Heterogenous patterns of sensory dysfunction in postherpetic neuralgia suggest multiple pathophysiologic mechanisms.

BACKGROUND: Postherpetic neuralgia (PHN) is considered by some investigators to be predominantly a deafferentation-type central pain syndrome; others suggest that activity of remaining peripheral nociceptors plays a critical role. The authors investigated the sensory dysfunction in subjects with PHN of varying duration and at different sites to gain further insight into the mechanisms responsible for the clinical features of neuropathic pain. In addition, the relationships between ongoing pain and pain evoked by mechanical and thermal stimuli were compared in patients with trigeminal and trunc

[Effect of Astragali Radix-Curcumae Rhizoma compatibility combined with 5-fluorouracil on Th17/Treg balance and tumor-related mRNA and protein expression in orthotopic xenograft model mice of CT26.WT colorectal carcinoma].

Astragali Radix-Curcumae Rhizoma(AR-CR) is a combination commonly used in the clinical treatment of tumors. Based on the T helper 17(Th17)/regulatory T cell(Treg) balance, the present study explored the possible mechanism of AR-CR combined with 5-fluorouracil(5-FU) on the tumor growth of orthotopic xenograft model mice of colorectal carcinoma. Ninety male BALB/c mice were randomly divided into nine groups, i.e., a blank group, a model group, a 5-FU group, high-, medium-, and low-dose AR-CR(2∶1) groups, and high-, medium-, and low-dose AR-CR+5-FU groups, with 10 mice in each group. The orthotop

Evaluation of femoral perfusion using dynamic contrast-enhanced MRI after simultaneous initiation of electrical stimulation and steroid treatment in an osteonecrosis model.

This study aimed to evaluate femoral perfusion using dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) for two weeks after the simultaneous initiation of electrical stimulation (ES) and steroid treatment in a steroid-induced osteonecrosis (ON) model. A single dose of methylprednisolone was injected into 14 rabbits. Seven rabbits underwent ES (ES group), and seven rabbits did not (control group). DCE-MRI was performed before steroid administration and 1, 5, 10, and 14 days after steroid administration. Regions of interest were set in the bilateral proximal femora. The enhancement r

Functionalized carbon nanotubes as nanoinhibitors of asphaltene aggregation: a molecular dynamics approach for enhanced oil recovery.

CONTEXT: Asphaltene aggregation and deposition pose major challenges in crude oil transport and processing. While extensive experimental studies have explored mitigation strategies, nanomaterials-particularly carbon nanotubes (CNTs)-have emerged as promising inhibitors due to their ability to adsorb asphaltenes and limit their clustering. However, experimental approaches provide limited insight into molecular-scale mechanisms. To address this, molecular dynamics (MD) simulations were performed to investigate interactions between model asphaltenes and CNTs in a toluene/heptane solvent. Five sys

Exploring the empowerment process in hip arthroplasty patients: Insights from rehabilitation nursing.

BACKGROUND: Population ageing has led to an increased incidence of hip osteoarthritis, a condition that frequently requires surgical intervention to restore functionality and relieve pain. Hip Arthroplasty (HA) constitutes a major therapeutic option; however, it may entail physical and emotional repercussions for individuals undergoing this procedure. In this context, individualized follow-up care is essential, with the Rehabilitation Nurse Specialist (RNS) assuming a central role in promoting autonomy and functional recovery. OBJECTIVE: To analyze Rehabilitation nurses' perceptions of the em

Early functional changes in lewy body dementia: roles of dynamics, locus coeruleus, and compensation.

Dementia with Lewy bodies (DLB) is marked by multidomain cognitive impairments, with fluctuations in cognition and alertness being among the most common clinical features. Disruptions in functional connectivity are thought to underlie these fluctuations, but it remains unclear whether such patterns are already present at prodromal stages. We investigated the presence of static and dynamic functional connectivity alterations and their contribution to the clinical phenotype of prodromal DLB, and explored their association with declining locus coeruleus integrity, while considering premorbid inte

Effect of Sex and Sporting Discipline on LV Adaptation to Exercise.

OBJECTIVES: This study sought to investigate the effect of different types of exercise on left ventricular (LV) geometry in a large group of female and male athletes. BACKGROUND: Studies assessing cardiac adaptation in female and male athletes indicate that female athletes reveal smaller increases in LV wall thickness and cavity size compared with male athletes. However, data on sex-specific changes in LV geometry in athletes are scarce. METHODS: A total of 1,083 healthy, elite, white athletes (41% female; mean age 21.8 ± 5.7 years) assessed with electrocardiogram and echocardiogram were con

Ipsilateral fractures of the femur and tibia: treatment with retrograde femoral nailing and unreamed tibial nailing.

Between 1989 and 1995 a total of 47 patients with 50 fractures of the ipsilateral femoral and tibial shafts without significant articular involvement were treated at Tampa General Hospital. Within this group were 24 patients with 26 fractures who were treated with intramedullary fixation of both bones using a technique of retrograde insertion of a femoral nail and unreamed insertion of an interlocking tibial nail. Information concerning the injuries, treatments, and much of the follow-up was gleaned from a trauma registry. Five of the femoral fractures (19%) and 14 of the tibial fractures (54%

Health-related outcomes with supervised exercise and myofascial release versus only supervised exercise in subacromial pain syndrome: a randomized controlled single-blind study.

BACKGROUND: Myofascial tissue plays a critical role in shoulder joint mobility disorders. Myofascial release therapy (MFR) is frequently utilized to restore the extensibility of fascial tissue and is considered beneficial for various clinical conditions such as low back pain and ankle injuries. However, no studies have yet evaluated the effects of MFR on periscapular muscles activation and shoulder mobility in patients with subacromial pain syndrome(SAPS). OBJECTIVE: The purpose of this study was to compare the effectiveness of MFR combined with supervised exercise(SE) and SE alone in patient

A Novel Protein Corona-Based Secretome Profiling Assisting Cell Multiomics Analysis.

Multiomics analysis integrates complementary molecular layers to decode biological complexity; however, the secretome, representing the extracellular and dynamic response of cellular function, remains largely absent from existing multiomics frameworks. Secretome profiling has been constrained by high background interference, poor sensitivity for low-abundance proteins, and perturbations caused by conventional enrichment strategies. Here, we develop a protein corona-based secretome profiling strategy and incorporate it into a multiomics analytical framework. By exploiting the selective adsorpti

How Survivors of Colorectal Cancer Experience ctDNA Testing and Fear of Recurrence.

BACKGROUND: Circulating tumor DNA (ctDNA) is a novel prognostic biomarker indicating minimal residual disease and high recurrence risk. In three randomised DYNAMIC trials, ctDNA was used to guide adjuvant chemotherapy (ACT) for early-stage colorectal cancer. While clinical applications of ctDNA are being established, the psychosocial impact, particularly on fear of cancer recurrence (FCR), is less understood. OBJECTIVE: To explore the psychosocial impact, especially FCR, of ctDNA-informed ACT decisions among a subset of DYNAMIC trial participants. METHODS: A qualitative study using semi-stru

Gut-Liver Axis Mechanisms Underlying Spontaneous Reversal of Liver Fibrosis: A Gut Microbiota-Metabolomics Analysis.

Background: The reversal of liver fibrosis is crucial for improving outcomes in chronic liver disease. The gut-liver axis, mediated by the intestinal microbiota, plays a significant role in this process. However, its dynamic changes and mechanisms during reversal remain unclear. This study aimed to systematically reveal these dynamics and explore the link between gut microbiota and metabolism in a spontaneous reversal model. Methods: Intestinal contents were collected from mouse model groups (fibrosis, 4-week reversal, and 12-week reversal). The use of 16S rRNA gene sequencing was employed to

Dose reduction for synaptic density PET imaging in Parkinson's disease.

OBJECTIVE: Dynamic PET imaging with 11C-UCB-J enables in vivo quantification of synaptic vesicle glycoprotein 2A (SV2A), with prior reports of lower synaptic density in areas such as the brainstem nuclei and substantia nigra (SN) in Parkinson's disease (PD). Lowering PET dose reduces radiation exposure but increases noise and compromises quantification. This study evaluated a self-supervised two-step deep image prior (TS-DIP) denoising method for SV2A PET using 1/10 of the standard dose. METHODS: Thirty healthy controls (HCs) and 30 PD patients underwent 60-minute PET scans to acquire full-co

Functional connectivity alterations in post-traumatic stress disorder: A resting-state fMRI study of brain network dysfunction.

INTRODUCTION: Emerging evidence positions post-traumatic stress disorder (PTSD) as a disease fundamentally involving disturbances in whole-brain network coordination. Nonetheless, comprehensive studies systematically integrating time-invariant connectivity metrics with time-resolved dynamic assessments remain limited. METHOD: This case-control investigation enrolled 33 PTSD patients and 33 healthy controls who completed resting-state functional magnetic resonance imaging and cognitive assessment using the Addenbrooke's Cognitive Examination-III. We evaluated static connectivity via functional

FAD-YOLO: a lightweight feature-refined and task-aligned framework for AIS-MIA discrimination on pulmonary CT.

INTRODUCTION: Adenocarcinoma in situ (AIS) and minimally invasive adenocarcinoma (MIA) typically present as ground-glass nodules (GGNs) on pulmonary computed tomography (CT), and their low contrast, blurred boundaries, and morphological variability pose substantial challenges to automated detection. METHODS: Using YOLO12n as the baseline, we propose FAD-YOLO (Feature-refinement, Alignment, and Dynamic-sampling YOLO), a lightweight yet accurate detection framework with three task-oriented improvements: an A2C2f-FRFN module that replaces the standard feed-forward network within the Area-Attenti

Decoding neuronal criticality firing patterns for large brain based EEG models.

The critical brain hypothesis suggests that the brain operates near critical points, balancing order and disorder for optimal processing via self-organized neural firing patterns. However, important gaps remain in understanding criticality in human in vivo brain dynamics using electroencephalography (EEG), particularly in translating these systems neuroscience concepts to operational applications (e.g., cognitive state detection, human-machine teaming). We address three key aspects that yet remain to be demonstrated: (1) how to decode large-scale EEG neural recordings coordination and self-org
