Comparing interventions for selective mutism: a pilot study.

OBJECTIVE: To examine the outcome within 6 to 8 months of medical and nonmedical intervention for children with severe selective mutism (SM). METHOD: Children with SM (n = 17) and their mothers, seen in a previous study, attended follow-up appointments with a clinician. Obtained by maternal report were: treatment received, current diagnosis (based on semi-structured interview), speech in various environments, and global improvement. An independent clinician also rated global functioning. RESULTS: The diagnosis of SM persisted in 16 children, but significant symptomatic improvement was eviden

Psychosocial consequences of developmental prosopagnosia: a problem of recognition.

OBJECTIVE: To provide the first systematic in-depth description of the consequences of developmental prosopagnosia (DP; 'face blindness') for psychosocial functioning and occupational disability, in order to determine what kind of professional intervention may be needed. METHODS: Semi-structured telephone interviews were carried out with 25 people whose self-reports of face recognition problems were confirmed by impaired scores on the Cambridge Face Recognition Test. Thematic analysis was used to inductively identify and understand common psychosocial consequences of DP. RESULTS: All partici

Social anxiety and peer relations in early adolescence: behavioral and cognitive factors.

This study investigated factors associated with social anxiety during early adolescence using multiple informants, including self and peer perspectives, teacher ratings, and direct observations. Negative social performance expectations, maladaptive coping strategies, and social skill deficits were examined as correlates of social anxiety and mediators linking social anxiety with poor peer relations. Participants were 84 middle school students (47 girls, 37 boys) over-sampled for elevated social anxiety. Analyses revealed correlations linking social anxiety with decreased peer acceptance and in

An experimental study of the role of vulnerability related perceptions in spider fear: comparing an imaginal and in vivo encounter.

The effect of manipulating perceptions of the uncontrollability, unpredictability and dangerousness of a spider was assessed using both an imaginal and in vivo task involving an encounter with a spider. Participants were randomly assigned to one of eight conditions formed by the crossing of factors. Experimental manipulations of uncontrollability, unpredictability and dangerousness all had a significant effect on Task Related Spider Fear in the in vivo exposure task. Results indicated a greater effect on task related fear for in vivo exposure (R(2)=.258) compared to imaginal exposure (R(2)=.05

Perceptions and metaperceptions of same-sex social interactions in college women with disordered eating patterns.

This study compared perceptions and metaperceptions after a social interaction between a woman with disordered eating (subclinical bulimia nervosa) and a female interaction partner. Hypotheses were: women with disordered eating would have more negative metaperceptions of social interactions than women without disordered eating, and women with disordered eating would show a greater discrepancy between metaperceptions and perceptions than women without disordered eating. All participants completed questionnaires about their perceptions and metaperceptions post-interaction. Results showed women w

Time-varying amygdala response to emotional faces in generalized social phobia.

BACKGROUND: Individuals with social phobia (SP) have altered behavioral and neural responses to emotional faces and are hypothesized to have deficits in inhibiting emotion-related amygdala responses. We tested for such amygdala deficits to emotional faces in a sample of individuals with SP. METHOD: We used functional magnetic resonance imaging (fMRI) to examine the neural substrates of emotional face processing in 14 generalized SP (gSP) and 14 healthy comparison (HC) participants. Analyses focused on the temporal dynamics of the amygdala, prefrontal cortex (PFC), and fusiform face area (FFA)

Neural mechanisms of automatic and direct processing of phobogenic stimuli in specific phobia.

BACKGROUND: The study aimed to identify brain activation during direct and automatic processing of phobogenic stimuli in specific phobia. METHODS: Responses to phobia-related and neutral pictures (spiders and mushrooms) were measured by means of event-related functional magnetic resonance imaging during two different tasks. In the identification task, subjects were asked to identify the object (spider or mushroom). In a demanding distraction task, subjects had to match geometric figures displayed in the foreground of the pictures. RESULTS: Phobics showed greater responses to spiders versus m

Neurophysiological correlates of habituation during exposure in spider phobia.

Imaging studies using symptom-provocation paradigms in specific phobia have yielded contradictory results, possibly reflecting a failure to account for habituation processes. Given that a single session of exposure in vivo can result in significant improvement in specific phobia, we used prolonged exposure to phobic stimuli to identify CNS regions showing habituation. Eighteen subjects (12 with spider phobia, 6 healthy controls) underwent H(2)(15)O-positron emission tomography while being continuously presented with pictures of spiders or butterflies. Results showed main effects (spiders>butte

Behavioural inhibition and symptoms of anxiety and depression: is there a specific relationship with social phobia?

OBJECTIVES: The aim of the study was to examine the relationships between tendencies towards different mental health problems assessed via questionnaires (social phobia, agoraphobia, general anxiety/panic, depression), the two latent dimensions of behavioural inhibition (childhood social/school fears, non-social fears/ illness), and sensory-processing sensitivity. DESIGN: A cross-sectional design was employed. METHOD: Volunteer participants (N = 234) from anxiety and depression self-help organizations completed five mailed questionnaires. These were the Social Phobia and Anxiety Inventory, B

School phobia.

Pediatricians and other primary care providers are in an ideal position to prevent, to diagnose and to treat children with school refusal. Detection requires recognition of high risk situations, and delineation of possible reality factors or environmental hazards. The physician will find school refusal associated with perceived or actual physical illness, debility, or vulnerability, family stresses including illnesses and marital problems, over-protective mother-child interaction patterns, and previous difficulties in achieving mother-child independence and separation. Confirming the diagnosis

School phobia and other childhood neuroses: a systematic study of the children and their families.

The authors developed reliable clinical rating scales to compare 35 children with school phobia and their families with a matched sample of children with other neuroses and their families. Twice as many school phobic children as children with other neuroses showed excessive separation anxiety, dependency, and depression. Although a mutually hostile-dependent interaction was found in most of the families of children with school phobia, the development of school phobia appeared to be dependent on defects in character development in the children as well. The authors discuss the etiological signif

Separation-individuation and reconstruction in psychoanalysis.

The development constructs of Mahler and her coworkers are seen as clinically based theory which enrich practice. Freud discovered the importance of the infantile past and psychosexual development in particular, he related these findings to the psychodynamics of neurotic structures, and devised a successful therapeutic model based on his findings. Mahler has uncovered and detailed other aspects of infantile mental life, related genetically not to neurotic byt also to other mental structures, thereby broadening the range of applicability of Freud's therapeutic model. Clinical vignettes are offe

Efficacy of the trial-based thought record, a new cognitive therapy strategy designed to change core beliefs, in social phobia.

WHAT IS KNOWN AND BACKGROUND: Social anxiety disorder (SAD) often follows a chronic course and is associated with substantial impairment in functioning. Although results from clinical trials clearly establish evidence for efficacy of cognitive behavioural therapy in treating this disorder, up to 50% of patients with SAD show little or no improvement. Thus, new approaches that have promised in improving the efficacy of treatment for SAD are needed. One such approach is the trial-based thought record (TBTR), which targets the restructuring of patients' core beliefs. OBJECTIVE: To determine whet

Pharmacological evaluation of the stress-induced social avoidance model of anxiety.

We have shown earlier that mild electric shocks induce a lasting social avoidance in male rats. Here we investigated whether shock-induced social avoidance can be developed into a laboratory model of stress-induced anxiety. The putative new model would assess sub-chronic, stress-induced anxiety (as opposed to tests based on natural fear) in a heterologous context (as opposed to classical fear conditioning). A single exposure to mild electric shocks induced a robust social avoidance that lasted more than 5 days. Low doses of chlordiazepoxide (0.5, 1 mg/kg), diazepam (0.5, 1, 5 mg/kg), buspirone
