleep, but with no clinical symptoms of vomiting or regurgitations at the time of the event, and ten control infants were studied. All infants had occasional episodes of regurgitations. Polygraphic monitoring of state of alertness, cardiorespiratory activity and low esop…

Videoendoscopy of the oropharynx was performed in 57 patients with habitual snoring and obstructive sleep apnoea syndrome. Endoscopy was done in waking supine subjects relaxed and during voluntary snoring, Esmarch's and modified Müller's manoeuvre. Evaluation of results for the first time was semiquantitative. Statistical analysis showed significant correlations (p less than 0.004) between movements of the base of the tongue and the modified Müller's manoeuvre. For the first time it could be shown that the dynamic changes in the pharyngeal walls during voluntary snoring are identical with thos

Sleep onset during the multiple sleep latency test was scored by three criteria for 21 patients with narcolepsy and 21 patients with obstructive sleep apnea: a single epoch of stage 1, three consecutive epochs of stage 1, and a single epoch of stage 2 or REM. Mean sleep latency for both groups was predictably shortest using a single epoch of stage 1 and longest using a single epoch of stage 2 or REM. All estimates of sleep latency were highly correlated. It was concluded that a single epoch of any stage of sleep is an appropriate measure of sleep latency for patients with narcolepsy, although …

Four male patients and one female patient of a new family with Joseph disease are reported. Their disease was characterized by autosomal dominant inheritance, bulging eyes, rigidity and spasticity of the lower extremities, dystonia, and bradykinesia. Cerebrospinal fluid homovanillic acid level was markedly reduced. Levodopa improved dystonia. Magnetic resonance imaging revealed mild atrophy of the frontal lobe and the cerebellum and marked atrophy of the lenticular nucleus and the brain stem. Polysomnographic studies revealed non-rapid eye movement stage central type sleep apnea syndrome. This…

A 6-year-old girl had subacute onset of hypoventilation and apnea during sleep. Diffuse dysautonomic changes were identified, including dilated, nonreactive pupils, decreased tearing and sweating, and abnormal temperature and cardiovascular control. All-night polysomnographic studies revealed frequent obstructive and central sleep apnea episodes. Her serum contained cytotoxic antineuroblastoma immunoglobulins. She died two years later during sleep. The general pathological examination revealed a ganglioneuroma originating in the sympathetic ganglia. Abnormalities in the brain were confined to …

Cardiorespirographic studies were performed in 11 prematures with episodes of apnea and bradycardia (gestational age: x = 31 weeks, birth weight: x = 1390g). Total registration time was 90 h; 45 h during spontaneous breathing, 45 h during CPAP-treatment. Episodes of bradycardia occured three times more frequently than apnea. The frequency of apnea and bradycardia, the duration of bradycardia and the deceleration area were significantly reduced during CPAP-therapy, whilst the duration of apnea was not influenced. Estimation of deceleration areas as compared to measurements of bradycardia durati…

Episodes of cyanosis, bradycardia, or apnea were studied in 26 newborns and prematures by continuous recording of tcPO2, beat-to-beat heart rate, and impedance pneumography. Hypoxemia related to therapeutic, diagnostic, or care procedures is very common and often is not accompanied by bradycardia or apnea. Idiopathic bradycardia lasting less than 30 seconds will not lead to hypoxemia. However, idiopathic bradycardia of more than 30 seconds' duration can lead to hypoxemia. Beat-to-beat cardiograms and transcutaneous oxygen measurement appear to be the most reliable methods for routine monitorin…

The sleep and respiratory patterns of eight apneic preterm infants were polygraphically recorded for 24 hours. This polygraphic study was designed to test and extend our previous finding that gently oscillating water beds reduce apnea in premature infants. The infants who ranged in gestational age from 27 to 32 weeks and in birth weight from 1,077 to 1,650 gm served as their own controls, off and on the water bed. The 24-hour recordings were divided into four time blocks with the infant being placed on the water bed during alternate six-hour periods. Apnea was significantly reduced while the i…
