Enhancing Recovery From Sepsis: A Review.

IMPORTANCE: Survival from sepsis has improved in recent years, resulting in an increasing number of patients who have survived sepsis treatment. Current sepsis guidelines do not provide guidance on posthospital care or recovery. OBSERVATIONS: Each year, more than 19 million individuals develop sepsis, defined as a life-threatening acute organ dysfunction secondary to infection. Approximately 14 million survive to hospital discharge and their prognosis varies. Half of patients recover, one-third die during the following year, and one-sixth have severe persistent impairments. Impairments includ

Is ondansetron beneficial for psychosis associated with dementia?

BACKGROUND: This systematic review identified published studies that evaluated the use of ondansetron in individuals with psychosis associated with dementia. METHODS: A literature search was conducted of PubMed, MEDLINE, EMBASE, PsycINFO, and Cochrane Collaboration databases for randomized controlled trials (RCTs), cohort studies, and case reports that evaluated the use of ondansetron for individuals with psychosis associated with dementia. Bibliographic databases of published articles were also searched for additional studies. RESULTS: A total of 4 studies were identified, all of which were

[Diabetes mellitus and dementia].

Diabetes mellitus (DM) has been proposed as a risk factors for the development of vascular dementia (VaD) and Alzheimer's disease (AD). We characterized a new clinical entity of a dementia subgroup associated with specific DM-related factors rather than with AD pathology or vascular disease. This type of dementia, not showing hypoperfusion in the parietotemporal lobe on SPECT or cerebrovascular lesions on MRI, was characterized by old age, high hemoglobin A1c level, long duration of diabetes, high frequency of insulin therapy, low frequency of apolipoprotein E 4 carrier, less severe medial tem

Executive and modulatory neural circuits of defensive reactions: implications for panic disorder.

The present review covers two independent approaches, a neuroanatomical and a pharmacological (focused on serotonergic transmission), which converge in highlighting the critical role of the hypothalamus and midbrain periaqueductal gray matter in the generation of panic attacks and in the mechanism of action of current antipanic medication. Accordingly, innate and learned fear responses to different threats (i.e., predator, aggressive members of the same species, interoceptive threats and painful stimuli) are processed by independent circuits involving corticolimbic regions (the amygdala, the h

Nonmotor manifestations in Parkinson disease.

BACKGROUND: Although the diagnosis of Parkinson disease (PD) still relies mainly on the appearance of its classical motor features of resting tremor, rigidity, bradykinesia, and postural instability, nonmotor manifestations in PD are now recognized as an integral component of this multisystem disorder. REVIEW SUMMARY: Nonmotor complications in PD occur commonly. The current understanding of cognitive dysfunction; neuropsychiatric manifestations including psychosis, impulsive control, and compulsive disorders, depression, anxiety and apathy; autonomic complications such as hypotension, erectil

A model of neurocognitive function in spina bifida over the life span.

Spina bifida myelomeningocele (SBM), a neural tube defect that is the product of a complex pattern of gene-environment interactions, is associated with naturally occurring, systematic variability in the neural phenotype and in environmental factors that lead to systematic variability in the cognitive phenotype. We characterize the basis for variability in the cognitive phenotype of children with SBM with reference to a model of key biological, cognitive, and environmental events unfolding over the course of development from infancy to middle age. The cognitive phenotype is not domain-specific,

[Pott's disease in a patient affected with epilepsy and behavioural abnormalities].

The authors report on a case of Pott's disease in a patient attending a long-term psychiatric hospital for about 34 years. Difficulties regarding case management concern either diagnosis, due to the numerous and misleading comorbidities, or pharmacological treatment, due to both the interference between neurotropic and antitubercular drugs and the capacity of some antimycobacterial drugs to reduce significantly the epileptogenic threshold, thus enhancing epileptic crises.

Auditory hallucinations, source monitoring, and the belief that "voices" are real.

The term source monitoring refers to a variety of cognitive processes individuals use to determine whether an experience originated within the self or came from an external source. A belief that auditory hallucinations are real entities independent of the self may be considered an error in source monitoring. The Source Monitoring Framework (SMF) is the most developed and empirically validated model of how ordinary individuals judge whether an event was self-generated or occurred in the outside world. This study of 41 acute inpatients is a first attempt to apply the SMF to autobiographical repo

Facilitated communication since 1995: a review of published studies.

Previous reviews of Facilitated Communication (FC) studies have clearly established that proponents' claims are largely unsubstantiated and that using FC as an intervention for communicatively impaired or noncommunicative individuals is not recommended. However, while FC is less prominent than in the recent past, investigations of the technique's efficacy continue. This review examines published FC studies since the previous major reviews by Jacobson, Mulick, and Schwartz (1995) and Simpson and Myles (1995a). Findings support the conclusions of previous reviews. Furthermore, this review critiq

Role of individual neurons and neural networks in cognitive functioning of the brain: a new insight.

The prevailing concept in modern neuroscience is that neuron networks play a dominant role in the functioning of the nervous system, whereas the role of individual neurons is rather insignificant. This concept suggests that "individuality" of single neurons is primarily determined by their place in a network rather than their intrinsic properties. Here I argue that individual neurons may play an important, if not decisive, role in performing cognitive functions of the brain. This tentative viewpoint is supported by experimental and clinical insights into disorders of cognitive functions and by

The relationship between cognitions and panic attack intensity.

OBJECTIVE: To investigate the relationship between panic attack intensity, catastrophic belief and cognitions. METHOD: Data from 540 panic diaries were collected from 42 patients with panic disorder with agoraphobia. RESULTS: When we controlled for individual variation, effects of treatment and number of symptoms, catastrophic belief contributed significantly to more intense panic attacks (r2 change = 0.10, P<0.0001). There were no significant differences in panic attack intensity between primary and secondary cognitions. CONCLUSION: Our results indicate that catastrophic belief is a reliab

Collaborative consultation: a systemic activity.

In a collaborative consultation model of speech-language service delivery, the treatment system is expanded beyond the traditional client-centered dyad. Further, services are provided in the context of the classroom rather than in the noncontextual environment of a therapy room, and treatment goals relate to aspects of the school, such as curriculum, classroom behavior, and social interactions with peers. The use of the model is illustrated with a child having a cognitive impairment.

Can hypoglycaemia cause obsessions and ruminations?

Two cases of obsessional ruminations apparently secondary to functional hypoglycaemia are described. Both patients are young Caucasian males with obsessional histories at the start of dietary treatment of 18 and 6 years, respectively. In both cases, reactive hypoglycaemia was confirmed by glucose tolerance test. The first case made a complete recovery on dietary treatment which has lasted over two years; previous treatment with drugs, behaviour therapy, and counselling were unsuccessful The response of the second case has been less outstanding due to poor compliance, but improvement and worsen

Subtypes of cognitive impairment in cerebellar disease identified by cross-diagnostic cluster-analysis: results from a German multicenter study.

BACKGROUND: Cognitive and neuropsychiatric impairment, known as cerebellar cognitive affective syndrome (CCAS), may be present in cerebellar disorders. This study identified distinct CCAS subtypes in cerebellar patients using cluster analysis. METHODS: The German CCAS-Scale (G-CCAS-S), a brief screening test for CCAS, was assessed in 205 cerebellar patients and 200 healthy controls. K-means cluster analysis was applied to G-CCAS-S data to identify cognitive clusters in patients. Demographic and clinical variables were used to characterize the clusters. Multiple linear regression quantified th
