Alzheimer's disease (AD) patients are particularly vulnerable to pneumonia and subsequent respiratory failure due to neurodegeneration-induced dysphagia and immunosenescence. Accurate prediction of mechanical ventilation (MV) need in this population remains a critical but unmet clinical challenge. This retrospective cohort study utilized the MIMIC-IV database to identify 793 critically ill AD patients diagnosed with pneumonia. Clinical variables were extracted and filtered using LASSO regression and multivariate logistic regression to identify key predictors. Five machine learning algorithms,

Motor skill learning is the process of developing new movements with practice until they can be performed automatically. This necessitates the interaction of high-level cognitive processes with low-level sensorimotor mechanisms. Skill learning involves not only changes in the motor system but also proprioception (position sense). Proprioceptive deficits increase variability and decrease accuracy of movement. The somatosensory cortex, where low-level proprioception is processed, is known to play a role in motor skill learning, but the involvement of higher-level proprioceptive regions is unclea

BACKGROUND: It is unknown whether brain-based predictive models derived from sleep features are useful for the clinical diagnosis of major depressive disorder (MDD).
METHODS: Using resting-state functional magnetic resonance imaging data from the curated ABCD (Adolescent Brain Cognitive Development) Study Data Release 3.0, we trained a connectome-based predictive model (CPM) on 35,778 pairwise connections (Pearson's r) from 2349 (237 participants with at least 1 psychiatric disorder and 2112 control participants) participants ages 11 to 12 to predict sleep duration (measured from a Fitbit tra

Parkinson's disease (PD) is a neurodegenerative disease that affects both the motor and nonmotor functions of an individual and is more prevalent in older adults. PD is preceded by an early stage called prodromal PD, which starts very early before the typical symptoms of the disease appear. If patients are managed and diagnosed at this initial stage, their quality of life can be maintained. Magnetic Resonance Imaging (MRI) is a widespread approach in neuroimaging that is very helpful in the diagnosis of brain-related diseases. Current studies of PD classification mostly use T1-weighted MRI or

INTRODUCTION: In low-income countries, clinicians trained through a context-specific trauma surgery fellowship program (TFP) can help reduce injury-related mortality to levels closer to those observed in higher-resource settings. Successful implementation, however, hinges on buy-in from local clinicians. We assessed clinician support for a potential TFP in Uganda, considering perceived need, curricular recommendations, barriers, and motivating factors.
METHODS: After cognitive interviews with experts and questionnaire pilot testing, we cross-sectionally surveyed Ugandan consultants (general s

BACKGROUND: Worldwide, populations are ageing. Together with improved home care and limited places in residential aged care facilities (RACF), this leads to people being admitted to RACF at more advanced ages and in increasingly fragile states of health, often with cognitive impairment. Advance directive completion rates are low, so many people are admitted to RACFs without having formally stated their wishes for care. Guidelines for advance care planning (ACP) in nursing homes exist, however they focus primarily on residents who maintain decision-making capacity. Programs are needed for peopl

BACKGROUND: Endoscopic peroral pyloromyotomy for medically refractory gastroparesis requires advanced endoscopic skill. This study describes … minimal time and outcome differences between in-person and video-based proctoring. Steps requiring cognitive training unique to endoscopic surgery (mucosotomy and tunneling) and standard myotomy had no statistical difference. This study supports an integrated in-person and video-proctoring framework as a safe and effective method to introduce advanced endoscopic su …

Biomarkers that aid in early detection of neurodegeneration are needed to enable early symptomatic treatment and enable identification of people who may benefit from neuroprotective interventions. Increasing evidence suggests that sleep biomarkers may be useful, given the bi-directional relationship between sleep and neurodegeneration and the prominence of sleep disturbances and altered sleep architectural characteristics in several neurodegenerative disorders. This study aimed to demonstrate that sleep can accurately characterize specific neurodegenerative disorders (NDD). A four-class machin

After stroke rehabilitation, patients need to reintegrate back into their daily life, workplace and society. Reintegration involves complex processes depending on age, sex, stroke severity, cognitive, physical, as well as socioeconomic factors that impact long-term outcomes post-stroke. Moreover, post-stroke quality of life can be impacted by social risks of inadequate family, social, economic, housing and other supports needed by the patients. Social risks and barriers to successful reintegration are poorly understood yet critical for informing clinical or social interventions. Therefore, the

BACKGROUND: In Parkinson's disease (PD), gait and balance is impaired, relatively resistant to available treatment and associated with falls and disability. Predictive models of ambulatory progression could enhance understanding of gait/balance disturbances and aid in trial design.
OBJECTIVES: To predict trajectories of ambulatory abilities from baseline clinical data in early PD, relate trajectories to clinical milestones, compare biomarkers, and evaluate trajectories for enrichment of clinical trials.
METHODS: Data from two multicenter, longitudinal, observational studies were used for mod

Parkinson's Disease (PD) is the second most common age-related neurological disorder that leads to a range of motor and cognitive symptoms. A PD diagnosis is difficult since its symptoms are quite similar to those of other disorders, such as normal aging and essential tremor. When people reach 50, visible symptoms such as difficulties walking and communicating begin to emerge. Even though there is no cure for PD, certain medications can relieve some of the symptoms. Patients can maintain their lifestyles by controlling the complications caused by the disease. At this point, it is essential to

Activity of hippocampal pyramidal cells is critical for certain forms of learning and memory, and work from our lab and others has shown that CA2 neuronal activity is required for social cognition and behavior. Silencing of CA2 neurons in mice impairs social memory, and mice lacking Regulator of G-Protein Signaling 14 (RGS14), a protein that is highly enriched in CA2 neurons, learn faster than wild types in the Morris water maze spatial memory test. Although the enhanced spatial learning abilities of the RGS14 KO mice suggest a role for CA2 neurons in at least one hippocampus-dependent behavio

When Huntington's disease (HD) patients are tested on cognitive tasks that involve both striatal and hippocampal memory systems, a decline in their striatal function is compensated for by an increase in hippocampal activity that allows these patients to achieve an optimal performance [Voermans, N. A behavioral technique often used to evaluate the cognitive performance of rats and mice is the fear conditioning paradigm. During conditioned fear experiments, freezing responses shown by rodents after exposure to environmental stimuli previously paired to an aversive experience provide a behavioral index of the animal's associative abilities. The present study examined the ability of a computer-controlled automated Freeze Monitor system for recording immobility behavior in mice. The sensitivity of the automated procedure to detect group differences caused by the application of various traini

A field that uses mitochondria to unify biological and psychological sciences and to better understand the basis of health has emerged-Mitochondrial Psychobiology. Mitochondria are the body's cellular hubs for energy transformation and signaling. Mitochondria regulate functions as diverse as metabolism, gene expression, and stress adaptation; consequently, their dysregulation is linked to most chronic illnesses and aging. Here, we propose that mitochondria provide a mechanistic bridge between body and the measurable functions of the mind, including perception, cognition, emotion, and behavior.

INTRODUCTION: Mixed pathology of Alzheimer's disease (AD) and dementia with Lewy bodies (DLB) are frequently observed in patients with cognitive impairment, and complicate clinical diagnosis. We aimed to develop a classification model using 18F-fluorodeoxyglucose (FDG) positron emission tomography (PET) to improve diagnostic accuracy for these challenging cases.
METHODS: We analyzed FDG-PET images from 277 participants who were categorized into AD, DLB, mixed disease, and healthy control (HC) groups. Deep learning-based classification models were trained on seven binary classification tasks a

Functional neurological disorder (FND) is a prevalent yet underrecognized cause of neurological disability and one of the most common reasons for neurology referral, yet access to structured rehabilitation pathways remains limited. We present the case of a 63-year-old woman who developed persistent dissociative seizures and disabling multi-domain symptoms in acute care following traumatic brain injury. Following transfer to an inpatient acquired brain injury rehabilitation unit, physiatrist-led diagnostic clarification, interprofessional team training and structured interdisciplinary patient a

Artificial intelligence and neuroimaging enable accurate dementia prediction but often involve 'black box' models that can be difficult to trust. Explainable artificial intelligence (XAI) aims to provide insights into the model's decisions; however, choosing the most appropriate method is non-trivial and often context-specific. We used T1-weighted MRI to train models on two tasks: Alzheimer's disease (AD) classification (diagnosis) and predicting conversion from mild-cognitive impairment (MCI) to all-cause dementia (prognosis). We applied eleven XAI methods across two popular image classificat
