Bone cement augmentation of short segment fixation for unstable burst fracture in severe osteoporosis.

OBJECTIVE: The purpose of this study was to determine the efficacy of short segment fixation following postural reduction for the re-expansion and stabilization of unstable burst fractures in patients with osteoporosis. METHODS: Twenty patients underwent short segment fixation following postural reduction using a soft roll at the involved vertebra in cases of severely collapsed vertebrae of more than half their original height. All patients had unstable burst fracture with canal compromise, but their motor power was intact. The surgical procedure included postural reduction for 2 days and bon

Effects of some beta-adrenoceptor antagonists on orthostatic hypotension in repeatedly cold- (SART-) stressed rats.

Rats stressed by specific alternation of rhythm in temperature (SART) show various symptoms of disautonomia, increased pulse rates, continuous hypotension, and severe orthostatic hypotension (OH) when they are subjected to postural change. The OH symptoms are improved by muscarinic M2-receptor blockers. In the present study, effects of beta-adrenoceptor blocking agents on OH in SART-stressed rats were investigated. Anesthetized rats were restrained on a board in the supine position, and direct blood pressure and ECG were measured automatically using Fluclet Jr.2. Postural change was performed

Left inferior parietal representations for skilled hand-object interactions: evidence from stroke and corticobasal degeneration.

Patients with ideomotor apraxia (IM) are frequently more impaired in the production and imitation of object-related (transitive) than non-object-related, symbolic (intransitive) gestures, but reasons for this dissociation, and its anatomical underpinnings, remain unclear. Our theoretical model of praxis (Buxbaum, 2001) postulates that left inferior parietal lobe (IPL) gesture representations store information about postures and movements of the body and hand for skillful manipulation of familiar objects; in contrast, bilateral fronto-parietal dynamic calculations provide constantly-updated inf…

Examination of the flexion relaxation phenomenon in erector spinae muscles during short duration slumped sitting.

OBJECTIVE: The purpose of this study was to examine the myoelectric activity of the erector spinae muscles of the back in order to determine if the flexion relaxation phenomenon occurs in seated forward flexion or slumped postures. BACKGROUND: The flexion relaxation phenomenon during standing forward flexion is well documented. However, flexion relaxation in seated forward flexion has not been studied. It is possible that flexion relaxation could be linked with low back pain that some individuals experience during seated work. METHODS: Twenty-two healthy subjects (11 males, 11 females) parti…

Dopa-responsive Rest Tremor Preceding Tachyphemia in Progressive Supranuclear Palsy.

Progressive supranuclear palsy (PSP) is characterized by progressive postural instability, falls, and supranuclear vertical gaze abnormalities. In this report, we present the case of a 71-year-old woman with dopa-responsive rest tremor followed by tachyphemia and postural instability. She initially presented with dopa-responsive slowness and tremor in the right hand. Two years later, she developed speech difficulties (tachyphemia) and a propensity for falls. Based on the diagnostic criteria for PSP, the patient was diagnosed with probable PSP-RS. The clinical manifestations observed in our pat

A Pilot Study of Idiopathic Toe Walking: Measures and Outcomes.

This pilot study sought to determine the feasibility of quantifying sensory processing, postural control, motor skill development and participation in daily life in children with idiopathic toe walking (ITW) compared to children without ITW. A nonrandomized, prospective case-control design was employed. Twenty participants were recruited. Computerized weight bearing and postural control data was successfully collected. Differences were found in forefoot/rearfoot weight bearing, balance, sensory-motor abilities and parent reported participation challenges. Sensory processing results were inconc

A Single-Level Epidural Blood Patch for Multiple Cerebrospinal Fluid Leaks: How it Works.

Spontaneous intracranial hypotension caused by cerebrospinal fluid (CSF) leak is a rarely encountered cause of persistent postural headaches in the absence of trauma or dural puncture. It presents with postural headache and is characterized by radiological findings of spinal CSF leak. The mainstay of management is an epidural patch with the patient's own blood (epidural blood patch [EBP]) and/or a fibrin glue product. We report here a case of spontaneous intracranial hemorrhage presenting with persistent headaches and bilateral subdural hematomas secondary to CSF leaks at multiple levels along

Physical deformity as sequela of chronic catatonia and response to electroconvulsive therapy: a case report.

Chronic catatonia with posturing can cause joint contractures leading to greater morbidity associated with the physical deformity. We report a case of a young man with chronic catatonic schizophrenia with posturing of bilateral upper limbs in flexion leading to fixed flexion contracture of left metacarpophalangeal joints. Initiation of electroconvulsive therapy along with physical rehabilitation measures helped him regain full range of motion in the right upper limb. The fixed flexion contracture, however, remained resistant to intensive treatment efforts. Early interventions in the form of el

Platypnea-orthodeoxia diagnosed by sitting transesophageal echocardiography.

Platypnea-orthodeoxia is a rare syndrome presenting with dyspnea and cyanosis induced by an upright position and relieved by recumbency. We report a patient with an excessively enlarged aortic root who had critical hypoxia due to a paradoxical shunt through the patent foramen ovale only when seated. The postural changes of the shunt, caused by changes in posture, were dynamically detected by transesophageal Doppler echocardiography. We believe that this syndrome should be considered when patients with aortic root dilatation present with inexplicable hypoxia, and that transesophageal Doppler ec…

Parkinsonism and essential tremor in a family with pseudo-dominant inheritance of PARK2: an FP-CIT SPECT study.

We report a family with 5 affected individuals manifesting either essential tremor (ET), Parkinsonism, or both, consistent with pseudo-dominant inheritance of PARK2. Two homozygotes presented postural and kinetic tremor several years before the onset of Parkinsonism. Postural and kinetic tremor mimicking ET was the only feature in 1 homozygous and 2 heterozygous carriers of the mutation. Striatal dopamine transporter density was reduced in accordance with phenotype and number of mutated alleles. In 3 homozygotes and 1 heterozygote, a 2-year follow-up single photon emission computed tomography

[Neuropeptide factors of pathogenesis of brain-related motor disorders].

Overall 136 patients with cerebral tumors, sequels of craniocerebral injury and brain strokes with motor disorders of varying intensity were examined. Factors of the peptide nature, provoking postural asymmetry of the homolateral hind limb in an experimental animal were detected in the blood and CSF of the overwhelming majority of the patients. Provided the compensatory process runs a favorable course (recovery of motor functions), the neuropeptide factors are inactivated with inactivating substances. The data obtained attest to the advisability of further studies into the activity and nature

Measurement of blood pressure and heart-rate variation while resting supine and standing for the evaluation of autonomic dysfunction.

Blood pressure and R-R interval variation were studied during postural changes using a tilting table. The subjects were 64 normal controls and 52 patients with various disorders. None of the normal controls showed postural falls of more than 15 mm Hg in mean blood pressure. The mean loge coefficient of variation of 100 R-R intervals was significantly reduced in groups with Parkinson's disease, spinocerebellar degeneration, Shy-Drager syndrome and diabetes mellitus, compared with a normal control group. Reduced heart-rate variation was frequently associated with sphincter disturbance and orthos

[Medulloadrenal secretion and peripheral sympathomimetic receptors in familial dysautonomia].

A case of familial dysautonomia in a 30-year old woman with severe postural hypotension is reported. A study of adrenal medulla secretion showed that it was inadapted to the fall in blood pressure, with no increase in dopamine secretion. Insulin-induced hypoglycaemia had no effect on adrenal medulla secretion. Changes in blood pressure and heart rate were studied after administration of alpha- and beta-adrenergic drugs combined with fludrocortisone. The increase observed in blood pressure and heart rate suggests that peripheral catecholamine receptors are present in dysautonomic patients. The

[Dystonia as a complication of gastroesophageal reflux: Sandifer syndrome].

A girl aged 4 years and 5 months was admitted because of vomiting, poor appetite, epigastrial pain, weight loss and an abnormal posturing of the head and neck since the age of 3 1/2 years. This abnormal posturing consisted of maximal lateral flexion of the head and neck to the left. A number of investigations including EEG and CT-Scan of the head, had been performed repeatedly, and despite normal results various neuropsychiatric diagnoses were made. In our hospital massive gastrooesophageal reflux and oesophagitis were demonstrated and the diagnosis of Sandifer's syndrome was established. A fu

Visual influences on balance.

This paper discusses the impact of vision on balance and orientation in patients with vestibular disorders and in anxiety patients with space and motion discomfort (SMD). When the vestibular system is impaired, vision has a greater influence on standing postural control, resulting in greater sway when individuals are presented with erroneous or conflicting visual cues. Studies have shown that individuals with other motion sensitivities, such as motion sickness, also tend to rely on vision for balance and do not disregard erroneous visual cues. Recently, patients with anxiety disorders that inc

Effects of executive function training on balance and auditory-cognitive dual-task performance in adults with and without hearing loss.

BACKGROUND: Multitasking, such as listening while balancing, relies on integrated processing in the sensory, cognitive, and motor systems; systems that often decline with age. Hearing loss is linked to increased risks of both falls and cognitive decline. Improving cognitive processing through executive function (EF) training may support balance, especially in older adults with hearing loss. This randomized controlled study conducted across age groups and hearing abilities, examined the effects of a 12-week EF training program on postural outcomes (center of pressure (COP)) using an auditory-co

Evaluation of lap belt-pelvis load transfer in frontal impact simulations using finite element occupant models.

OBJECTIVE: The goal of this study was to examine the relationship between lap belt tension and force measured at the iliac wing and the effects of model type and torso posture on this relationship. From this analysis, preliminary transfer functions were developed to predict loads applied to the iliac wing as a function of lap belt tension at magnitudes typically measured in sled and vehicle crash tests. METHODS: A DOE study was conducted to provide a robust assessment of the lap belt-pelvis load relationship under various conditions. The GHBMC, THUMS, and THOR FE models were positioned in upr…

Motor Function Characteristics of Adults With Late-Onset Pompe Disease: A Systematic Scoping Review.

BACKGROUND AND OBJECTIVES: Pompe disease is a rare neuromuscular disease caused by a deficiency of the lysosomal enzyme acid α-glucosidase. The late-onset Pompe disease (LOPD) in adults is characterized by weakness of ventilatory, axial, and proximal extremity muscles. These muscle impairments progressively impair various motor functions such as locomotion and postural control. Nearly 87% of adults with LOPD (aLOPD) report walking problems, and more than 80% report instability and falls. Knowledge of these motor functions is now sufficient to provide a clear and comprehensive overview of motor
