Nightmares and sleep disturbance are fundamental concerns for victims of trauma. This study examined the efficacy of a manualized cognitive-behavioral treatment (CBT) for chronic nightmares in trauma-exposed individuals via a randomized clinical trial. Participants were randomly assigned to a treatment group or wait-list control group, with 27 participants completing the treatment. At the 6-month follow-up assessment, 84% of treated participants reported an absence of nightmares in the previous week. Significant decreases were also reported in symptoms of depression and posttraumatic stress, f

What factors distinguish war-exposed veterans who experience posttraumatic stress symptomatology (PTSS) from those who do not? This study used structural equation modeling procedures to examine the complex interplay among predeployment, war-zone, and postdeployment factors as they relate to PTSS in a sample of Gulf War I veterans. A primary goal was to determine to what extent previously documented associations among Vietnam veterans would replicate in this more contemporary veteran cohort. Results supported a multivariate etiological perspective on PTSS, with war-zone factors accounting for t

While many studies have demonstrated relationships between trauma and dissociation, relatively little is known about other factors that may increase children's risk for developing dissociative symptoms. Drawing on betrayal trauma theory and Discrete Behavioral States frameworks, the current study examined the contributions of maternal factors (including mothers' dissociation, betrayal trauma experiences, and inconsistent parenting) to children's dissociation. Seventy-two mother-child dyads completed self-report questionnaires. Maternal dissociation was found to relate positively to maternal be

The National Vietnam Veterans Readjustment Study (NVVRS) researchers reported that 30.9% of all men who served in that conflict developed posttraumatic stress disorder (PTSD) even though only about 15% had been assigned to combat units. Scholars, mainly historians, have questioned the accuracy of the PTSD prevalence rate. The purpose of this article is to evaluate the merits of several hypotheses adduced to explain the high apparent PTSD prevalence in the NVVRS. Empirical and conceptual analysis suggests that malingering is unlikely to account for many cases. Also, deployment to Vietnam in the

The lifetime prevalence of posttraumatic stress disorder (PTSD) is about 8%-14% in the general population, and trauma victimization (51%-98%) and PTSD (up to 42%) are even more prevalent among persons treated within public-sector mental health clinics. Despite this, individuals with PTSD and severe mental illness (SMI) who are treated within the public sector tend to receive inadequate mental health services. In addition, treatments for PTSD for this population remain undeveloped, with virtually no available empirical treatment outcome data to guide clinicians. We propose a model for a compreh

For patients, being diagnosed with cancer means being confronted with a life-threatening illness and can thus be considered a traumatic experience. This situation is compounded by the fact that having cancer is usually not an isolated event but is generally a long-term stressor. The aim of the present study was a) the assessment of psychotraumatic and comorbid distress in tumor patients both in the acute stage (T1) and over time (T2), b) the identification of possible protective factors, and c) the identification of risk factors for the development of a posttraumatic stress reaction (PTSR) fol

The objective of this study is to look for a relationship between childhood trauma and hostility as an adult. Toward this end, 294 recently abstinent cocaine or opiate dependent patients were asked to complete two questionnaires: the Childhood Trauma Questionnaire (CTQ) and the Hostility and Direction of Hostility Questionnaire (HDHQ). It was found that there were significant correlations between the HDHQ total hostility score and scores on the CTQ for childhood emotional abuse, physical abuse, sexual abuse, emotional neglect and physical neglect. Thus, these results suggest the possibility th

Although previous research has implicated a history of childhood trauma in the development of dissociative tendencies, insufficient cognizance (in this context) has been taken of the distinction between pathological and nonpathological dissociation. In this study, the relationship between childhood trauma and both pathological and nonpathological dissociation was investigated in a sample of 100 Australian adults. Pathological dissociation was positively predicted by dimensions of childhood trauma, but no such relationship was found for nonpathological dissociation (psychological absorption). T

The final traumatic event recalled by Anna O. during her treatment with Josef Breuer was a terrifying hallucination she once had of a black snake attacking her ailing father. This event has been variously interpreted as indicating an underlying psychodynamic conflict, as a temporal lobe seizure, and as an hypnotic confabulation. We argue, however, that the hallucination--during which Anna O.'s arm was reportedly "asleep" due to nerve blockage--was probably a sleep paralysis nightmare. Sleep paralysis nightmares continue to be overlooked or misdiagnosed in clinical practice, and, in recent year

In this paper I describe a special form of ego-state therapy that integrates writing letters to one's self as an internal therapeutic communication among various ego states previously not fully aware of each other or engaged in destructive power struggles. Expressing one's thoughts, feelings, and urges in writing transforms primary process activities of the self into secondary processes that are more mature and adaptive to healthy functioning in day-to-day living. This method also reinforces the idea of patient empowerment in the form of self-healing and utilizing internal resources in the pro

In 1979 a program called Operation Outreach began to provide comprehensive and integrated psychosocial services to Vietnam veterans. The 136 vet centers, housed in storefronts and other nongovernment buildings, are staffed by Vietnam veterans and others knowledgeable about the horrors of that war; many of them are not mental health professionals. To date the centers have treated approximately 100,000 veterans out of the several hundred thousand who may be experiencing the effects of posttraumatic stress disorders. The author briefly discusses the history of the centers and describes the theore

To determine whether posttraumatic reactions occurred in injured individuals not confronted with litigation or compensation (as reckoned in the U.S.), a parallel study was undertaken in a social-democratic country (Austria) between ski- and work-injured groups. Although each group had a significantly different way of regarding the injury, there appeared to be a comparable incidence of posttraumatic psychiatric reaction in each. The absence of compensation (in contrast to disability maintenance) did not inhibit the appearance of the posttraumatic reaction. A discussion of the effects of compens

Cortisol or its medicinal form hydrocortisone (HCT) in accessible biofluids is widely used as a biomarker of stress and stress-related disorders, including Cushing syndrome, chronic fatigue syndrome, post-traumatic stress disorders, and fibromyalgia. Here, we report a simple nanomaterial-free electrochemical sensing strategy for selective cortisol detection based on a molecularly imprinted polymer (MIP) formed directly on indium tin oxide (ITO). The resulting sensor exhibited a wide linear dynamic range (100 pg/mL to 160 ng/mL) in artificial sweat, covering clinically relevant cortisol concent

Acute stress disorder (ASD) and posttraumatic stress disorder (PTSD) may occur after traumatic event and also cause significant life time impairment. P300 event-related potential (ERP) is a potential biological marker for PTSD and can reflect cognitive impairment in information processing and attention. Despite the usefulness of ERP, there are few attempts to reveal relationships between ASD and P300. In the present study, we aimed to determine if the P300 of the patients who were the victims of sexual abuse reflected the quantitative trait of ASD or if P300 is applicable as a state marker for

Building upon prior research documenting differential effects of psychological maltreatment, physical, and sexual abuse on youth mental health outcomes (Spinazzola et al., 2014), the present study sought to clarify the relative predictive contributions of type of maltreatment compared to salient exposure characteristics. The sample included 5058 clinic-referred youth from the Core Dataset (CDS) of the National Child Traumatic Stress Network (NCTSN) with lifetime histories of exposure to one or more of three specific types of maltreatment: psychological maltreatment (PM), physical abuse (PA), a

Episodic memory impairments beyond the traumatic event might be a characteristic hallmark of post-traumatic stress disorder (PTSD). Although several studies support such a claim, empirical findings are inconsistent. Therefore, we performed a random-effects meta-analysis including data from a total of 47 studies and 3,062 subjects. As main finding, we can show that PTSD patients show episodic memory deficits compared to all controls. This effect was significantly stronger for PTSD vs. non-traumatized healthy controls as compared to PTSD vs. traumatized controls without PTSD. Finally, episodic m

It is known from clinical studies that some patients attempt to cope with the symptoms of post-traumatic stress disorder (PTSD) by using recreational drugs. This review presents a case report of a 19-year-old male patient with a spectrum of severe PTSD symptoms, such as intense flashbacks, panic attacks, and self-mutilation, who discovered that some of his major symptoms were dramatically reduced by smoking cannabis resin. The major part of this review is concerned with the clinical and preclinical neurobiological evidence in order to offer a potential explanation of these effects on symptom r

Background: Forced displacement is globally increasing, with children and adolescents being particularly affected. Exposure to armed conflict, the flight itself and challenges of settling in new environments put them at risk of experiencing potentially traumatic events and developing mental health problems, such as post-traumatic stress disorder (PTSD) and depression. Still, limited information is available regarding the traumatic experiences, psychological stress and treatment goals of treatment-seeking refugee children and adolescents. Methods: This cross-sectional study evaluated data from r

The purpose of this study was to clarify the mental health needs of Iraqi immigrants who arrived in the United States in the 1990s after the Persian Gulf War. The records of 375 clients were examined at a clinic that serves Arab Americans. More posttraumatic stress disorder and health problems were found in Iraqi refugees than in other clients. Results suggest the need for further research on immigrants with traumatic histories to facilitate effective treatments.

BACKGROUND: In France, approximately 381,000 minors receive Child Welfare Services (Aide Sociale à l'Enfance, ASE) measures, a population highly exposed to early adversities. Despite the expected high prevalence of trauma-related disorders, screening for post-traumatic stress disorder (PTSD) remains insufficient.
OBJECTIVE: To assess the prevalence of PTSD and complex trauma indicators through systematic analysis of educational case files of adolescents in child protection services. To assess the training needs of child protection professionals regarding psychotrauma.
METHOD: Retrospective a
