BACKGROUND: Patients who undergo radiotherapy following mastectomy may develop radiation-induced ulcers (RIUs) years later that are often refractory to healing. Reconstruction of these refractory ulcers remains a challenge for surgeons, and conventional surgical approaches can have disastrous outcomes.
METHODS: We present a detailed itemization and analysis of specific surgical methods-including local random-pattern flaps, pedicled flaps, and free flaps-as well as conservative treatments, along with their therapeutic outcomes, advantages, and disadvantages.
RESULTS: Chest wall reconstruction

BACKGROUND: Chronic low back pain is notoriously challenging to diagnose and manage, especially when imaging fails to reveal a cause. Superior cluneal nerve entrapment is an increasingly recognized, but often overlooked, source of pain that can mimic lumbosacral radiculopathy.
CASE REPORT: We describe a 43-year-old woman with longstanding chronic low back pain unresponsive to physical therapy, extracorporeal shockwave therapy, and targeted sacroiliac joint injections. A physical examination revealed focal tenderness along the posterior iliac crest, a positive Tinel sign, and pain over the ili

BACKGROUND: Hallux valgus is one of the most common foot deformities in adults. In addition to established open procedures-ranging from distal metatarsal osteotomy to first tarsometatarsal (TMT-I) arthrodesis-modern minimally invasive techniques have gained increasing clinical relevance.
METHODS AND RESULTS: This article provides a comprehensive overview of third- and fourth-generation MIS hallux valgus techniques, including the minimally invasive Chevron-Akin osteotomy (MICA), percutaneous Chevron-Akin osteotomy (PECA) and MIS Scarf osteotomy. Mid- and long-term outcomes are comparable to th

BACKGROUND: Cutaneous adnexal carcinomas are a very rare group of cancers arising from appendageal structures of the skin. More than half occur in the head and neck region. No standard treatments exist for metastatic disease.
METHODS: A 48-year-old postmenopausal female presented with an enlarging scalp lesion. Biopsy showed adenocarcinoma that was positive for estrogen and progesterone receptors. She underwent wide local excision with adjuvant radiation. Unfortunately, she presented with metastatic disease 1.5 years later.
RESULTS: Given the tumor's hormone receptor profile, she was started

BACKGROUND: We compared treatment delays, costs, and management impact between oropharyngeal squamous cell carcinoma (OPSCC) patients undergoing in-office versus general anesthesia panendoscopy-guided biopsies in two large tertiary centers in a public healthcare system.
METHODS: All OPSCC patients included were treated between 2010 and 2024 with either curative radiation alone or chemoradiation.
RESULTS: Demographic and tumor characteristics were comparable, except for smaller tumors in the panendoscopy group (n = 36) compared to the in-office group (n = 37) (mean size; 29.4 mm vs. 36.6 mm,

Background: Thyroid cancer (TC) is infrequently encountered in Birt-Hogg-Dubé (BHD) syndrome. We describe three BHD patients with TC and review the relevant literature. Patient Findings: Patient 1, a 55-year-old male with BHD, developed dedifferentiated oncocytic TC with distant metastases, requiring systemic therapy and radiation. Genetic testing revealed pathogenic variants (PVs) in FLCN, DAXX, and TP53. Patient 2, a 51-year-old female, and her 30-year-old daughter (patient 3) were diagnosed with papillary TC and treated with surgery and radioiodine. Tumor testing in patient 3 demonstrated P

BACKGROUND: Radiation-induced alopecia (RIA) typically presents as transiently or persistently non-scarring alopecia with a distinct geometric pattern, and minoxidil sometimes serves as an effective therapeutic agent. However, there have been no reported cases of severe unilateral alopecia following radiotherapy for pediatric glioblastoma multiforme (p-GBM), that was unresponsive to minoxidil treatment.
CASE REPORT: This case presents a 17-year-old male with severe unilateral alopecia, who was diagnosed with glioblastoma multiforme (GBM) and consequently received radiochemotherapy. Remarkably

BACKGROUND: Medical decision-making in indigent patients with baseline cognitive deficits and lack of supportive resources is a difficult problem that reveals many ethical issues. Performing high-level medical procedures (eg, invasive surgery, chemotherapy, radiation therapy) on these patients without supportive resources brings into question the efficacy and necessity of treatment if it places the patient at higher risk of additional complications and mortality.
CASE PRESENTATION: In this article, we present a case of a 62-year-old indigent man with a long history of encephalopathy secondary

BACKGROUND: Delayed radiation-induced complications after stereotactic radiosurgery (SRS) for arteriovenous malformations (AVM) have scarcely been described in the literature, and their incidence, pathophysiology, and treatment remain unclear. Additionally, the literature regarding these complications is confusing. The authors present a well-documented case report describing these late complications, adding evidence to the possible common pathophysiological mechanism underlying them, and illustrating an effective treatment modality when they occur.

Low-density lipoprotein (LDL) plays a central role in the development of atherosclerosis, making its detection critical for cardiovascular disease management. Radionuclide imaging of LDL offers distinct advantages over other modalities but remains limited by poor specificity and the need for long-lived isotopes. We present a click chemistry-based PET imaging strategy using gallium-68 (68Ga), a short-lived radionuclide, for the specific detection of LDL accumulation in atherosclerotic plaques. The approach relies on a two-step inverse electron-demand Diels-Alder reaction between trans-cycloocte

BACKGROUND: Ventricular septal rupture (VSR) post myocardial infarction (MI) is a rare, high-risk complication requiring urgent intervention. Fusion imaging (FI), integrating echocardiography and fluoroscopy, offers a novel approach for percutaneous closure in unstable patients.
CASE SUMMARY: A 60-year-old man developed cardiogenic shock and basal VSR after inferior MI stent placement. Echocardiography confirmed the defect with a left-to-right shunt. Using FI, percutaneous closure with a 16-mm Amplatzer device was completed under real-time guidance. Post-procedure imaging revealed a well-seat
