Gait Rehabilitation for Early rheumatoid Arthritis Trial (GREAT): lessons learnt from a mixed-methods feasibility study and internal pilot trial.

BACKGROUND: People with rheumatoid arthritis experience foot and lower limb pain due to active synovitis, resulting in impaired lower limb function. Earlier intervention may help with prevention of functional decline. The aims of this research were to develop and evaluate a new gait rehabilitation intervention for people with early rheumatoid arthritis, evaluate its feasibility, and to test whether or not gait rehabilitation plus usual care is more clinically and cost-effective than usual care alone. DESIGN AND METHODS: We undertook a single-arm, repeated-measures, pre- and post-intervention,

Barriers to pregnancy prevention for adolescents in rural Haiti: perceptions of healthcare providers.

BACKGROUND: Little is known about healthcare providers' (HCPs) contraceptive views for adolescents in Haiti, who experience high rates of unintended pregnancy. We sought to describe HCPs' perspectives on barriers and facilitators to contraceptive care delivery in rural Haiti. METHODS: We conducted a cross-sectional survey and qualitative interviews with HCPs in two rural communities in Haiti from 08/2021-03/2022. We assessed demographics, clinical practice behaviors and explored contraception perspectives according to Theory of Planned Behavior constructs: attitudes, subjective norms, and per

Dietary strategies for chemotherapy-induced nausea and vomiting: A systematic review.

BACKGROUND & AIMS: Chemotherapy-induced nausea and vomiting (CINV) is one of the most distressing cancer treatment side effects, affecting 20-70% of patients despite routine antiemetic prescription. Although dietary modifications are routinely recommended in clinical practice, there is lack of data synthesis to determine which dietary strategies for managing CINV are supported by quality evidence. This systematic review was conducted to examine the effect of dietary strategies on incidence and severity of CINV in adults compared with no intervention, usual care, or alternative strategies. MET

Real-world persistence and costs among patients with chronic migraine treated with onabotulinumtoxinA or calcitonin gene-related peptide monoclonal antibodies.

BACKGROUND: Chronic migraine (CM) is a common neurologic disorder that imposes substantial burden on payers, patients, and society. Low rates of persistence to oral migraine preventive medications have been previously documented; however, less is known about persistence and costs associated with innovative nonoral migraine preventive medications. OBJECTIVE: To evaluate real-world persistence and costs among adults with CM treated with onabotulinumtoxinA (onabotA) or calcitonin gene-related peptide monoclonal antibodies (CGRP mAbs). METHODS: This was a retrospective, longitudinal, observational

Assessment of an Innovative Mobile Dentistry eHygiene Model Amid the COVID-19 Pandemic in the National Dental Practice-Based Research Network: Protocol for Design, Implementation, and Usability Testing.

BACKGROUND: Amid COVID-19, and other possible future infectious disease pandemics, dentistry needs to consider modified dental examination regimens that render quality care, are cost effective, and ensure the safety of patients and dental health care personnel (DHCP). Traditional dental examinations, which number more than 300 million per year in the United States, rely on person-to-person tactile examinations, pose challenges to infection control, and consume large quantities of advanced-level personal protective equipment (PPE). Therefore, our long-term goal is to develop an innovative mobil

Global, regional, and national disability-adjusted life-years (DALYs) for 333 diseases and injuries and healthy life expectancy (HALE) for 195 countries and territories, 1990-2016: a systematic analysis for the Global Burden of Disease Study 2016.

BACKGROUND: Measurement of changes in health across locations is useful to compare and contrast changing epidemiological patterns against health system performance and identify specific needs for resource allocation in research, policy development, and programme decision making. Using the Global Burden of Diseases, Injuries, and Risk Factors Study 2016, we drew from two widely used summary measures to monitor such changes in population health: disability-adjusted life-years (DALYs) and healthy life expectancy (HALE). We used these measures to track trends and benchmark progress compared with e

Smartphone-Based Ecological Momentary Assessment to Monitor Opioid Use and Overdose Among People Who Use Opioids: Prospective Observational Feasibility Study.

BACKGROUND: Opioids account for 76% of drug overdose deaths in the United States, with nearly 80,000 opioid overdose deaths annually. The risk of overdose is dynamic and influenced by rapidly changing behaviors and contexts that are not well captured by retrospective or infrequent assessments. Ecological momentary assessment (EMA) allows repeated near real-time reporting of behaviors and experiences in natural settings. OBJECTIVE: This study evaluated the feasibility and acceptability of smartphone-based EMA among people who use opioids to monitor opioid use, overdose experiences, and naloxon

Testing the effectiveness of the Responsible, Engaged, and Loving Fathers (REAL Fathers) intervention for improving early childhood development and reducing family violence in Uganda: Study protocol for a cluster-randomized controlled trial.

BACKGROUND: Witnessing and experiencing violence impedes children's healthy development and learning, inhibits positive relationships, provokes low self-esteem and emotional distress, and can lead to self-harm and aggressive behavior across the life course. Evidence-based programs are needed that incorporate violence prevention strategies alongside methods to improve wellbeing and healthy development for children and their families. This trial evaluates the Responsible, Engaged, and Loving Fathers (REAL Fathers) intervention, a Ugandan-led multilevel community-based mentoring program for young

The efficacy of BLS training among fifth-year medical students-a randomized, assessor-blinded, parallel group trial.

BACKGROUND: Proper basic life support (BLS) skills are crucial for laypeople and health care professionals to increase the survival of cardiac arrest patients. A practical examination at the end of a BLS course may be beneficial for prolonging skill retention. We aimed to investigate the efficacy of our BLS training and the effect of BLS practical examinations on skill retention among fifth-year medical students compared with the effect of additional practice and continuous assessment. METHODS: In this randomized, assessor-blinded, parallel group study, fifth-year medical students took a prac

Network analysis of anxiety, depression and insomnia in the elderly in Jiangsu Province.

INTRODUCTION: HIV in the United States is concentrated in the South, an impoverished region with marked health disparities and high rates of incarceration, particularly among African Americans. In the Deep South state of Alabama, a policy directive to reduce prison overcrowding has diverted large numbers of convicted felons to community supervision. Probation and parole offices have yet to provide the HIV education and testing services that are offered in state prisons. This study sought to implement on-site HIV services for probationers and parolees through an intersectoral programme involvin

Limitations of blood supply and walking blood bank implementation in Forward Resuscitative Surgical Detachments during large-scale combat operations: A Monte Carlo simulation model.

BACKGROUND: Hemorrhage remains the leading cause of preventable battlefield mortality. Although early whole blood resuscitation improved outcomes during the Global War on Terror, this relied on permissive operational conditions unlikely to persist in future large-scale combat operations (LSCO). Projected LSCO casualty volumes may rapidly overwhelm current Role 2 Forward Resuscitative Surgical Detachments (FRSDs), and walking blood banks (WBB) may provide only minimal augmentation. This study evaluates blood supply resilience for a Role 2 FRSD across LSCO casualty scenarios and quantifies the i

Delayed discharges in older adults with mental illness and/or dementia in hospital: a scoping review.

BACKGROUND: Delayed discharge among older adults is rising in hospitals across many countries, placing increased strain on healthcare systems and negatively affecting patients' emotional and physical well-being due to prolonged hospital stays and uncertainty. Despite this growing issue, limited research has explored the specific impact of delayed discharge on older adults living with a mental illness and/or dementia. This scoping review aims to examine what is currently known about older adults (aged 65 and over) with a diagnosis of dementia and/or mental illness who experience delayed dischar

Road traffic injuries in Kandy District, Sri Lanka: a prospective analysis of trauma and disability at a tertiary care center.

BACKGROUND: Road traffic injuries (RTIs) are a leading cause of mortality and disability globally, with the greatest burden borne by low- and middle-income countries (LMICs). Sri Lanka continues to experience substantial RTI burden, yet prospective data describing injury characteristics and short-term functional outcomes outside the capital remain limited. We aimed to describe patterns of injury and trauma severity, modes of transport and access to care, and short-term functional outcomes at discharge and during early follow-up among patients presenting with road traffic injuries to a tertiary

Adaptation of the Transition Readiness Assessment Questionnaire for Pediatric-Onset Multiple Sclerosis: Protocol for a User-Centered Design Approach.

BACKGROUND: Pediatric-onset multiple sclerosis (POMS) is a chronic, progressive neurologic condition requiring lifelong management and coordinated transition from pediatric to adult care. Evidence-based guidelines identify transition readiness assessment as a core component of successful transition; however, most POMS clinics do not formally assess readiness, and existing tools do not address POMS-specific challenges, such as fluctuating disability, complex treatment regimens, and cognitive impairment. This gap underscores the need for a transition readiness measure tailored to POMS. OBJECTIV
