Lateral femoral cutaneous nerve transposition: Renaissance of an old concept in the light of new anatomy.

Meralgia paresthetica causes pain in the anterolateral thigh. Most surgical procedures involve nerve transection or decompression. We conducted a cadaveric study to determine the feasibility of lateral femoral cutaneous nerve (LFCN) transposition. In three cadavers, the LFCN was exposed in the thigh and retroperitoneum. The two layers of the LFCN canal superficial and deep to the nerve were opened. The nerve was then mobilized medially away from the ASIS, by cutting the septum medial to sartorius. It was possible to mobilize the nerve for 2 cm medial to the ASIS. The nerve acquired a much stra

Compression neuropathy of the common peroneal nerve secondary to a ganglion cyst.

There are various causes of the common peroneal nerve palsy. However, common peroneal nerve palsy caused by ganglia are uncommon. We hereby present a case of a 55-year-old man with a 1 week history of foot drop and swelling in the region of the right leg. Physical examination and nerve conduction study studies confirmed a diagnosis of common peroneal nerve palsy. Magnetic resonance imaging (MRI) revealed a lobulated, elongated cystic-appearing mass anterior to the head of fibula. Surgical decompression of the nerve with removal of the mass was performed. Surgical pathology reports confirmed th

Case report: Congenital dacryocystocele and dacryocystitis.

Congenital dacryocystocele is a uncommon type of nasolacrimal duct obstruction. Differential diagnosis for masses in the medial canthal region of a newborn include encephalocele, hemangioma, nasal glioma, and dermoid cyst. Because of the risk of becoming infected (acute dacryocystitis) and potentially lethal due to septicemia, aggressive management, including admission for intravenous antibiotics and surgical removal, is now advocated by many pediatric ophthalmologists if the cyst cannot be decompressed. Because of the commonly associated nasal cyst, infants with nasolacrimal dacryocyctocele m

Isolated primary intradural extramedullary spinal neurocysticercosis: a case report and review of literature.

BACKGROUND: In spite of being the most common parasitic infestation of central nervous system (CNS), spinal cysticercosis remains a rare entity. CASE REPORT: We report an unusual case of a 45-year-old-male with primary isolated localization of spinal intradural extramedullary cysticercosis at thoracic 3/4 level. The lesion was surgically addressed to decompress the cord in combination with administration of oral albendazole. The weakness improved after treatment but the pain and numbness persisted. The available treatment options, diagnostic strategies and the pathophysiology of this rare con

Thoracic outlet syndrome.

Thoracic outlet syndrome (TOS) is the term commonly used to describe patients with symptoms attributed to compression of the brachial plexus and subclavian vein and artery in the region of the thoracic inlet/outlet. TOS remains extremely controversial with respect to its existence, diagnosis, conservative management, and surgical treatment. The diagnosis is based upon clinical evaluation, reproduction of patient symptoms with arm elevation, and the absence of other relevant pathology. Conservative management should be directed towards correction of postural abnormalities and muscle imbalance i

[Degenerative spondylolisthesis].

Results of surgical treatment for degenerative spondylolisthesis in 31 patients are presented. Disabling lumbar and radicular pain not responding to conservative treatment was the indication for surgery in all cases. Twenty-six patients underwent neural decompression and segmental fusion; in 5 patients with no radicular symptoms fusion alone has been performed. In 15 patients with partially preserved intervertebral disc the fusion embraced the pedicle and processus transversus, in other 15 with damaged disc posterior fusion was done and in one case intervertebral fusion was performed. In 7 cas

[The role of surgery in acute epididymitis].

One third of the acute épididymitis (A.E.) admitted in the Military Hospital in Marseilles have been operated, i.e. 33 cases; out of them the last 12 cases have been systematically monitored by ultrasonography. Twice out of three times, clinical picture suggested a possible twisting that led to an emergency surgical exploration. Interest of ultrasonography of the scrotum is underlined as far as diagnosis is concerned but also in monitoring A.E. Indeed, despite medical treatment (antibiotics + anti-inflammatory drugs), it is possible that some A.E. are evolving either to abscess or to their "va

Acute respiratory arrest. A complication of malformation of the shunt in children with myelomeningocele and Arnold-Chiari malformation.

Three children had hydrocephalus associated with myelomeningocele and the Arnold-Chiari malformation. They all experienced neck pain and opisthotonos, followed by the sudden onset of respiratory arrest as a result of the malfunction of a CSF shunt. There were no changes in pupillary response, and each patient remained alert until shortly before the respiratory arrest. Prompt surgical revision of the CSF shunt was carried out, and all three patients recovered. The mechanism of acute respiratory arrest seems to be acute compression of the brainstem as a result of hydrocephalus. The supratentoria

Hybrid Paraclavicular Decompression for Venous Thoracic Outlet Syndrome with Primary Endovascular Reconstruction.

BACKGROUND: Venous thoracic outlet syndrome (vTOS) is characterized by severe stenosis and potential thrombosis of the axillary-subclavian vein (effort thrombosis) with significant effects on patient mobility, quality of life, and risks associated with possible anticoagulation. Treatment goals are aimed at symptomatic improvement and freedom from recurrent thrombosis. To date, there exist no clear protocols or recommendations on surgical approach that result in optimal outcomes. We highlight our institution's experience with a systematized, paraclavicular approach with intraoperative balloon a

Radical decompression without fusion for L5 radiculopathy due to foraminal stenosis.

BACKGROUND: The highest incidence of lumbar foraminal stenosis (LFS) occurs in the L5-S1 segment and its anatomical features differ from those of other segments. Few previous reports have exhaustively assessed surgical outcomes after decompression surgery, limiting the materials to patients with LFS at the L5-S1 segment. We aimed to prospectively investigate instability and neurological improvement following our novel surgical technique for LFS at L5-S1, named "radical decompression" of the nerve root. METHODS: Patients with foraminal stenosis at L5-S1 who underwent surgery using our techniqu

Outcome of posterior decompression for spinal epidural lipomatosis.

BACKGROUND: In contrast to osteoligamentous lumbar stenosis (LSS), outcome of surgical treatment for spinal epidural lipomatosis (SEL) is still not well defined. We present risk factors for SEL and clinical long-term outcome data after surgical treatment for patients with pure SEL and a mixed-type pathology with combined SEL and LSS (SEL+LSS) compared to patients with pure LSS. METHODS: From our prospective institutional database, we identified all consecutive patients who were surgically treated for newly diagnosed SEL (n = 31) and SEL+LSS (n = 26) between 2018 and 2022. In addition, a match

Successful use of stereotactic navigation in posterior spinal fusion T10-S2 with bilateral iliac screw fixation in a patient with prior spinal surgeries and osteoporosis: A case report.

INTRODUCTION AND IMPORTANCE: Degenerative lumbar scoliosis is a prominent cause of adult spinal deformity with an increasing prevalence as the population ages. This pathology is associated with debilitating symptoms, including radicular back pain and lower extremity claudication. Surgical realignment of the spine and restoration of sagittal imbalance can reduce low back pain. Chronic sacroiliac dysfunction commonly causes low back radicular pain. We present a complicated case where stereotactic navigation facilitated an extensive fusion and decompression procedure for adult spinal deformity in
