Safety and efficacy of intrathecal morphine in early onset scoliosis surgery.

Intrathecal morphine (IM) is a popular adjunct for pain management in spinal deformity surgery for idiopathic scoliosis. It has not been studied in patients with early onset scoliosis (EOS). We retrospectively reviewed EOS patients undergoing growth-friendly surgery who received IM or did not receive IM (non-IM). Data from initial insertion and final fusion procedures were studied. IM was not used for lengthening procedures, short procedures (<3 h), patients with significant underlying respiratory issues, paraplegia, unsuccessful access and anesthesiologist discretion. We assessed pediatric IC

Cauda equina syndrome after unilateral medial branch blocks of the lower lumbar zygapophyseal joints.

Medial branch blockade of the lumbar facet joints is widely performed and generally accepted as a safe intervention. We present a case of neurological damage following a medial branch blockade with local anesthetic and steroid. A patient suffering from chronic low back pain radiating to the buttocks and thighs underwent nine medial branch blockades over a few years. Three months after successful back surgery to remove a herniated L2-3 disk, the pain recurred, and left L3-4 , L4-5, and L5 -S1  medial branch blocks were performed under fluoroscopy. Immediately following the procedure, the patien

Advances in diagnosis and treatment of trigeminal neuralgia.

Various drugs and surgical procedures have been utilized for the treatment of trigeminal neuralgia (TN). Despite numerous available approaches, the results are not completely satisfying. The need for more contemporaneous drugs to control the pain attacks is a common experience. Moreover, a number of patients become drug resistant, needing a surgical procedure to treat the neuralgia. Nonetheless, pain recurrence after one or more surgical operations is also frequently seen. These facts reflect the lack of the precise understanding of the TN pathogenesis. Classically, it has been related to a ne

Retrosternal abscess after trigger point injections in a pregnant woman: a case report.

INTRODUCTION: Although retrosternal abscess is a well known complication of sternotomy and intravenous drug abuse, to date it has not been described as a consequence of trigger point injections. There are reported cases of serious complications as a result of this procedure including epidural abscess, necrotizing fasciitis, osteomyelitis and gas gangrene. CASE PRESENTATION: A 37-year-old African-American woman, who was 20 weeks pregnant, presented to our emergency room with complaints of progressively worsening chest pain and shortness of breath over the course of the last two months. She was

Neuronavigation in the minimally invasive presacral approach for lumbosacral fusion.

INTRODUCTION: Intraoperative 3D navigation (3D NAV) is gaining importance in spinal surgery, especially with the advancement of minimally invasive techniques in this field. We hypothesized that 3D NAV may be of benefit in the recently described minimally invasive presacral approach for L4-S1 fusion (AxiaLIF). METHODS: Five patients with refractory low back pain and L5-S1 degenerative disease underwent 3D NAV-assisted placement of a presacral L5-S1 screw. Additionally, a patient recently underwent a two-level AxiaLIF procedure (L4-S1) which was also performed with 3D NAV. Navigation was perfor

[Radioscapholunate fusion: long-term results].

INTRODUCTION: A review of the literature on long-term results (> or = 10 years) following radiocarpal arthrodesis as recommended by Gordon and King shows a paucity of data. Regarding the suitability of this procedure for treating radiocarpal arthrosis, especially in younger patients, we collected and evaluated long-term results of this surgical procedure. MATERIALS AND METHODS: Five patients (four men, one woman), who were treated between 1978 and 1984 at our institution with a partial radiocarpal arthrodesis as described by Gordon and King were reexamined clinically and radiologically by the

[Lumbar arthrodesis: results after more than 10 years].

Two hundred and seventy four patients had a lumbar spine fusion between 1949/1971. Seventy-five of them were available for review more than 10 years after the procedure. There were twenty spondylolisthesis, forty-eight degenerative discopathies, six cases of Pott's disease and one benign tumour. All the patients had been treated conservatively before the surgical procedure for an average of 7 years and were still complaining of chronic low back pain. All the cases reviewed had fused. The functional results were not related to the surgical technique, which varied during this period: eight poste

Tubeless uniportal VATS in thoracic surgery - Indications, ERAS pathways, and outcomes: A review.

Tubeless uniportal video-assisted thoracoscopic surgery (VATS) is an innovative approach characterized by the use of non-intubated (spontaneous-breathing) anesthesia, the omission of routine postoperative chest drainage, and single-port access. This technique has gained traction in recent years for a variety of thoracic procedures. While practices reported in the literature may differ, this review primarily examines the combined non-intubated and drainless approach. This narrative review provides a comprehensive overview and critical analysis of its current clinical applications, including sym

Post-dural Puncture Headache: Pathophysiology, Risk Stratification, Prevention, and Evidence-Based Management for Practicing Anesthesiologists.

Post-dural puncture headache (PDPH) is a well-recognized secondary headache disorder resulting from persistent cerebrospinal fluid (CSF) leakage following iatrogenic dural disruption during neuraxial procedures, including spinal anesthesia, diagnostic lumbar puncture, dural puncture epidural (DPE), and unintentional dural puncture (UDP). The syndrome reflects a complex interaction between CSF hydrodynamics, craniospinal compliance, vascular compensation, and nociceptive sensitization. Reduction in CSF volume produces intracranial hypotension and compensatory venous dilation in accordance with

Acupotomy for Cervical Spondylosis in Rats: A Visualized Protocol for Model Establishment and Intervention.

Cervical spondylosis (CS) is a chronic degenerative disease. Degenerative changes in the intervertebral discs of the cervical spine, along with subsequent pathological alterations, affect surrounding tissues and structures, leading to clinical manifestations such as neck pain, stiffness, limited mobility, and occasionally numbness and radiating symptoms in the upper limbs. Among these, nerve root compression is the most common. Therefore, this study established a rat model of CS induced by cervical nerve root compression. Acupotomy therapy has shown potential in clinical CS treatment by releas

Anesthesia management in oncology patients undergoing irreversible electroporation outside the operating room: a case series.

BACKGROUND: Irreversible electroporation is a novel, non-thermal tumor ablation technique that induces cell death through electrical disruption of cellular membranes while preserving surrounding connective tissue structures. Despite its clinical advantages, the high-voltage pulses used during the procedure can provoke hemodynamic and electrophysiological instability, requiring specific anesthetic precautions. Data on anesthesia management during irreversible electroporation, particularly in non-operating room anesthesia, remain limited. METHODS: This case series describes 12 oncology patients
