Dynamics of anal HPV infection after 9vHPV vaccination in men who have sex with men (MSM) with HIV: A prospective, controlled cohort study.

Men who have sex with men (MSM) with HIV are disproportionately affected by persistent human papillomavirus (HPV) infections and related anogenital cancers. No controlled trial has evaluated the efficacy of the nonavalent HPV vaccine (9vHPV) in this population. This prospective, controlled cohort study offered eligible MSM with HIV a 3-dose 9vHPV series and followed both the vaccinated group (vaccinees) and the unvaccinated group (controls). Anal specimens were collected for HPV genotyping, cytology, and microbiota profiling. The primary outcomes were the clearance and incidence of 9vHPV-cover

A Novel Classification System of Renal Hilar Tumors for Surgical Guidance: Technique, Outcome, and Safety.

BACKGROUND: This study was designed to develop an innovative classification and guidance system for renal hilar tumors and to assess the safety and effectiveness of robot-assisted partial nephrectomy (RAPN) for managing such tumors. METHODS: A total of 179 patients undergoing RAPN for renal hilar tumors were retrospectively reviewed. A novel classification system with surgical techniques was introduced and the perioperative features, tumor characteristics, and the efficacy and safety of RAPN were compared within subgroups. RESULTS: We classified the tumors according to our novel system as fo

Diagnostic and prognostic impact of new pathophysiology-based categorization of type 1 and type 2 myocardial infarction: data from the French RICO survey.

BACKGROUND: A new classification of type 1 and 2 myocardial infarction (MI) derived from the fourth universal definition of MI (UDMI) has been recently proposed, based on pathophysiology of coronary artery disease (CAD). We assessed the impact of this new MI categorization on epidemiology and outcomes, considering type 1 MI (T1MI) and type 2 MI (T2MI), with and without CAD. METHODS: Retrospective study including all consecutive patients hospitalized for an acute MI in a multicenter database (RICO). MI was defined according to current UDMI. Rates and outcomes of T1MI and T2MI were addressed ac

Stress resilience and subsequent risk of type 2 diabetes in 1.5 million young men.

AIMS/HYPOTHESIS: Psychosocial stress in adulthood is associated with a higher risk of type 2 diabetes, possibly mediated by behavioural and physiological factors. However, it is unknown whether low stress resilience earlier in life is related to subsequent development of type 2 diabetes. We examined whether low stress resilience in late adolescence is associated with an increased risk of type 2 diabetes in adulthood. METHODS: We conducted a national cohort study of all 1,534,425 military conscripts in Sweden during 1969-1997 (97-98% of all 18-year-old men nationwide each year) without prior d

A Comprehensive Review of the Literature Supporting Recommendations From the Canadian Diabetes Association for the Use of a Plant-Based Diet for Management of Type 2 Diabetes.

Type 2 diabetes mellitus is considered one of the fastest growing diseases in Canada, representing a serious public health concern. Thus, clinicians have begun targeting modifiable risk factors to manage type 2 diabetes, including dietary patterns such as a plant-based diets (PBDs). The Canadian Diabetes Association has included PBDs among the recommended dietary patterns to be used in medical nutrition therapy for persons with type 2 diabetes. To support knowledge translation, this review summarizes the current literature relating to PBDs and the prevalence of type 2 diabetes, its clinical ap

Zabramski classification in predicting the occurrence of symptomatic intracerebral hemorrhage in sporadic cerebral cavernous malformations.

OBJECTIVE: The authors aimed to investigate the evolutionary characteristics of the Zabramski classification of cerebral cavernous malformations (CCMs) and the value of the Zabramski classification in predicting clinical outcome in patients with sporadic CCM. METHODS: This retrospective study consecutively included cases of sporadic CCM that had been untreated from January 2001 through December 2021. Baseline and follow-up patient information was recorded. The evolution of the Zabramski classification of a sporadic CCM was defined as the initial lesion type changing into another type for the

Rethinking Disability Assessment in Italy: Innovations and Challenges in the Medico-Legal Perspective of Type 2 Diabetes Mellitus.

The recent reform of the Italian civil disability assessment system has introduced a new integrated evaluation framework grounded in the International Classification of Functioning, Disability and Health (ICF), the International Classification of Diseases (ICD), and the WHO Disability Assessment Schedule (WHODAS 2.0). This person-centered, context-sensitive model aims to transcend the limitations of a purely clinical evaluation. However, adopting new instruments and criteria presents significant challenges, particularly in medico-legal settings. This paper, through a narrative analysis of the

Role of the Genetic Variants of Fetuin-A and Adiponectin in Type 2 Diabetes Mellitus: A Narrative Review.

Type 2 diabetes mellitus (T2DM) is a global health concern driven by insulin resistance and beta-cell dysfunction. Understanding the genetic basis of T2DM is crucial for effective interventions. This narrative review explores the involvement of the genetic variants of fetuin-A and adiponectin, adipokines crucial for metabolic regulation, in T2DM risk. Fetuin-A, primarily produced by the liver, and adiponectin, secreted by the adipose tissue, play pivotal roles in the metabolism of glucose and the sensitivity of insulin. This review explores the genetic variants of fetuin-A and adiponectin in T

Light-chain amyloidosis with dysphagia as the main symptom: a case report.

BACKGROUND: Immunoglobulin light-chain amyloidosis is a relatively rare condition with a worldwide incidence of 5.1-12.8 cases per million person-years (Baker, 2022). It is characterized by a clonal population of immunoglobulin-secreting cells that produce a monoclonal light chain of κ or λ type as either an intact molecule or a fragment. CASE PRESENTATION: A 69-year-old East Asian (Chinese) male patient who presented with progressive dysphagia visited multiple hospitals repeatedly for more than 2 years and was finally diagnosed with immunoglobulin light-chain amyloidosis. CONCLUSIONS: Otola

Pheochromocytoma/Paraganglioma Syndrome Type 1 Presenting with Atypical Symptoms and a Novel Pathogenic Variant in the SDHD Gene: A Case Report.

This case report presents a 10-year-old patient diagnosed with pheochromocytoma/paraganglioma syndrome type 1 (PPGL1), underlined by a novel heterozygous pathogenic variant (c.154_161del, p.ser52Profster14) in the SDHD gene. Initially, the patient manifested symptoms unusual for pheochromocytoma, including polyuria and polydipsia; however, further diagnostic investigations revealed a pheochromocytoma (PCC) tumor in the adrenal gland. Subsequently, whole exome sequencing (WES) test identified a pathogenic frameshift variant in the SDHD gene, strongly suggestive of PPGL1. This study highlights t

Multidisciplinary Care of a Large Brain Metastasis in a Patient with Hormone-Receptor-Positive Breast Cancer with Ataxia-Telangiectasia Mutation.

Poly (adenosine diphosphate-ribose) polymerase inhibitors (PARP)i are emerging as standard oncology treatments in various tumor types. The indications will expand as PARPi are being investigated in various breast cancer subtypes. Currently, except for BRCA1/2 mutation carriers with human epidermal growth factor receptor 2 (HER2)-negative breast cancer, there is inadequate identification of predictive biomarkers of response. We present a 57-year-old woman with metastatic breast cancer, hormone-receptor-positive, HER2 negative with a germline ataxia-telangiectasia mutation with a large brain met

K-Rod: An Innovative Method of Personalized Rib Splinting for Expeditious Management of Flail Chest in Acute Care Settings.

Flail chest is a severe type of multiple rib fracture that can cause ventilation problems and respiratory complications. Historically, flail chest has been mainly managed through pain control and ventilatory support as needed. Operative fixation has recently become popular for the condition, and some studies have revealed its potentially positive effects on the outcomes of patients with flail chest. However, for those for whom surgery is unsuitable, few treatment options, other than simply providing analgesia, are available. Herein, we introduce our innovative method of applying personalized r

Radiotherapy of lymphomas.

Radiotherapy for Hodgkin lymphomas has evolved a lot over time, but still plays an important role, almost always in addition to chemotherapy, for the management of the early stages. The major objective is to preserve the quality of life of patients who will be cured from this disease in the vast majority of cases. Also, the personalization of the indications for the purpose of de-escalating toxicity is very refined and is essentially based on the pre- and pertherapeutic assessment by FDG-PET. The indications for radiotherapy are more limited for non-Hodgkin lymphomas, but the same principles a

Protocol for Reducing COVID-19 Transmission Risk in EEG Research.

Electroencephalogram (EEG) recordings provide a valuable, noninvasive methofd for measuring human brain activity. This protocol modifies our general protocol for EEG recording (Farrens et al., 2019) for use during the COVID-19 pandemic. It was created with the help of numerous experts, and it specifies a clear set of steps for interacting with research participants, using personal protective equipment (PPE), and disinfecting equipment, all with the goal of reducing the COVID-19 risks for both laboratory personnel and participants. It focuses on the use of EEG in relatively simple research stud

Diabetic calcaneal osteomyelitis.

Diabetic foot infection (DFI), a multi-facetted disease requiring a multidisciplinary approach for successful treatment, mostly affects the forefoot. Calcaneal osteomyelitis (CO) is an uncommon presentation of DFI with a somewhat different epidemiology, clinical features, and approach to management. These patients, compared to those with non-calcaneal DFI, more often require special surgical techniques and off-loading approaches. In this narrative review targeted to non-surgical clinicians, we explore how CO differs from other types of DFI affecting other anatomical locations. Based on our rev

Borderline personality disorder and bipolar disorder: what is the difference and why does it matter?

Borderline personality disorder (BPD) and bipolar disorder (types I and II) are frequently confused because of their symptomatic overlap. Although affective instability is a prominent feature of each, the pattern is entirely different. BPD is characterized by transient mood shifts that occur in response to interpersonal stressors, whereas bipolar disorder is associated with sustained mood changes. These disorders can be further distinguished by comparing their phenomenology, etiology, family history, biological studies, outcome, and response to medication. Their distinction is of great clinica

A brief review of recent advances in stem cell biology.

Stem cells have the remarkable potential to develop into many different cell types, essentially without limit to replenish other cells as long as the person or animal is still alive, offering immense hope of curing Alzheimer's disease, repairing damaged spinal cords, treating kidney, liver and lung diseases and making damaged hearts whole. Until recently, scientists primarily worked with two kinds of stem cells from animals and humans: embryonic stem cells and non-embryonic "somatic" or "adult" stem cells. Recent breakthrough make it possible to convert or "reprogram" specialized adult cells t

Trauma and dissociation: implications for borderline personality disorder.

Psychological trauma can have devastating consequences on emotion regulatory capacities and lead to dissociative processes that provide subjective detachment from overwhelming emotional experience during and in the aftermath of trauma. Dissociation is a complex phenomenon that comprises a host of symptoms and factors, including depersonalization, derealization, time distortion, dissociative flashbacks, and alterations in the perception of the self. Dissociation occurs in up to two thirds of patients with borderline personality disorder (BPD). The neurobiology of traumatic dissociation has demo

Doubt in the psychoanalysis of a paedophile.

In this single case study, I will address the question of how incestuous sexual abuse causes doubt of three distinct types: (1) honest doubt that is essential to the pursuit of knowledge; (2) inherent doubt that can result from a trauma that leaves the victim's mind fragmented and confused; and (3) the sadistic imposition of doubt intended to deceive, disorient and attack unbearable knowledge in the victim or another person familiar with the abuse (in this case, the analyst and the victim's later victims). During the course of my psychoanalysis of a paedophile, it became clear that this third
