Since the mid 1980s, sonography has been used to evaluate the rotator cuff. It is inexpensive, well tolerated, rapidly performed, and accurate. With the recent improvements in imaging equipment and refinements in scanning technique, an increasing number of radiologists are performing shoulder sonography. This article emphasizes the pertinent musculoskeletal anatomy, sonographic technique, normal sonographic anatomy, pathophysiology of rotator cuff and biceps tendon disease, sonographic findings of rotator cuff tears, and biceps tendon pathology. The accuracy of ultrasound in diagnosing rotator

This article describes a modified endoscopic technique of posterior cruciate ligament reconstruction using bone-patellar tendon-bone or calcaneus tendon allografts. Three portals were used: a parapatellar anteromedial portal, a lateral anterolateral portal, and a proximal posteromedial portal. The tibial tunnel was made through the anterolateral tibial cortex 2 cm lateral to the tibial tuberosity to the posterior flat spot of the tibia 1 cm below the articular margin and just lateral to the midline. By this method, a less acute angle at the turning point of the tibial tunnel and a straighter a

Spiral and oblique metacarpal shaft fractures frequently develop shortening through the fracture site. The acceptable amount of fracture shortening has not been well established. The goal of this study was to elucidate the acceptable limits of metacarpal shaft fracture shortening in a cadaver model by assessing the magnitude of the metacarpophalangeal (MCP) joint extensor lag produced. Nine fresh-frozen cadaver hands were used to create a metacarpal shaft fracture model in the second and fifth metacarpal bones. Sequential shortening up to 10 mm in 2-mm increments was performed. The results rev

A technique of delayed extensor tendon reconstruction and centralization using a palmaris longus tendon graft is presented. The tendon graft is woven through the base of the proximal phalanx and up and over the reconstructed extensor tendon to simulate the natural sagittal bands. As described, the technique allows gliding of the extensor tendon while maintaining its vitally important central location.

In conclusion, the sports medicine specialist is able to use a spectrum of diagnostic, surgical, and rehabilitation techniques to identify etiologic factors and to choose optimal treatment regimens for patients with Achilles tendinitis or traumatic rupture. Correction of pathomechanic factors, anatomic restoration, and aggressive postoperative rehabilitation allows an early return to sport without significant loss of strength or mobility.

To date, the lesions of the popliteal system, which are responsible for the so-called postero-lateral knee instability, are still an imperfectly known chapter of knee conditions. We retrospectively analyzed 215 consecutive MR examinations performed May-August 1991 to evaluate MR capabilities in correctly depicting both the normal anatomy and the pathologic conditions of the popliteal system. Using our standard protocols for knee study and two different magnets (0.064 and 0.5 T), we identified 23 lesions of the popliteus and of its meniscal and peritoneal insertions, and 17 lesions involving al

Twenty-seven shoulders (in 26 patients) that had surgical repair of a rupture of the long head of the biceps tendon between 1962 and 1981 were evaluated at three to 22 years after injury (mean, 13 years). Thirty similar patients had been treated nonsurgically and were evaluated two to 15 years (mean, 4.6 years) after tendon rupture. Biomechanical testing was performed on ten patients in the surgical group and 13 in the nonsurgical group. Residual arm pain was infrequent in both surgically and nonsurgically treated groups. Residual subjective weakness at the elbow was reported in four of the su

A brief historical, anatomic, and functional review of the finger extensor system is complemented by more recent refinements relative to possible surgical restoration of tendon and retinacular disruption at the three articular levels.

Three patients were treated for snapping scapholunate subluxation in four wrists, with disabling pain in three wrists. Conventional roentgenographic techniques were not helpful in diagnosing any of these wrist problems. No complete description of the precise abnormality encountered was found in the surgical or roentgenographic literature. Three of the involved wrists have had surgical correction with good early results.

Two different or general types of arthroplasty for treatment of the saddle-joint-arthrosis are usual: particular bone-resection (base of first metacarpal or part of the trapezium) or complete resection of the trapezium, both combined with an interposition. The last one is essentially used in carpometacarpal arthroplasty. Two main groups are known: tendon-interposition and silastic prostheses. Most authors describe the dislocation of the first metacarpal, followed by pain and instability, in 20% of all cases. Neglecting of the ligament conditions seems to be the mainfault. In nearly all cases t

1. Trauma to soft tissues is associated with tissue rupture and formation of a haematoma. 2. Such lesions cannot be expected to resolve without complications in the absence of appropriate treatment. 3. Myositis ossificans should be anticipated in lesions of muscles adjacent to their attachments. 4. Chronic tendonitis and enthesopathies not associated with inflammatory connective tissue disease are primarily degenerative in nature and are probably the result of comparative ischaemia. Mesenchymoid transformation is a feature of both.

Tourniquet paralysis, arterial spasm, aneurysm formation, and the post-ischemic hand syndrome are all complications of penumatic tourniquet use in surgery of the extremities. This is a case report of another complication of tourniquet use, rupture of the long tendon of the biceps brachii muscle. Some underlying pathologic or degenerative condition usually exists before rupture can occur. Both rheumatoid arthritis and local corticosteroid injections into the long tendon of the biceps probably accounted for weakening the tensile strength of the tendon and subsequent rupture in the case described

The anatomy of the sulcus of the extensor carpi radialis tendon was analyzed in order to establish the importance of anatomic variation of the bony bed for the development of habitual dislocation of the tendon. Length, width and slope of the lateral walls were measured. Additional morphologic description permitted to distinguish 4 different types of sulcus. The results mean that the anatomy of the sulcus has only an indirect connection with dislocation, via laxity of the retinacula.
