Clinical Characteristics and Fracture Patterns Among Postmenopausal Women with Isolated Osteoporosis at the Forearm.

Patients found to have isolated osteoporosis at the 1/3 radius (1/3RO) represent a therapeutic dilemma. It is unknown whether 1/3RO is associated with an increased risk of fragility fractures, and is therefore unclear whether these patients should be treated similarly to those with osteoporosis at central sites. This retrospective study investigated the clinical significance of 1/3RO by comparing medical history, fracture prevalence, areal BMD, and Trabecular Bone Score in postmenopausal women with 1/3RO (n = 107) to age-matched women with osteoporosis at the hip and/or spine (PMO, n = 214), a

Impaired healing of fragility fractures in type 2 diabetes: clinical and radiographic assessments and serum cytokine levels.

BACKGROUND: Diabetes induces bone alterations accompanied by altered cytokine expression patterns. These alterations lead to modified fracture healing, contributing to musculoskeletal fragility in the elderly. AIMS: We evaluated the inflammatory immune response in diabetic patients during fracture healing relative to clinical and radiographic assessments. METHODS: Fifty patients of both sexes with fragility fractures were studied: 30 diabetics (group A, mean age 73.4 ± 11.2 years) and 20 normoglycemic controls (group B, mean age 75.1 ± 16.9 years). Two subgroups comprised those with hip or w

Indications for operative treatment of fractures in childhood.

Fractures in children heal more quickly than in adults. Stiffness of joints hardly ever occurs in children, even after prolonged immobilization. Pseudoarthrosis and Sudeck's atrophy are extremely rare. Axial, lateral and longitudinal malpositioning can be largely compensated by increased growth. Only rotation malpositions remain permanent in children. By and large, the treatment of fractures in childhood is therefore still conservative. Operative treatment is recommended for some types of fractures near joints, for fractures of the neck of the femur, and for some types of the fractures around

External validation of a novel HR-pQCT based fracture risk assessment tool (μFRAC) in a male cohort: the osteoporotic fractures in men (MrOS) study.

Fracture risk estimates can be used clinically to inform treatment decision-making in osteoporosis. Current fracture risk assessment tools have a low sensitivity in predicting fractures in males. This study aims to evaluate and validate the performance of a new fracture prediction tool-the Microarchitecture Fracture Risk Assessment Calculator ($\mu $FRAC)-in a multicentre cohort (MrOS) of older community-dwelling men. The performance of $\mu $FRAC was assessed in a population of 1586 men aged $\geq 77$ years in the United States. All participants underwent HR-pQCT scanning (61 $\mu $m) of the

Better oral hygiene is associated with a reduced risk of osteoporotic fracture: a nationwide cohort study.

BACKGROUND: The aim of this study was to examine the longitudinal association between oral health parameters and osteoporotic fracture. METHODS: The study included participants who received oral health screening by dentists from the National Health Screening cohort database of Korea between 2003 and 2006. The primary outcome was osteoporotic fracture occurrence, which was defined using specific international classification of diseases-10 codes; vertebral fracture (S22.0, S22.1, S32.0, S32.7, T08, M48.4, M48.5, and M49.5), hip fracture (S72.0 and S72.1), distal radius fracture (S52.5 and S52.6

The anterior limited approach of the elbow for the treatment of capitellum and trochlea fractures: Surgical technique and clinical experience in eight cases.

When a coronal fracture affects the capitellum and the trochlea, the Kocher lateral approach may be inadequate for the correct visualisation, reduction and fixation of the fracture. In such cases an associated medial elbow approach may be required, or a posterior transolecranon approach may be preferred. The anterior limited approach to the elbow (ALAE) could be a valid option when treating these types of fracture, as it does not involve the detachment of any muscle group or ligament, thereby facilitating the recovery process. We can also treat associated injuries such as fractures of the radi

Utility of the Three-Point Index in the determination of reduction loss during the conservative treatment of pediatric forearm mid-third fractures.

BACKGROUND: Forearm fractures constitute approximately 40% of all pediatric fractures. Generally, in conservative treatment,a plaster cast is applied with the elbow in 90° flexion. Success of the treatment depends on theprevention of the reduction in the correct position and suitable duration of the plaster cast. Failure, or the risk of angulation within the cast, is associated with movement within the cast. The aim of this study was to evaluate the applicability of the Cast Index (CI) and Three-Point Index (TPI) measurements, which indicate the loss of reduction, in pediatric mid-third forear

Thrombocytopenia-Absent Radius (TAR): Case report of dental implant and surgical treatment.

Thrombocytopenia-absent radius (TAR) syndrome is a congenital malformation in which affected individuals present reductions in the number of platelets, hypoplasia, or absence of radial bone unilaterally or bilaterally. Hematologic, skeletal, cardiac (particularly tetralogy of Fallot and septal-atrial defects), and gastrointestinal anomalies are most commonly associated with TAR syndrome. Skeletal changes result in a higher risk of dental and craniofacial trauma in patients with the syndrome. Thus, it is important for the dentist to be aware of the characteristics of TAR syndrome and its clinic

Clinical utility of denosumab for treatment of bone loss in men and women.

While most older patients with osteoporosis are treated with antiresorptive bisphosphonates such as alendronate, risedronate, ibandronate, and zoledronic acid, such drugs have side effects, remain in bone for extended periods, and lead to poor adherence to chronic treatment. Denosumab is a humanized monoclonal antibody and antiresorptive agent that works by decreasing the activity of the receptor activator of nuclear factor kappa B ligand. In major trials in postmenopausal women, denosumab increased bone mineral density by dual energy x-ray absorptiometry in the spine, hip, and distal third of

Chapter 3: Impact of primary hyperparathyroidism.

At present, primary hyperparathyroidism is most often discovered in an asymptomatic patient, but can sometimes be revealed by a renal or bone complications. In all cases, a full work-up is recommended, with assessment of renal function (glomerular filtration rate), 24-hour calciuria, screening for risk factors for lithiasis, and renal and urinary tract imaging (ultrasound or CT scan) to look for stones or nephrocalcinosis. Bone densitometry, with measurements of the spine, femur and radius, is the recommended reference test for demineralization. Standard X-rays of the spine or other imaging te

Late results in fractures of the radial head treated by excision.

Late results of radial head excision were studied in 60 patients who had isolated fractures of the radial head. Symptomatic elbows were found in 43% of the patients, osteoarthritis in 52%, and valgus deformity in 30%. Elbow motion was restricted in 63%, and new bone formation at the surgical site was noted in 57% of the patients. Forearm rotation was limited in 58%. A proximal radioulnar synostosis was found in 7%, and proximal migration of the radius was present in 47%. A definite and symptomatic subluxation of the distal radioulnar joint was found in 25%. Overall results were unsatisfactory

[Transposition of the extensor indicis tendon in reconstruction of thumb extension after rupture of the extensor pollicis longus tendon].

During a period of 13 years we performed 56 extensor indicis proprius (EIP) transpositions for reconstruction of the ruptured or severed extensor pollicis longus tendon. The open injuries (n = 9) involved failed primary repair or untreated tendon injuries. The subcutaneous ruptures occurred after distal radius fractures (n = 17) or other closed injuries of the wrist (n = 10), without trauma in 14 patients and in 6 patients by rheumatoid synovialitis or collagenosis. 35 patients returned for follow-up examination 8 months to 10.5 years after operation. According to the evaluation scheme suggest…

The role of arthroscopy in the treatment of wrist injuries in the athlete.

As a minimally invasive procedure, wrist arthroscopy provides the advantages of accurate, direct visual inspection of intra-articular structures for diagnostic purposes and dramatically reduced morbidity for surgical treatment of certain disorders. For an athlete who has a limited window of opportunity to compete, arthroscopy can offer an earlier, more accurate, more complete diagnosis than other imaging techniques for soft-tissue disorders of the wrist. Arthroscopic surgery can frequently offer an abbreviated recuperation period and earlier return to competition. Arthroscopy offers advantageo

Radial neck fracture in children: anatomic and functional results of Metaizeau technique.

Fractures of the radial neck accounts for 1% of all childhood fractures and 5% to 10% of childhood traumatic lesions involving the elbow. Intramedullary percutaneous nail reduction (Metaizeau technique) is considered the most effective surgical technique. The purpose of this study was to identify the main clinical features of radial neck fracture in children and to evaluate the anatomical and functional results of the Metaizeau technique. In this retrospective study, we evaluated 22 patients under the age of 16 who were treated for radial neck fracture at the orthopedic and trauma surgery depa
