Anti-inflammatory 25(OH)D3, a natural steroid hormone, may complement all-trans retinoic acid therapy for differentiation syndrome in acute promyelocytic leukemia.

Differentiation syndrome (DS) is a serious complication with an unclear pathogenesis that arises following all-trans retinoic acid (ATRA) or arsenic trioxide induction therapy in acute promyelocytic leukemia (APL). DS symptoms include pyrexia, respiratory compromise, increased body mass, fluid accumulation, pulmonary infiltrates, hypotension, tachycardia, edema, and sepsis. It can also affect the kidneys and the central nervous system. The standard treatment to counteract DS involves the temporary cessation of ATRA or arsenic trioxide treatment and the administration of high-dose steroids to m

A review of the risk and precipitating factors for spontaneous coronary artery dissection.

INTRODUCTION: Spontaneous coronary artery dissection (SCAD) accounts for 1%-4% of cases of acute coronary syndrome (ACS). SCAD is caused by separation occurring within or between any of the three tunics of the coronary artery wall. This leads to intramural hematoma and/or formation of false lumen in the artery, which leads to ischemic changes or infarction of the myocardium. The incidence of SCAD is higher in women than in men, with a ratio of approximately 9:1. It is estimated that SCAD is responsible for 35% of ACS cases in women under the age of 60. The high frequency is particularly observ

COVID-19 mortality and its predictors in the elderly: A systematic review.

Источник пометил публикацию как отозванную

BACKGROUND AND AIMS: Older people have higher rates of comorbidities and may experience more severe inflammatory responses; therefore, are at higher risk of death. Herein, we aimed to systematically review the mortality in coronavirus disease 2019 (COVID-19) patients and its predictors in this age group. METHODS: We searched PubMed, Web of Science, and Science Direct using relevant keywords. Retrieved records underwent a two-step screening process consisting of title/abstract and full-text screenings to identify the eligible studies. RESULTS: Summarizing findings of 35 studies demonstrated t

[Pathophysiological mechanisms of major mental disorders related to cardiovascular disease].

Although the relationship of mental health with cardiovascular dysfunction is not a recent finding, scientific data has appeared approximately at the middle of the last century. Firstly, depression was studied as a risk factor for premature death in cases of cardiovascular disease (CVD). Much later, the mechanism of psychosis and schizophrenia in the development of CVD were studied, as it was observed that most premature deaths in schizophrenia were related to cardiovascular disease. This interaction is supported both by epidemiological data and by the associated mechanisms. Inflammation, oxid

Thyroid dysfunction may be associated with poor outcomes in patients with COVID-19.

BACKGROUND: Coronavirus disease (COVID-19) has resulted in considerable morbidity and mortality worldwide. Thyroid hormones play a key role in modulating metabolism and the immune system. However, the prevalence of thyroid dysfunction (TD) and its association with the prognosis of COVID-19 have not yet been elucidated. In this study, we seek to address this gap and understand the link between TD and COVID-19. METHODS: Herein, we enrolled patients who were hospitalized with COVID-19 and had normal or abnormal thyroid function test results at the West Court of Union Hospital in Wuhan, China, be

Clinical and Molecular Phenotypes of Low-Penetrance Variants of NLRP3: Diagnostic and Therapeutic Challenges.

OBJECTIVE: Cryopyrin-associated periodic syndromes (CAPS) result from gain-of-function mutations in the NLRP3 gene, which causes excessive release of interleukin-1β (IL-1β) and systemic inflammation. While pathogenetic NLRP3 variant phenotypes are well-characterized, low-penetrance NLRP3 variants represent a significant clinical challenge. The aims of this study were to determine the clinical phenotype, the in vitro biologic phenotype, and the effect of anti-IL-1 treatment in patients with low-penetrance NLRP3 variants. METHODS: A multicenter study of consecutive symptomatic patients with low

Plasmatic isoforms of cytokeratin 18 and RAGE after severe trauma: a longitudinal cohort study.

BACKGROUND: Life-threatening traumatic injuries lead to a complex inflammation-driven pathophysiology. Receptor of advanced glycation end product (RAGE) is a multiligand receptor of several endogenous alarmins, while cytokeratin 18 is a structural component of the filament of epithelial cells. Both proteins can be frequently found in plasma of patients with different diseases, whereby they have distinct underlying mechanism of formation. In this prospective observational study, we wanted to shed light on the kinetic of plasmatic RAGE and cytokeratin 18 isoforms after severe trauma, thereby als

Current advances in restoring intestinal barrier homeostasis by natural medicines.

BACKGROUND: The intestinal mucosa serves as a critical interface separating the internal environment of the human body from the external milieu, with its barrier function determining the appropriate entry of luminal contents. Dysfunction of this barrier is closely associated with the development of intestinal disorders, such as inflammatory bowel disease and irritable bowel syndrome, as well as a spectrum of extraintestinal diseases. Both Eastern and Western traditional medicine regard the intestine as central to overall health, and since most natural medicines are administered orally, they ar

Pharmacological mechanisms of probenecid for SARS-CoV-2 and RSV co-infection: findings of system pharmacology, molecular docking, molecular dynamics simulation, and structure-activity relationship.

BACKGROUND: The clinical consequences of the co-infection with novel severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and respiratory syncytial virus (RSV) are not optimistic. Nevertheless, there is currently no approved therapeutic regimen specifically targeting SARS-CoV-2/RSV co-infection, with existing monotherapies showing limited efficacy. According to recent studies, probenecid has both anti-SARS-CoV-2 and anti-RSV effects. Therefore, as one probable molecular candidate for the co-infection with SARS-CoV-2 and RSV, probenecid was researched in this exploration. METHODS: Usin

The detection of endocarditis, post implantation grafts, arteritis and other related disorders by 18F-FDG PET/CT.

INTRODUCTION: Positron emission tomography with computed tomography (18F-FDG PET/CT) is a nuclear medicine diagnostic method which, unlike other technological modalities that asses anatomical features, detects increased glucose metabolism inside the cells, thus is very helpful in diagnosing cardiovascular infection and inflammation and also in therapy planning. AIM: Aim of this study was to assess the significance of 18F-FDG PET/CT in detection of an active disease in patients with infection and inflammation of cardiovascular system. MATERIAL AND METHODS: In this cohort retrospective study 7

Drug administration in chronic liver disease.

Cirrhosis encompasses a range of pathophysiological changes that may alter drug disposition. Drugs that are dependent primarily on the liver for their systemic clearance are more likely to be subject to reduced elimination and subsequent accumulation. Drug accumulation may lead to excessive plasma drug concentrations and adverse effects, if the adverse effects of the drug are concentration-dependent. The effects of hepatic insufficiency on the pharmacokinetics of drugs are not consistent or predictable. Furthermore, the influence of hepatic disease on the disposition of various drugs can vary,

Muscle wasting in patients with end-stage renal disease or early-stage lung cancer: common mechanisms at work.

BACKGROUND: Loss of muscle mass worsens many diseases such as cancer and renal failure, contributes to the frailty syndrome, and is associated with an increased risk of death. Studies conducted on animal models have revealed the preponderant role of muscle proteolysis and in particular the activation of the ubiquitin proteasome system (UPS). Studies conducted in humans remain scarce, especially within renal deficiency. Whether a shared atrophying programme exists independently of the nature of the disease remains to be established. The aim of this work was to identify common modifications at t

Complement activation in acute coronary syndromes.

The complement system is part of the host defence response. However, considerable evidence suggests that complement plays an important role in the pathophysiology of ischemic heart disease. The aim of this study was to evaluate complement activation in patients with all forms of acute coronary syndromes (ACS) and to examine the relationship between the degree of complement activation and myocardial injury. The study population included 152 subjects (26 females): 82 with ACS (35 acute myocardial infarction (AMI), 22 non-Q wave MI (NQMI), 25 unstable angina (UAP)) (Group A), 35 stable angina (SA

LXR-β regulates microglial efferocytosis and neuroinflammation in CPSP via STAT6 activation.

BACKGROUND: Central post-stroke pain (CPSP) is a chronic neuropathic pain syndrome that develops following cerebrovascular injury and currently lacks effective treatment options. Previous research from our group has found a significant number of apoptotic cells in the thalamus of CPSP rats, and in the nervous system, the failure to promptly clear apoptotic cell debris can activate microglia, triggering a persistent neuroinflammatory response that contributes to the onset and progression of CPSP. Microglia clear apoptotic cells in the central nervous system through efferocytosis, a process that

Comorbidities and Symptomatology of SARS-CoV-2 (Severe Acute Respiratory Syndrome Coronavirus 2)-Related Myocarditis and SARS-CoV-2 Vaccine-Related Myocarditis: A Review.

Myocarditis is an inflammatory disease of the heart muscle, with manifestations that include myocardial infarction, arrhythmia, and even sudden death. The primary etiology of myocarditis is a viral infection, with studies demonstrating that infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) can lead to myocarditis. This enzyme is involved in many body tissues, including the gastrointestinal system and the cardiac system. This enzyme is responsible for converting angiotensin I to angiotensin II in the renin-angiotensin system of our body. This review aims to characteriz

Anti-CD20 Therapies in Pediatric Acquired Demyelinating Syndromes: Evidence Across MS, AQP4-IgG-Positive NMOSD and MOGAD.

Pediatric-acquired demyelinating syndromes, including multiple sclerosis (MS), aquaporin-4-IgG-positive neuromyelitis optica spectrum disorder (AQP4+NMOSD), and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) constitute distinct entities arising during critical periods of neurodevelopment. Early and effective treatment is therefore crucial to prevent long-term disability. Converging evidence supports a central role of B-cells in central nervous system autoimmunity, extending beyond antibody production to include antigen presentation, cytokine production, and T-cell modu
