Comparative diagnostic assessment by ultrasonography (US) and computed tomography (CT) was performed in each of 17 patients with adrenal disorders. We measured urinary and plasma aldosterone in normal subjects, aged 20 to 59 years, during a period of unrestricted sodium intake and after sodium depletion, using furosemide or a 20 meq sodium diet. The response of urinary diurnal tetrahydroaldosterone (TH-aldo) excretion for fluodrocortisone administration (0.3 mg q.i.d. for 3 days) was studied. In normal subjects (n = 13) and in patients with essential hypertension (n = 8), urinary TH-aldo decreased to 36 per cent (range 19-48) and to 51 per cent (range 33-61) of the control value, respectively. Six children with Bartter's syndrome aged 6 years 4 months to 13 years 11 months were treated with indomethacin (1.7 to 4.3 mg/kg/day) during 7 to 27 months. A catch up growth was first observed, then growth curve was parallel to the normal. A catch up weight was also observed. The osseous maturation was the faster it was more delayed. These changes were observed despite a partial correction of potassium and plasma renine activity.

Multiplane tomography can be employed successfully to image the adrenal glands after the intravenous administration of 6 beta-131I-iodomethyl-19-nor cholest-5(10)-en-3 beta-ol (NP-59). The tomographic display allows separation of the adrenal glands from other organs that often obscure the adrenal image when gamma cameras are used and the depth of the adrenal glands may be determined without obtaining lateral views.

We were able to separate Bartter's syndrome patients into two groups: Group 1 patients are seen at a young age, with hyponatremia and episodes of extracellular volume contraction. Group 2 patients are older and most have tetany; plasma sodium values and renal sodium balances are normal. An efficient protocol for the evaluation of hypertensive cases included renal arteriography, suppression and stimulation of the renin-aldosterone system, and, when indicated, renal venous renin measurements.
