Pulmonary hypoplasia is increasing as a cause of neonatal death. To understand the pathophysiology of pulmonary hypoplasia, the physiology of fetal lung growth must first be understood. Lung fluid production and fetal breathing are primary factors regulating lung growth. Interruption of pulmonary arterial flow also decreases fetal lung growth. To define the relationship of pulmonary arterial flow to other factors known to be important for fetal lung growth, breathing and lung fluid production were measured after postductal main pulmonary artery (MPA) ligation in fetal sheep. Surgical preparati

Background: The global aging population faces an increasing prevalence of type 2 diabetes mellitus (T2DM), often complicated by frailty, cognitive decline, and impaired manual dexterity. These factors make glucose self-monitoring particularly challenging. Minimally invasive glucose monitoring methods, particularly continuous glucose monitoring (CGM) as well as emerging non-invasive glucose monitoring technologies offer potential solutions, but remain insufficiently evaluated in older adults. Objective: To systematically review and synthesize available evidence on the advantages of continuous g

Children in rural communities encounter unique environmental exposures, many of which can result in negative long-term health consequences. Children are particularly at risk from these exposures due to their close interaction with the environment and developing physiology. The authors describe 3 rural environmental hazards: wood stove smoke, well water contaminants, and agricultural pollutants. Contaminants found in these exposures have adverse respiratory, neurodevelopmental, cardiometabolic, and carcinogenic effects, among others. The authors recommend that rural pediatric clinicians screen

A group of Russian specialists dealing with the problems of auditory function in premature babies touches upon important issues of early detection of hearing loss and deafness in this contingent of children born before the date of physiological birth. The purpose of the article was to argue the need for a personalized approach to the diagnosis of auditory function in premature and full-term babies depending on the timing of gestation and their somatic state at the time of birth, as well as the comprehensive rehabilitation of children with hearing loss and deafness. The article describes the ad

Belching is a physiological event that allows venting of swallowed gastric air. Excessive belching is a common presentation to gastroenterology clinics and could be isolated complains or associated with other gastrointestinal problems. PURPOSE OF THIS REVIEW: It is to describe the presentation, diagnosis, and treatment of belching disorders RECENT FINDINGS: These demonstrate that learned abnormal behaviors in response to unpleasant feeling in the abdomen are the driving causes for excessive belching and addressing these behaviors by speech pathology and cognitive behavior therapy considered as

Crisponi syndrome is a rare and severe heritable disorder characterised by muscle contractions, trismus, apnea, feeding troubles, and unexplained high fever spikes with multiple organ failure. Here we report perioperative care for endoscopic gastrostomy of a 17 month-old female child with Crisponi syndrome. Temperature in the surgery room was strictly monitored and maintained at 19 °C. The patient was exposed to both inhaled and intravenous anesthetic agents. Surgical and perioperative periods were uneventful. Episodes of fever in Crisponi syndrome arise from CRLF1 mutation, which differs from

Noninvasive ventilation (NIV) has emerged as a powerful tool for the pediatric emergency management of acute respiratory failure (ARF). This therapy is safe and well tolerated and seems to frequently prevent both the need for invasive mechanical ventilation and the associated risks/complications. Although NIV can be the primary treatment of ARF resulting from multiple respiratory disease states, it must be meticulously monitored and, when unsuccessful, may aid in preoxygenation for prompt endotracheal intubation and invasive mechanical ventilation. The following article reviews the physiologic

Therapeutic Drug Monitoring (TDM) is based on drug-level control in biological matrices and serves as a diagnostic approach for individualization of pharmacotherapy and drug safety. Drug levels of antibiotics are distinctly influenced by comorbidity, physiological changes and various concomitant drugs in patients on intensive care units. Several factors should be taken into account for calculation of relevant pharmacokinetic parameters (elimination half-life, bioavailability, and clearance) to deduce a recommendation for dosage. TDM is a diagnostic standard for the individualization of polypha

Acute respiratory distress syndrome (ARDS) is a common critical pulmonary complication after esophagectomy and other thoracic surgeries (e.g., lung transplantation, pulmonary thromboendarterectomy). Direct pulmonary ischemia-reperfusion injury (PIRI) is known to play the main role in induction of ARDS in these cases. Large animal models are an appropriate choice for ARDS as well as PIRI study because of their physiological and anatomic similarities to the human body. With regard to large animal models, we reviewed different methods of inducing in situ direct PIRI and the commonly applied metho

Metabolic bone disease (MBD) of prematurity remains a significant problem for preterm, chronically ill neonates. The definition and recommendations for screening and treatment of MBD vary in the literature. A recent American Academy of Pediatrics Consensus Statement may help close the gap in institutional variation, but evidence based practice guidelines remain obscure due to lack of normative data and clinical trials for preterm infants. This review highlights mineral homeostasis physiology, current recommendations in screening and monitoring, prevention and treatment strategies, and an added

Failure to thrive is common in infants with hypoplastic left heart syndrome and its variants and those with poor growth may be at risk for worse surgical and neurodevelopmental outcomes. The etiology of growth failure in this population is multifactorial and complex, but may be impacted by nutritional intervention. There are no consensus guidelines outlining best practices for nutritional monitoring and intervention in this group of infants. The Feeding Work Group of the National Pediatric Cardiology Quality Improvement Collaborative performed a literature review and assessment of best nutriti

Recombinant Factor VIIa, a new therapeutic tool to treat severe bleeding caused by inhibitory haemophilia cases, some cases of thrombocytopenia and thrombocytopathy (e.g. severe type III von Willebrand disease) receives growing attention in clinical practice. Exogeneous FVIIa-in a supraphysiological concentration (clearly over 6 U/ml) seems to be able to generate quickly and safely (without thrombotic side effects) thrombin--the final enzyme of clotting--in physiological, or pathological conditions. A concise review about the possible mechanisms of action, indications, monitoring and clinical

A method was developed to isolate and test coproantibodies from mice infected with Trichinella or mice fed cholera toxin. It was rapid as results were obtained within 3 h, and it was applicable to a single faecal pellet. It reduced the number of experimental animals required 7-fold, and reduced variability, since samples were obtained from the same experimental animal over time. As it was noninvasive, it did not alter normal gut physiology, nor the course of the parasitic infection. The immune response was monitored over time in individual mice, and specific sIgA and IgG isotypes were detected

A technique is described which elicits hostility via provocation in a competitive reaction time task incorporating a predetermined failure rate of 50%. When the subject loses he is exposed to a white noise which increases in intensity through the experiment. When he wins he is able to administer one of 8 levels of noise to his opponent. Heart rate and skin conductance level and fluctuations were monitored throughout the experiment. Self-ratings of mood, aggression and anxiety were completed both pre and post task. It was found that the task elicited hostility which could be measured behavioura

Nonobstetric disease requiring surgery may complicate pregnancy and jeopardize maternal and fetal well-being. Surgery may be safely done if the physician is aware of anatomic and physiologic alterations during gestation that necessitate an altered approach to diagnosis and management. Fetal exposure to all diagnostic and therapeutic agents should be minimized, particularly during organogenesis. However, the risk to the fetus of diagnostic irradiation is justifiable when information essential to maternal health is likely to be obtained. Furthermore, the broad range of available antibiotic, anal

While miniaturized wearable ultrasound technologies excel at physiological monitoring, detecting small-molecule biochemicals remains challenging due to their inherent "acoustic invisibility". To address this, we establish a mechano-acoustic transduction mechanism that translates biochemical stimuli into quantifiable acoustic signatures. Central to this approach is a "Core-Shell Cell" (CSC) architecture, comprising a bubble-infused hydrogel core encapsulated by a stimuli-responsive hydrogel shell. Upon exposure to target analytes, the responsive shell swells and exerts compressive stress on the

BACKGROUND: Purulent pericarditis is a rare but severe presentation of acute pericarditis that can rapidly progress to effusive-constrictive pericarditis (ECP), a syndrome characterized by concomitant cardiac tamponade and constrictive pericarditis.
CASE SUMMARY: A 62-year-old woman with diabetes presented with chills, dyspnea, and pleuritic chest pain. Despite initial treatment for idiopathic acute pericarditis, she rapidly developed cardiac tamponade. Emergent pericardiocentesis yielded purulent fluid consistent with Streptococcus pneumoniae pericarditis, and postdrainage echocardiogram dem

Bone turnover markers (BTMs) are biomedical indicators used to assess the bone metabolism processes reflecting the activity of osteoblasts and osteoclasts. During childhood and adolescence, bone metabolism is highly active, leading to distinct levels and trends of BTMs compared with those of adults. BTMs correlate significantly with age, gender and environmental factors, making them valuable for evaluating bone health and developmental trajectories in pediatric populations. Due to the non-invasive characters and dynamic monitoring capabilities, BTMs are increasingly employed in research and cl

It is crucial to monitor hydrogen sulfide (H2S) because H2S plays a vital role in the regulation of many physiology and pathology processes. Many evidences indicate that endogenous H2S is closely associated with many diseases such as inflammation and cancers. Herein, we reported a novel fluorescent probe BTDI to monitor the fluctuation of H2S based on the excited-state intramolecular proton transfer (ESIPT) mechanism both ex vivo and in vivo. The selectivity of BTDI for H2S is significantly higher than that for biothiols and other potential anions. After the probe responded to H2S, the nucleop
