UNLABELLED: Hydroxyethyl starch (HES) solutions are effective plasma volume expanders. Impairment of coagulation occurs with large HES volumes infused perioperatively. The landscape of acute migraine medication has changed fundamentally in recent years. It has been a gradual, often unnoticed process characterized less by spectacular introduction of new substances than by changes in patients' access to medications and individualized selection of treatment adapted to the patients' needs. Four triptans are now available over-the-counter in Germany which has a major influence on self-medication. The main new introduction was the 5HT1F-agonist lasmiditan as an alternative to triptans.

The German living donor register Safety of the Living Kidney Donor - The German National Register (SOLKID-GNR) collects data of the medical and psychosocial outcome of living kidney donors. For the first time in Germany, a prospective data collection allows a scientifically based long-term analysis of how a living kidney donation influences the psychological and physical health of living kidney donors. This will contribute directly to improve the information and care of living kidney donors.

This article is a review of the proceedings of the 34th Annual Meeting of the German Society for Vascular Surgery and Vascular Medicine held in Bonn, Germany, from October 17 to 20, 2018. The programme of the Meeting contained reports concerning treatment of aortic dissections and aneurysms, lower-limb ischaemia, management of wounds in vascular pathology, infection in vascular surgery, carotid artery diseases, venous system pathology, vascular biology, as well as vascular access in patients on programmed haemodialysis.

A 62-year-old man presented to our department with abdominal pain and diarrhea for 3 weeks on a background of previous branched endovascular repair for a thoracoabdominal aneurysm. A triple-phase computed tomography scan of his abdomen and pelvis showed a large aortocaval fistula caused by a type III endoleak from a dislodged superior mesenteric artery stent. He was successfully treated with a BeGraft (Bentley Innomed, Hechingen, Germany) by using an endovascular technique.

During paleoparasitological analyses on several Neolithic sites in Switzerland (Arbon-Bleiche 3) and southwestern Germany (Hornstaad-Hörnle I, Torwiesen II, and Seekirch-Stockwiesen), numerous eggs of Diphyllobothrium sp. were recovered. This is one of the earliest occurrences of this parasite during the prehistoric period in the Old World. The prevalence of this helminth in the samples studied raises the question as to how important parasitic diseases were during the Neolithic period and what their actual consequences were.

During 1998-2000 a quality assurance program for diagnosis and treatment of depression was conducted in 24 hospitals for psychiatry and psychotherapy in Baden-Wurttemberg (southern Germany). Process and outcome quality of 3,000 depressive patients was documented at admission and discharge. The article focuses on therapeutic measures, duration, outcome, patient satisfaction, and their interactions. The results show that the patients' satisfaction with the care received is very high and the pre-post effect sizes of inpatient treatment for depression are high.

Although the tuberculosis rate in Germany and in the Western industrialized nations is stable, it is still one of the most significant infectious diseases worldwide. Immigrants and asylum seekers make up a large proportion of new cases, and with 15%, the occurrence of multidrug-resistant tuberculosis patients from the former Soviet Union is above average. This study, describing a patient with ovarian tuberculosis, undertakes a classification of risk groups as well as presents the potential differential diagnosis and its limitations.

A case of systemic mycosis due to a Rhizopus sp. infection is described in a dead-stranded, 10-yr-old, male harbor porpoise (Phocoena phocoena) found on the beach of Neustadt, Schleswig-Holstein on the Baltic Sea (Germany). At necropsy, granulomatous mycotic lesions in brain, lung, kidneys, testis, and draining lymph nodes were found. In addition, a focal ulcerative gastritis of the first stomach, due to a nematode infection, was present and is suspected to be the portal of entry for the fungus.

After initial false diagnoses cutaneous leishmaniasis of the lids was diagnosed in a Portuguese woman and a Turkish man, both living and working in Germany. This paper describes the clinical pictures, histological and electron-microscopic findings in the two cases. Factors which may lead to false diagnosis are also discussed. It is pointed out that false diagnoses can be avoided best by taking a detailed and accurate history and identifying the pathogens as early as possible in smear cultures or by electron microscopy.

The German Society of Endocrinology (DGE) has a long-standing scientific and clinical focus on the pituitary. The pituitary working group 'Arbeitsgemeinschaft Hypophyse' is an interdisciplinary special interest group with a focus on advancing diagnosis and treatment of pituitary conditions. On the occasion of the English publication of the S2K clinical guideline Diagnosis and therapy of clinically hormonally inactive pituitary tumors, we present here a series of 12 articles from internationally renowned authors from inside and outside of Germany.

A 19-year-old woman presented with liver capsule pain and a liver lesion on sonography, which contained microvesicular cystic, necrotic and solid fibrotic formations typical for alveolar echinococcosis (AE). The diagnosis was confirmed by serology and histopathology. This parasitic infection which is endemic in Germany is feared because of its malignant growth. The increasing expansion of E. multilocularis in Europe will lead to a higher incidence of AE with an occurrence of cases outside classical endemic regions.

In Germany the numbers of penetrating thoracic injuries are rare compared to Anglo-American countries; however, the number of cases has increased in recent years due to an increase in violent incidents. This article reports the case of a patient who suffered such a penetrating thoracic injury after a domestic accident. Operative treatment was performed according to the well-established standard treatment algorithms. Contrary to the initial assessment of the emergency doctor, substantially more severe injuries were found.

A DNA variant, rs734232, altering a RUNX1 binding site was recently reported as susceptibility allele at PSORS2 (17q25) in cohorts of psoriasis patients from the US. A testing of this variant in psoriasis patients from Germany did not confirm this association in 300 trios nor in two case-control studies with 281 patients with psoriasis vulgaris and 375 patients with psoriatic arthritis, respectively. These results fail to support rs734232 as a psoriasis susceptibility factor in German psoriasis patients.

In view of current events the question is discussed to what extent the risk of bioterrorism may be an issue relevant for forensic medicine also in Germany, although at present there seems to be no concrete threat. The cases which became known so far were either false alarms or foolish pranks (copycats), which have to be, and are indeed, prosecuted by the state (section 126 German Criminal Code). Reference is also made to the measures of disinfection recommended by the Robert Koch Institute.

Based on iodine deficiency in Germany autonomous functioning nodules develop and are loaded with the risk of hyperthyroidism. Even subclinical hyperthyroidism has health implications including general symptoms, diseases of the cardiovascular system (atrial fibrillation, risk of thrombembolism, stroke and other embolic events) and a reduction of axial bone mineral density. Fortunately functional nodules have a very low probability of being malignant. However, there exist a coexistence of thyroid carcinoma outside the hot nodule.

In order to elucidate the circumstances of sexual crimes, the forensic laboratory is increasingly forced to consider whether the offender has used a condom. The detection presented here depends on Lycopodium spores used in several condom coatings. They can easily be detected e.g. in vaginal swabs. A screening of 40 various condoms available in Germany revealed that 32 (= 80%) were coated with Lycopodium spores. The microscopical investigation of the spores is explained and some sources of error in interpretation are discussed.

A short review of deficiencies in immune reactions in older adults resulting in an increase of vaccine-preventable diseases (e.g. influenza) in this age group is presented. Reasons for so-called vaccine failures (primary, secondary) in persons previously vaccinated against measles are discussed. The immune status against poliomyelitis of the population in Germany, based on repeated serosurveillance studies is presented, the immunity of persons previously TOPV-vaccinated against a poliovirus type 3 variant (Finland) is demonstrated.

A comparative study was carried out on the psychological situation of students in East and West Germany after the political changes of 1989. Results concerning the direction of the locus of control are reported. East german students show a higher internal and external locus of control. This, as internal attributions are concerned, unexpected constellation is discussed under aspects of the specific political situation, correlations with signs of adjustment disorders and possible inherent menace to the psychological equilibrium of the east german students.

Fulminant hepatic failure is a relatively rare disease. It develops in approximately 400 patients per year in the Federal Republic of Germany after viral hepatitis, toxic hepatitis or hypoxia of the liver. The mortality ranges between 80 - 90%. Therapy must start early. It consists mainly of intensive conservative care. Of the variety of invasive therapeutic measures only baboon liver perfusion and plasma exchange applied early in the disease seem to be of any benefit.
