OBJECTIVES: The aim of this study was to determine the changes in sympathetic nerve activity (SNA) after atrioventricular junction (AVJ) ablation in patients with chronic atrial fibrillation (AF).
BACKGROUND: Polymorphic ventricular tachycardia (PMVT) has been reported after AVJ ablation in patients paced at a rate of < or =70 beats/min. Although first described about 100yr ago, atrial fibrillation (AF) is now recognized as the most common of all arrhythmias. It has a substantial morbidity and presents a considerable health care burden. We examined brains clinicopathologically from 54 consecutive paroxysmal atrial fibrillation (PAF) patients aged 70 years or older and compared them with those of 59 age-matched controls. Symptomatic cerebral infarctions were present in 29 PAF patients (53.7%) and in 13 controls (22.0%) (p < 0.001). Symptomatic brain infarction was 2.4 times more common in PAF cases than in PAF-free controls; approximately 60% of the infarctions in the PAF cases were judged to be cardioembolic in origin. PAF in the elderly can be an important cause for cardiogenic cerebral embolism.

External jugular venous (EJV) catheterization is a frequent method of gaining central venous access because of its low complication rate. We describe a unique experience during EJV in which a patient with chronic atrial fibrillation converted suddenly to a sustained sinus rhythm without perioperative complication from this event. Our experience depicts a rare consequence of the central catheter placement and we, as anesthesiologists, should be aware of its potential occurrence in the operating room.

Cerebral blood flow was measured by transcranial Doppler sonography (TCD) in 15 patients during and after an attack of paroxysmal atrial fibrillation. We present a unique patient with a 12-year history of chronic atrial fibrillation resistant to external direct current shocks associated with incessant ventricular tachycardia. Both arrhythmias were treated with a single shock delivered through a catheter at the atrioventricular junction. The other unusual feature is that sinus rhythm has been maintained in a follow-up of 6 months after the intervention.
