Glial cell line-derived neurotrophic factor stimulates fiber formation and survival in cultured neurons from peripheral autonomic ganglia.

Human recombinant glial cell line-derived neurotrophic factor (GDNF) was tested for its ability to stimulate fiber formation and neuron survival in primary cultures of peripheral ganglia dissected from the chicken embryo. GDNF, first characterized by its actions on central nervous system (CNS) neurons, had a marked stimulatory effect on fiber outgrowth in sympathetic and ciliary ganglia. Weaker responses were evoked in sensory spinal and nodose ganglia and in the ganglion of Remak. In addition, survival of neurons from the sympathetic and ciliary ganglia was stimulated by GDNF at 50 ng/ml. The

Role of the central nervous system in hemodynamic and sympathoadrenal responses to cocaine in rats.

These studies were undertaken to examine the contribution of central nervous system mechanisms to the cardiovascular and sympathoadrenal effects of cocaine. Changes in systolic and diastolic blood pressure, heart rate and plasma catecholamine concentrations were determined in response to cocaine injected i.a. or i.c.v. in conscious unrestrained rats. Systemically administered cocaine produced brisk, transient dose-related increases in systolic and diastolic pressure at doses of 0.05 to 5 mg/kg i.a. Plasma catecholamine concentrations increased in a dose-related manner, reaching peak levels at

Does stress induce bowel dysfunction?

Psychological stress is known to induce somatic symptoms. Classically, many gut physiological responses to stress are mediated by the hypothalamus-pituitary-adrenal axis. There is, however, a growing body of evidence of stress-induced corticotrophin-releasing factor (CRF) release causing bowel dysfunction through multiple pathways, either through the HPA axis, the autonomic nervous systems, or directly on the bowel itself. In addition, recent findings of CRF influencing the composition of gut microbiota lend support for the use of probiotics, antibiotics, and other microbiota-altering agents a…

The expanding universe of disorders of the basal ganglia.

The basal ganglia were originally thought to be associated purely with motor control. However, dysfunction and pathology of different regions and circuits are now known to give rise to many clinical manifestations beyond the association of basal ganglia dysfunction with movement disorders. Moreover, disorders that were thought to be caused by dysfunction of the basal ganglia only, such as Parkinson's disease and Huntington's disease, have diverse abnormalities distributed not only in the brain but also in the peripheral and autonomic nervous systems; this knowledge poses new questions and chal…

[Diffuse digestive ganglioneuromatosis].

Discovery of intestinal ganglioneuromatosis implies the presence of the MEN IIb syndrome or, more rarely, von Recklinghausen's neurofibromatosis. The two conditions are due to dysfunction of the neural crest. This very rare intestinal pathology is illustrated by two observations. The intestinal motility disorders, due to the diffuse proliferation of the peripheral autonomous nervous system which ganglioneuromatosis represents, may have serious implications and indeed prove fatal. The etiologic role of excessive nerve growth factor production in these two entities is mentioned. The physiopathol

Assessment of autonomic function in a cohort of patients with ANCA-associated vasculitis.

OBJECTIVE: To assess symptoms and objective parameters of autonomic dysfunction (AD) in patients with ANCA-associated vasculitides. METHODS: Symptoms and objective parameters of AD were assessed in patients with ANCA-associated vasculitis and in age-matched healthy controls. Autonomic symptoms were explored by COMPASS31, a validated questionnaire addressing symptoms of six autonomic domains (orthostatic, vasomotor, secretomotor, gastrointestinal, pupillomotor, and bladder dysfunction). Objective autonomic parameters consisted of expiratory/inspiratory (E/I) ratio during the deep breathing tes

Clinical laboratory evaluation of autoimmune autonomic ganglionopathy: Preliminary observations.

Several forms of chronic autonomic failure manifest as neurogenic orthostatic hypotension, including autoimmune autonomic ganglionopathy (AAG) and pure autonomic failure (PAF). AAG and PAF are thought to differ in pathogenesis, AAG reflecting decreased ganglionic neurotransmission due to circulating antibodies to the neuronal nicotinic receptor and PAF being a Lewy body disease with prominent loss of sympathetic noradrenergic nerves. AAG therefore would be expected to differ from PAF in terms of clinical laboratory findings indicating post-ganglionic noradrenergic denervation. Both diseases ar

Cardiac sympathetic degeneration informs the duration of the prodromal stage of body-first Lewy body disease.

Body-first Lewy body disease (LBD) is hypothesized to begin in the peripheral autonomic nervous system years before nigrostriatal involvement. Isolated REM sleep behaviour disorder (iRBD) is considered an early manifestation of body-first LBD but is preceded by an unknown period of peripheral autonomic degeneration. Consequently, the total duration of the prodromal phase remains unknown. We used longitudinal imaging data to model the progression of cardiac sympathetic and nigrostriatal dopaminergic degeneration and employed the resulting curves to estimate the prodromal period of body-first LB…

Cardiac Neuromodulation and Neurocardiology.

Neurocardiology has mostly been a specialty of medicine led by anatomists and physiologists. The characterization of the cardiac autonomic nervous system has resulted in a new understanding and appreciation of neurocardiology, leading to potential novel neuromodulation therapies in clinical cardiology and cardiac electrophysiology. Sympathectomy or spinal cord stimulation for the treatment of angina pectoris, as well as cardiac sympathetic denervation for the treatment of long QT syndrome associated with malignant ventricular arrhythmias, have been available and performed for more than half a …

Cardiac 123I-Meta-Iodobenzylguanidine Imaging as a Biomarker for Body-First Parkinson's Disease: Linking Peripheral α-Synuclein to Clinical Subtyping.

Recent neuropathological and imaging studies support the concept of "brain-first vs. body-first" Parkinson's disease (PD), which is based on the α-synuclein origin site and connectome model. The body-first phenotype is characterized by early involvement of the peripheral autonomic nervous system, particularly the cardiac sympathetic nerves and enteric nerves. 123I-meta-iodobenzylguanidine (123I-MIBG) myocardial scintigraphy is a well-established method for evaluating cardiac sympathetic innervation. This review explores the potential of 123I-MIBG scintigraphy as a biomarker to differentiate th…

The effects of Rajyoga mindfulness meditation training on heart rate variability in panic disorder: A randomized controlled trial.

BACKGROUND: Panic disorder is associated with autonomic nervous system dysfunction and reduced heart rate variability (HRV). Enhancing HRV may help mitigate symptoms of panic disorder. This study investigated the effects of Rajyoga mindfulness meditation training on HRV in patients with panic disorder. METHODS: This prospective randomized controlled trial included 110 patients with panic disorder recruited from a psychiatry department in India. Participants were randomized to either an 8-week Rajyoga meditation intervention (n = 55) or a waitlist control group (n = 55) receiving standard trea…

The Clinical Significance and Application of Heart Rate Variability in Dialysis Patients: A Narrative Review.

Autonomic nervous system (ANS) dysfunction is prevalent in end-stage kidney disease (ESKD) patients, carrying significant risks for morbidity and mortality. Heart rate variability (HRV) is a simple and non-invasive method to evaluate ANS functions and predict prognoses in specific patient populations. Since there is a lack of a clear understanding of the clinical significance of HRV in predicting prognoses in ESKD patients, an updated review on this topic is urgently warranted. The clinical significance of HRV in dialysis patients includes its associations with metabolic syndrome, nutritional …

The intricate brain-heart connection: The relationship between heart rate variability and cognitive functioning.

In the last years, there has been a growing interest in the brain-heart connection. A core aspect of this connection appears to be the autonomic nervous system, particularly through the vagus nerve. Accordingly, vagally mediated heart rate variability (vmHRV) is currently considered as an index of top-down control processes involved in cognition and emotion regulation. Recent evidence indicates that higher vmHRV is associated with enhanced cognitive performance across multiple domains, such as executive functions, memory, attention, and language skills. From this premises, this study examined …

Microstates of the cortical brain-heart axis.

Electroencephalographic (EEG) microstates are brain states with quasi-stable scalp topography. Whether such states extend to the body level, that is, the peripheral autonomic nerves, remains unknown. We hypothesized that microstates extend at the brain-heart axis level as a functional state of the central autonomic network. Thus, we combined the EEG and heartbeat dynamics series to estimate the directional information transfer originating in the cortex targeting the sympathovagal and parasympathetic activity oscillations and vice versa for the afferent functional direction. Data were from two

Sudomotor dysfunction in diabetic peripheral neuropathy (DPN) and its testing modalities: A literature review.

Long term consequences of diabetes mellitus (DM) may include multi-organ complications such as retinopathy, cardiovascular disease, neuronal, and kidney damage. One of the most prevalent complication is diabetic peripheral neuropathy (DPN), occurring in half of all diabetics, and is the main cause of disability globally with profound impact on a patient's quality of life. Small fiber neuropathy (SFN) can develop in the pre-diabetes stage preceding large fiber damage in DPN. Asymptomatic SFN is difficult to diagnose in early stages, with sudomotor dysfunction considered one of the earliest mani

Transcutaneous electrical vagus nerve stimulation to suppress premature ventricular complexes (TREAT PVC): study protocol for a multi-center, double-blind, randomized controlled trial.

BACKGROUND: The autonomic nervous system can be responsible for the initiation and maintenance of arrhythmias. Low-level tragus stimulation (LLTS), a noninvasive form of autonomic neuromodulation, has been shown to be effective in treating atrial fibrillation. We intended to treat frequent premature ventricular complexes (PVCs) with LLTS. METHODS AND DESIGN: The present study will be a prospective multicenter, double-blind, randomized, controlled trial to assess the antiarrhythmic effects of LLTS on frequent PVCs in patients without structured heart disease (SHD). A total of 100 patients with…

Effect of Hyperventilation on Periodic Repolarization Dynamics.

Heart and lung functions are closely connected, and the interaction is mediated by the autonomic nervous system. Hyperventilation has been demonstrated to especially activate its sympathetic branch. However, there is still a lack of methods to assess autonomic activity within this cardiorespiratory coupling. Periodic repolarization dynamics (PRD) is an ECG-based biomarker mirroring the effect of efferent cardiac sympathetic activity on the ventricular myocardium. Its calculation is based on beat-to-beat variations of the T wave vector (dT°). In the present study, we investigated the effects of…

Central Sensitization-Based Classification for Temporomandibular Disorders: A Pathogenetic Hypothesis.

Dysregulation of Autonomic Nervous System (ANS) and central pain pathways in temporomandibular disorders (TMD) is a growing evidence. Authors include some forms of TMD among central sensitization syndromes (CSS), a group of pathologies characterized by central morphofunctional alterations. Central Sensitization Inventory (CSI) is useful for clinical diagnosis. Clinical examination and CSI cannot identify the central site(s) affected in these diseases. Ultralow frequency transcutaneous electrical nerve stimulation (ULFTENS) is extensively used in TMD and in dental clinical practice, because of …
