Copper: not too little, not too much, but just right. Based on the triennial Pewterers Lecture delivered at the National Hospital for Neurology, London, on 23 March 1995.

Copper is an essential trace element with a well established mechanism for maintaining balance of the metal in the body, which is controlled by at least two genes. Disruption of one gene on the X chromosome determines a defect in the process of copper absorption, with consequent deficiency of available copper at the cellular level. This results in abnormalities of collagen formation and brain maturation, leading to early death. As yet there is no effective treatment. Disruption of the other copper controlling gene, located on chromosome 13, is associated with accumulation of excess copper in t

Iron and aluminum increase in the substantia nigra of patients with Parkinson's disease: an X-ray microanalysis.

The levels of different elements were studied by x-ray microanalysis in the substantia nigra and the central gray substance of patients with Parkinson's disease, progressive supranuclear palsy, and matched controls. In control brains, only iron, potassium, silicum, sodium, sulfur, and zinc were within the limit of detection of the technique. The abundance of each element was different, but their respective concentrations in the two brain regions were similar, except for sulfur levels which were higher on neuromelanin aggregates in the substantia nigra than in nigral regions lacking neuromelani

A developmental component to Huntington's disease.

Huntington's disease is a dominantly inherited disorder characterized by the dysfunction and death of cortical and striatal neurons. Striatal degeneration in Huntington's disease is due, at least in part, to defective cortical signalling to the striatum. Although Huntington's disease generally manifests at the adult stage, mouse and neuroimaging studies of presymptomatic mutation carriers suggest that it may affect neurodevelopment. In support of this notion, the development of the cortex is altered in mice with Huntington's disease and the foetuses of human Huntington's disease gene carriers.

Neurotrauma Update.

Traumatic brain injury (TBI) continues to be a leading cause of morbidity and mortality worldwide with older adults having the highest rate of hospitalizations and deaths. Management in the acute phase is focused on preventing secondary neurologic injury from hypoxia, hypocapnia, hypotension, and elevated intracranial pressure. Recent studies on tranexamic acid and continuous hypertonic saline infusion have not found any difference in neurologic outcomes. Care must be taken in prognosticating TBI outcomes, as recovery of consciousness and orientation has been observed up to 12 months after inj

[Inflammation and Parkinson's disease].

Parkinson's disease is a neurodegenerative disorder associated with aging characterized by a motor extrapiramidal alteration secondary to the progressive death of dopaminergic neurons of the substantia nigra pars compacta. The cause of this neuronal loss remains unknown but post mortem studies on brains of parkinsonian patients showed high index of inflammatory mechanism markers. This point has gone to open new lines of research in order to ascertain what role have these inflammatory process in neuronal degeneration and has opened new therapeutic possibilities to stop or at least to brake the

A new mutant of Japanese quail (Coturnix coturnix japonica) characterized by generalized glycogenosis.

A flock of Japanese quail with generalized glycogenosis has been established. Affected quail showed difficulty in raising their wings. Excessive accumulation of glycogen was seen in the liver, heart, skeletal muscle and brain, apparently due to decreased acid maltase activity. The condition appeared between 2 and 12 weeks of age and tissue deposition of glycogen increased with age. The growth of affected quail was normal and there were no deaths from the condition. Although genetic analysis has not yet been completed, an autosomal recessive inheritance is suspected.

Progressive multifocal leukoencephalopathy.

Three cases of progressive multifocal leukoencephalopathy have occurred in one hospital in a 3-year period. The 3 patients had been receiving chemotherapy for chronic myeloid leukaemia, lymphocytic lymphoma and acute myeloid leukaemia respectively, and developed progressive neurological illnesses which resulted in their deaths. The routine neurological investigations were unhelpful and although the diagnosis was suspected in 2 cases, treatment was not given. It is felt that the disease is not as rare as originally believed and that brain biopsy should be more readily considered since progressi

Postmortem Redistribution of Oxycodone.

The abuse of oxycodone (OC) as a "recreational" drug has increased in the last two decades the risk of fatalities. We report two drug-related deaths in which OC intoxication was considered the cause of death. Two brothers, a 19-year-old male (Subject 1) and a 27-year-old male (Subject 2), were found dead in their bedroom, lying on their beds. They were testified as alive in the morning, both deeply sleeping and loudly snoring in their beds; they were found dead later in the afternoon. On death scene investigation, two packs of 40 mg OxyContin extended release and one alprazolam blister were fo

Establishment and validation of a nomogram model for predicting early death in patients with endometrial cancer bone metastases.

BACKGROUND: Patients with endometrial cancer bone metastases (ECBM) are clinically rare and have a poor prognosis, including a higher incidence of early death (survival ≤ 3 months). Currently, no practical tools exist to predict early mortality in these patients. Thus, there is an urgent need to develop clinically applicable predictive models, such as nomograms, for individualized assessment of early death risk in ECBM. METHODS: Relevant clinical and pathological data for ECBM patients from the SEER database (2010-2021). Univariate and multivariate logistic regression analyses were performed

Brain tumor classification based on neural architecture search.

Brain tumor is a life-threatening disease and causes about 0.25 million deaths worldwide in 2020. Magnetic Resonance Imaging (MRI) is frequently used for diagnosing brain tumors. In medically underdeveloped regions, physicians who can accurately diagnose and assess the severity of brain tumors from MRI are highly lacking. Deep learning methods have been developed to assist physicians in detecting brain tumors from MRI and determining their subtypes. In existing methods, neural architectures are manually designed by human experts, which is time-consuming and labor-intensive. To address this pro

Liquid biopsies for the diagnosis and surveillance of primary pediatric central nervous system tumors: a review for practicing neurosurgeons.

OBJECTIVE: Primary brain tumors are the most common cause of cancer-related deaths in children and pose difficult questions for the treating physician regarding issues such as the risk/benefit of performing a biopsy, the accuracy of monitoring methods, and the availability of prognostic indicators. It has been recently shown that tumor-specific DNA and proteins can be successfully isolated in liquid biopsies, and it may be possible to exploit this potential as a particularly useful tool for the clinician in addressing these issues. METHODS: A review of the current literature was conducted by

Inflammatory cells and cytokines in the olfactory bulb of a rat model of neuroinflammation; insights into neurodegeneration?

This study examined inflammatory cell and cytokine production in brain tissue from a lipopolysaccharide (LPS)-treated rat model that mimics many of the neuropathologic changes associated with neurodegenerative diseases We also monitored the appearance of a glial cell line-derived neurotrophic factor (GDNF) and circulating nitric oxide (NO) levels, as well as an immune system-associated cells in a selected area of the brain, the olfactory lobe. The studies were based on the hypothesis that LPS treatment stimulates temporal changes within the brain and that these responses include immune cell re

Normothermic Regional Perfusion in Controlled Donation After Circulatory Death: Growing Evidence for Liver Transplantation.

Normothermic regional perfusion (NRP) has emerged as a pivotal strategy in controlled donation after circulatory death (cDCD) liver transplantation, mitigating ischemia-reperfusion injury and improving graft outcomes. Compared to super rapid recovery (SRR), NRP significantly reduces rates of early allograft dysfunction, primary nonfunction, and biliary non-anastomotic stricture (NAS), with biliary complications reported in 5%-16% and NAS as low as 0%-2%. Outcomes from cDCD-NRP grafts are increasingly comparable to those of donation after brain death (DBD). Viability assessment during NRP remai…

Strawberry notch 1 safeguards neuronal genome via regulation of Yeats4 expression.

Neurons are subjected to various stresses, including high metabolic demand, physiological activity, and transcriptional regulation, to which their genomic DNA are vulnerable. Genome stability of neurons is essential for proper physiological brain function. Failure in accurate genomic DNA repair can result in abnormal neuronal functions or cell death. Genomic instability has been implicated in increased risks of neurodevelopmental and neurodegenerative disorders. However, the molecular mechanisms underlying neuronal genome stability remain poorly understood. Mutations in the Strawberry Notch Ho

Fatal air embolism caused by air entry into the cardiopulmonary bypass circuit during minimally invasive atrial septal defect repair: a case report.

An 18-year-old female patient was preoperatively diagnosed with atrial septal defect (ASD) and tricuspid regurgitation. The patient's had a complex secundum-type superior sinus venosus ASD, rather than a primum ASD, which was anatomically unsuitable for transcatheter closure. The patient underwent minimally invasive ASD repair under general anesthesia using three-port totally thoracoscopy with cardiopulmonary bypass (CPB). During the initiation of CPB, poor venous drainage through the right internal jugular vein cannula allowed air to enter the CPB circuit, leading to intraoperative cardiac ar

Overcoming the Boundaries of Heart Warm Ischemia in Donation After Circulatory Death: The Padua Case.

A 45 year old male obese patient with a previous history of repaired congenital heart disease developed worsening heart failure making heart transplantation listing mandatory. Unfortunately, due to his anthropometric measures, the search for a suitable brain-dead donor was unsuccessful. For this reason, he accepted to be enrolled in the controlled donation after circulatory death (cDCD) program. According to the Italian Law regulating death declaration after cardiac arrest (no-touch period of 20 minutes-one of the longest in the world), we faced a 34 minute cardiac asystole, after which the he

Methotrimeprazine is a neuroprotective antiviral in JEV infection via adaptive ER stress and autophagy.

Japanese encephalitis virus (JEV) pathogenesis is driven by a combination of neuronal death and neuroinflammation. We tested 42 FDA-approved drugs that were shown to induce autophagy for antiviral effects. Four drugs were tested in the JE mouse model based on in vitro protective effects on neuronal cell death, inhibition of viral replication, and anti-inflammatory effects. The antipsychotic phenothiazines Methotrimeprazine (MTP) & Trifluoperazine showed a significant survival benefit with reduced virus titers in the brain, prevention of BBB breach, and inhibition of neuroinflammation. Both dru

Targeting ferroptosis as novel therapeutic approaches for epilepsy.

Epilepsy is a chronic disorder of the central nervous system characterized by recurrent unprovoked seizures resulting from excessive synchronous discharge of neurons in the brain. As one of the most common complications of many neurological diseases, epilepsy is an expensive and complex global public health issue that is often accompanied by neurobehavioral comorbidities, such as abnormalities in cognition, psychiatric status, and social-adaptive behaviors. Recurrent or prolonged seizures can result in neuronal damage and cell death; however, the molecular mechanisms underlying the epilepsy-in

Deficient adaptation to centrosome duplication defects in neural progenitors causes microcephaly and subcortical heterotopias.

Congenital microcephaly (MCPH) is a neurodevelopmental disease associated with mutations in genes encoding proteins involved in centrosomal and chromosomal dynamics during mitosis. Detailed MCPH pathogenesis at the cellular level is still elusive, given the diversity of MCPH genes and lack of comparative in vivo studies. By generating a series of CRISPR/Cas9-mediated genetic KOs, we report here that - whereas defects in spindle pole proteins (ASPM, MCPH5) result in mild MCPH during development - lack of centrosome (CDK5RAP2, MCPH3) or centriole (CEP135, MCPH8) regulators induces delayed chromo
