The longitudinal course of PTSD among disaster workers deployed to the World Trade Center following the attacks of September 11th.

This study examined the long-term mental health outcomes of 2,960 nonrescue disaster workers deployed to the World Trade Center site in New York City following the September 11, 2001 (9/11) terrorist attacks. Semistructured interviews and standardized self-report measures were used to assess the prevalence of posttraumatic stress disorder (PTSD) and other psychopathology 4 and 6 years after the attacks. Clinician-measured rates of PTSD and partial PTSD 4-years posttrauma were 8.4% and 8.9%, respectively, in a subsample of 727 individuals. Rates decreased to 5.8% and 7.7% for full and partial P

Today's Role of External Fixation in Unstable and Complex Pelvic Fractures.

INTRODUCTION: The treatment of pelvic fractures has undergone a change over the past few years. As there seems to be a trend away from external towards internal fixation, the goal of this study was to investigate whether the use of an external fixator is still a standard procedure for the initial as well as - if necessary - for the definitive treatment of complex and unstable pelvic injuries. METHODS: During a period of five years at the Level I Trauma Center, an external fixator was applied in 28 (11%) out of 236 pelvic ring fracture cases. The common indications were open fractures, complex

Bayesian hierarchical spatial modeling of substance abuse patterns following a mass trauma: the role of time and place.

To illustrate how spatial modeling methods may provide insight about the relation between proximity to mass trauma and substance use, we examined the role of proximity to a terrorist event in determining risk of substance use related diagnoses. Previous analyses that have assessed changes in substance use following mass traumas such as terrorist attacks have produced conflicting results. We used Bayesian hierarchical modeling methods to assess whether distance from the World Trade Center (WTC) site in the aftermath of the September 11, 2001 terrorist attacks was associated with risk of substan

Open-book pelvic fractures with perineal open wounds: a significant morbid combination.

Open-book pelvic fractures (OBPF) with concomitant intra-abdominal injuries carry a high morbidity and mortality; the significance of associated perineal open wound (OBPF-POW) has not been defined. We hypothesize that the presence of perineal open wounds increases morbidity, mortality, and concomitant use of hospital resources. Patients diagnosed with OBPF over a 5-year period at a Level I trauma center were identified by trauma registry review, and were retrospectively reviewed under an Institutional Review Board-approved protocol. Patients with OBPF without a perineal open wound were compare

Ipsilateral talar and calcaneal fractures: a retrospective review of complications and sequelae.

BACKGROUND: Ipsilateral talar and calcaneal fractures represent a rare combination injury that has only recently been reported in the literature with small case series. OBJECTIVE: To identify the commonly observed fracture patterns, complications, and outcomes of a consecutive series of patients with ipsilateral talar and calcaneal fractures. METHODS: Forty-five cases of ipsilateral talus and calcanal fractures were identified from an orthopaedic trauma registry at a University-based, level I trauma center for retrospective review. MAIN OUTCOME MEASUREMENTS: Post-operative complications, th

The role of lung function in brain tissue oxygenation following traumatic brain injury.

OBJECTIVE: Previous studies have demonstrated that periods of low brain tissue oxygen tension (PbtO2) are associated with poor outcome after head trauma but have primarily focused on cerebral and hemodynamic factors as causes of low PbtO2. The purpose of this study was to investigate the influence of lung function on PbtO2 with an oxygen challenge (increase in fraction of inspired oxygen [FiO2] concentration to 1.0). METHODS: This prospective observational cohort study was performed in the neurointensive care unit of the Level 1 trauma center at San Francisco General Hospital. Thirty-seven pa

The morbidity of penetrating colon injury.

UNLABELLED: The purpose of this study is to define the current morbidity and mortality associated with penetrating colon injury and to determine the impact of management strategy on outcome. METHODS: A retrospective review was performed of all penetrating colon injuries managed at a level I trauma center (1990-2000), n=186. Stepwise logistic regression was used to determine the independent predictors for colostomy and morbidity following colon injury. RESULTS: Fifty-three percent of the patients were managed with primary repair or anastomosis while 47% received a colostomy. Independent predi

Major vascular injuries in the pediatric population.

Ischemia requiring revascularization is rare in the pediatric population. We have evaluated the immediate and late outcomes for children undergoing peripheral bypass for ischemia secondary to trauma to both upper and lower extremities. Children requiring vascular repair with saphenous vein bypass at a pediatric hospital and trauma center over a 5-year time period were reviewed. Nineteen patients sustained major vascular injuries requiring bypass, with a mean age at time of injury of 13.9 years (5-18). Nine of the 19 patients were available for late follow-up. The mechanism of injury was evenly

Ipsilateral fractures of the talus and calcaneus.

Nine ipsilateral fractures of the talus and calcaneus were treated at Tampa General Hospital between 1991 and 1994 and entered into the trauma registry of this level 1 trauma center. During this same period, a total of 78 talar fractures and 334 calcaneal fractures were entered into the registry. The patients who sustained this rare combined injury were studied retrospectively to characterize the fractures that occurred, examine the treatments instituted, and determine outcomes. Four patients had severe intra-articular damage of the subtalar joint surfaces and underwent either primary or delay

[Injury pattern after fall from great height. An analysis of 101 cases].

There are few references discussing the typical injury patterns of adults after a fall from a height. We present the pattern of injury in 101 adult patients who were treated between 1987 and 1990 at our trauma center after falling from an average height of 7.2 m. In 62 patients the fall was accidental; 39 patients tried to commit suicide by jumping from a height. The most common injuries were fractures of the thoracic and lumbar spine (83.0%) with a preference for the thoracolumbar junction. Fracture of the lower limbs occurred in 45%. The most frequent injuries were fractures of the os calcis

[Ulnar luxation of the extensor pollicis longus. Anatomic and clinical study].

The authors report five cases of ulnar dislocation of the Extensor pollicis longus caused by rupture of the hood at the metacarpophalangeal of joint the thumb. This rarely described lesion is well known in long fingers but seems to be less common in the thumb and no previous series can be found in the literature. This dislocation gives a deficit on active extension of the metacarpophalangeal joint of the thumb. During trauma of MP, the expansion of abductor pollicis brevis cans be damaged which provokes ulnar dislocation of EPL. An anatomical study of this region is reported by the authors. 10

Splenic conservation after multiple trauma in adults.

The traditional management of splenic injuries is undergoing serious challenge. During the four year period from 1980 to 1983, 60 of 230 patients with injured spleens operated upon at the Maryland Institute for Emergency Medical Services' Shock Trauma Center have had splenic salvage. Motor vehicle accidents accounted for 57 of the 60 injuries (95 per cent). Fifty-eight patients (97 per cent) had major associated extra-abdominal injuries (average 1.9 injuries per patient) requiring additional operative procedures. Concurrent intra-abdominal injury was present in 37 patients (62 per cent). The m

Predicting Hospital Length of Stay in Orthopedic Trauma Patients Using Fracture-Specific Machine Learning Models: A Multicenter Retrospective Prediction-Modeling Study.

BACKGROUND AND AIMS: Hospital length of stay (LOS) is a key indicator for resource allocation, bed management, and discharge planning in orthopedic trauma care. Given the substantial heterogeneity of LOS across fracture types and the limitations of conventional approaches, this study aimed to develop and validate fracture-specific machine learning models for predicting short vs. long hospital stay. METHODS: In this multicenter retrospective prediction-modeling study, routinely collected hospital information system data from three trauma centers were analyzed. From 326,323 diagnosis-level reco…

Let the resident try: Evaluation of central venous catheter placement in patients with hypotensive trauma using trauma video review.

BACKGROUND: Intraosseous access has the greatest initial success rate in patients with hypotensive trauma and is significantly faster than central venous catheter access. However, central venous catheters play a critical role in the resuscitation of patients with hypotensive trauma. We sought to identify factors associated with successful placement of central venous catheter and hypothesized that attending surgeons would be more successful than trainees in the placement of central venous catheters. METHODS: We analyzed data abstracted from audiovisual recordings from a multicenter study of in
