OBJECT: One of the feared consequences of craniovertebral junction diseases is apnea. Although several cases of patients with central apnea have been described, obstructive sleep apnea has been identified as the most frequent manifestation of sleep respiratory disorder. Neuronal involvement may be responsible for both central and obstructive apneas. The objective of this work was to study the effect of posterior fossa decompressive surgery on respiratory parameters during sleep in patients with craniovertebral junction malformations and breathing-related sleep disorders.

It is likely sleep apnea is highly prevalent in patients with congestive heart failure (CHF). However, as awareness is low and consensus guidelines do not exist, sleep apnea is not routinely screened for or diagnosed in CHF practice. Untreated sleep apnea may promote fatigue as well as left ventricular dysfunction, disease progression, and increased mortality. Available screening tools lack sensitivity or specificity, and there are insufficient numbers of sleep laboratories to accommodate the potentially large number of patient referrals with CHF for definitive diagnosis. In CHF patients with

Obstructive and central sleep apnea are common in heart failure, and may participate in its progression by exposing the heart to intermittent hypoxia, increased preload and afterload, sympathetic activation, and vascular endothelial dysfunction. Treatment of sleep apnea in patients with heart failure may reverse these detrimental effects, in addition to alleviating symptoms of sleep apnea. In patients with heart failure and obstructive sleep apnea, short-term randomized trials have demonstrated that continuous positive airway pressure (CPAP) improves cardiac function, and lowers sympathetic ac

There are different treatment options for obstructive sleep apnea, which are selectively used based on severity of sleep apnea, physical structure of the upper airway, and other medical aspects including co-morbidities. Weight loss as well as avoidance of alcohol and other CNS depressants is generally recommended. Positional training, oral appliances and surgery of the upper airways are used in selected cases. CPAP is the most effective method for treating obstructive sleep apnea irrespective of disease severity. In patients with central sleep apnea or Cheyne-Stokes respiration (CSA/CSR) diagn

STATEMENT OF PROBLEM: The efficiency of an appliance for treatment of sleep apnea shows inordinate interindividual difference. The mechanism of its therapeutic effects remains unresolved.
PURPOSE: This study examined the effect of the device on sleep apnea, and masticatory and tongue muscles.
MATERIAL AND METHODS: Fifteen patients with sleep apnea syndrome were evaluated polysomnographically, with and without the appliance. Electromyograms (EMG) of genioglossal, masseter, and lateral pterygoid muscles were recorded and EMG amplitudes measured before, during, and after the apneas. Apneas were

PURPOSE: The characteristics of central sleep apnea (CSA) in real-world sleep referral populations remain poorly understood. We used the electronic health record of a large health system to understand the prevalence and risk factors for CSA among patients referred for sleep evaluation.
METHODS: We reviewed the records of 29,803 patients who underwent diagnostic polysomnography at six sleep laboratories over 14 years. A validated algorithm was used to extract polysomnographic results. CSA was defined as having an apnea hypopnea index (AHI) ≥ 5, a central apnea index ≥ 5, and a central apnea in

This cross-sectional study aimed to investigate whether body fat distribution, physical activity levels and dietary intakes are associated with insomnia and/or obstructive sleep apnea among overweight middle-aged men. Participants were 211 Finnish men aged 30-65 years. Among the 163 overweight or obese participants, 40 had insomnia only, 23 had obstructive sleep apnea only, 24 had comorbid insomnia and obstructive sleep apnea and 76 were without sleep disorder. The remaining 48 participants had normal weight without sleep disorder. Fat mass, levels of physical activity and diet were assessed b

BACKGROUND: Sleep apnea is a common disorder characterized by recurrent episodes of upper airway obstruction or impaired respiratory drive, leading to disrupted sleep and significant cardiopulmonary consequences. While anatomical and neuromuscular factors are well-established contributors to obstructive sleep apnea (OSA), and central sleep apnea (CSA) is often linked to cardiac or neurological conditions, the impact of endocrine disorders, particularly hypothyroidism, is frequently overlooked.
CLINICAL PRESENTATION: We describe a case of severe sleep apnea that exhibited persistent sleep apne

By means of polygraphic sleep recording, the sleep apnea profile with respect to the number and duration of inactive, obstructive and mixed apneic episodes as well as periodic breathing has been investigated in infants born preterm at 40, 52 and 64 weeks conceptional age and compared to that of term infants. At 40 weeks preterm infants showed significantly more apnea and periodic breathing compared to term infants. The difference was essentially due to obstructive and mixed apnea in non-REM sleep. There was a sharp decrease in all apneic variables--inactive, obstructive and mixed apnea as well

Obesity is a primary risk factor for the development of obstructive sleep apnea in humans, but the impact of obesity on central sleep apnea is less clear. Given the comorbidities associated with obesity in humans, we developed techniques for long-term recording of diaphragmatic EMG activity and polysomnography in obese mice to assess breathing patterns during sleep and to determine the effect of obesity on apnea generation. We hypothesized that genetically obese ob/ob mice would exhibit less variability in breathing across the 24-h circadian cycle, be more prone to central apneas, and be more

OBJECTIVES: Recent retrospective studies found sleep disorders, including obstructive sleep apnea and its symptoms to occur more often in patients following aneurysmal subarachnoid hemorrhage, but studies investigating the incidence of subarachnoid hemorrhage in patients with diagnosed obstructive sleep apnea [OSA] compared to other sleep disorders are missing.
METHODS: To test our hypothesis that aneurysmal subarachnoid hemorrhage occurs more often in patients with OSA compared to other sleep disorders, we analyzed clinical data of 5514 patients with OSA, 4150 with other sleep disorders, and

BACKGROUND: Heart failure is a highly prevalent disorder that continues to be associated with repeated hospitalizations, high morbidity, and high mortality. Sleep-related breathing disorders with repetitive episodes of asphyxia may adversely affect heart function. The main aims of this study were to determine the prevalence, consequences, and differences in various sleep-related breathing disorders in ambulatory male patients with stable heart failure.
METHODS AND RESULTS: This article reports the results of a prospective study of 81 of 92 eligible patients with heart failure and a left ventr

Sleep apnea has been recorded in many infants, but little data exist concerning the amount and range of apnea in infants. We studied 49 infants referred to the sleep disorders unit. Single polysomnographic studies were performed on each infant. We examined the amount of apnea, presence and amount of upper airway obstruction and the sleeping pattern in each infant. Central apnea was common to all infants and varied in amount. Upper airway obstruction, recorded as mixed apnea, was found in 36 infants. Twenty of these infants had only occasional mixed apnea ( < 2 apneas/hour), whereas 16 infants

BACKGROUND: Frailty is associated with an increased susceptibility to functional decline, impairment, hospitalization, and mortality among the older adults. However, the potential reversibility of frailty lies in identifying modifiable factors that could prevent, mitigate, or interrupt its progression. While there is a suggestion that sleep disorders may increase the risk of frailty and impairment, the risk stratification of this relationship remains inconclusive.
OBJECTIVE: Stratify the risk of frailty and impairment and investigate potential connections with sleep quality, excessive daytime

STUDY OBJECTIVES: Obstructive sleep apnea can induce hypertension. Apneas in REM may be particularly problematic: they are independently associated with hypertension. We examined the role of sleep stage and awakening on acute cardiovascular responses to apnea. In addition, we measured cardiovascular and sympathetic changes induced by chronic sleep apnea in REM sleep.
METHODS: We used rats with tracheal balloons and electroencephalogram and electromyogram electrodes to induce obstructive apnea during wakefulness and sleep. We measured the electrocardiogram and arterial pressure by telemetry an

OBJECTIVE: Sleep deprivation is a risk factor for sudden infant death syndrome (SIDS). Recent changes in normal life routines were more common among SIDS victims, compared with control infants. Sleep deprivation can result from handling conditions or from sleep fragmentation attributable to respiratory or digestive conditions, fever, or airway obstructions during sleep. Compared with matched control infants, future SIDS victims exhibited fewer complete arousals by the end of the night, when most SIDS cases occur. Arousal from sleep could be an important defense against potentially dangerous si

Obstructive apnea during sleep elevates the set point for efferent sympathetic outflow during wakefulness. Such resetting is attributed to hypoxia-induced upregulation of peripheral chemoreceptor and brain stem sympathetic function. Whether recurrent arousal from sleep also influences daytime muscle sympathetic nerve activity is unknown. We therefore tested, in a cohort of 48 primarily nonsleepy, middle-aged, male (30) and female (18) volunteers (age: 59±1 years, mean±SE), the hypothesis that the frequency of arousals from sleep (arousal index) would relate to daytime muscle sympathetic burst
