Chronic widespread pain in spondyloarthritis.

The pain associated with spondyloarthritis (SpA) can be intense, persistent and disabling. It frequently has a multifactorial, simultaneously central and peripheral origin, and may be due to currently active inflammation, or joint damage and tissue destruction arising from a previous inflammatory condition. Inflammatory pain symptoms can be reduced by non-steroidal anti-inflammatory drugs, but many patients continue to experience moderate pain due to alterations in the mechanisms that regulate central pain, as in the case of the chronic widespread pain (CWP) that characterises fibromyalgia (FM

Factors affecting risks of congenital malformations. I. Analysis of epidemiologic factors in congenital malformations. Report from the Collaborative Perinatal Project.

An analysis was made of over 30 demographic, biologic and medical variables in relation to risks of congenital malformations which occurred in the Collaborative Perinatal Project. The study population consisted of 46,689 single and multiple live births and fetal deaths about whom complete information was available on all study variables. Included in the analysis were 5 general malformation categories, 27 specific major, and 7 specific minor malformations. Several new associations were detected between epidemiologic factors and risks of specific malformations while other previously reported ass

Utilizing directional preference in the management of cervicogenic headache: a case series.

BACKGROUND/PURPOSE: Headaches are among the most common complaints requiring medical care, and annual expenditures for this condition are estimated to be 14 billion US dollars. The International Headache Society (IHS) describes cervicogenic headache (CGH) as a secondary type of headache emanating from the cervical spine which may be referred to one or more regions of the head and/or face. Mechanical Diagnosis and Therapy (MDT) is an approach shown to be effective in the management of spinal musculoskeletal disorders; however, there is limited evidence as to its efficacy in the management of CG

How muscle stiffness affects human body model behavior.

BACKGROUND: Active human body models (AHBM) consider musculoskeletal movement and joint stiffness via active muscle truss elements in the finite element (FE) codes in dynamic application. In the latest models, such as THUMS™ Version 5, nearly all human muscle groups are modeled in form of one-dimensional truss elements connecting each joint. While a lot of work has been done to improve the active and passive behavior of this 1D muscle system in the past, the volumetric muscle system of THUMS was modeled in a much more simplified way based on Post Mortem Human Subject (PMHS) test data. The stif

Obesity class versus the Edmonton Obesity Staging System for Pediatrics to define health risk in childhood obesity: results from the CANPWR cross-sectional study.

BACKGROUND: Disease severity in paediatric obesity is usually defined using the body-mass index (BMI). Although informative at the population level, its usefulness on an individual level has limitations. The use of a clinical staging system-Edmonton Obesity Staging System for Pediatrics (EOSS-P)-in identifying health risk has been proposed. This study aimed to examine the association between BMI class and EOSS-P stage. METHODS: This cross-sectional study was done in children with obesity aged 5-17 years who enrolled in the Canadian Pediatric Weight Management Registry (CANPWR) between May 31,

Cannabis Use in Orthopaedic Surgery: Effects on Fracture Healing, Opioid Requirements, and Clinical Outcomes.

Cannabis is commonly used by patients presenting for orthopaedic care, including musculoskeletal pain, sleep disturbances, anxiety, or recreational purposes. Because orthopaedic care depends on predictable bone healing, effective perioperative analgesia, and prevention of postoperative complications, the interaction between cannabis exposure and these domains remains uncertain. This narrative review synthesizes mechanistic, preclinical, and clinical evidence regarding the effects of cannabis and cannabinoid exposure on fracture healing, opioid requirements, and broader clinical outcomes in ort

Deleterious NKAP Mutations Are Associated with Musculoskeletal Abnormalities in Hemizygous Males and Skewed X Chromosome Inactivation in Heterozygous Females.

NKAP (NF-kappa-B-activating protein) is a ubiquitously expressed nuclear protein involved in multiple biological processes. Males with missense NKAP mutations have been reported to present with marfanoid features and behavioral and musculoskeletal abnormalities. We have previously reported that a disruptive NKAP mutation resulted in extremely skewed X chromosome inactivation (XCI), leading to phenotypic manifestation of hemophilia A (HA) in a HA carrier. In this study, with the aim of exploring the phenotypic manifestations of deleterious NKAP mutations in males, as well as their involvement i

Physiologically constrained neuromuscular synergy extraction using a deep off-policy dynamic neuro-fuzzy system in wheelchair propulsion.

Manual wheelchair propulsion (MWP) is a repetitive activity that risks upper limb injuries, necessitating analysis of intramuscular coordination for effective intervention. The state-of-the-art synergy extraction methods struggle with MWP's nonlinear, dynamic nature and often overlook biomechanical constraints. This study introduces a deep reinforcement learning-based dynamic neuro-fuzzy (DRL-DNF) system that models complex electromyography (EMG) patterns and refines synergy structures in real time. Data from 24 manual wheelchair users included EMG signals from six muscles, joint kinematics, a
