Do excoriations in psoriasis delay or prevent the effect of topical treatment?

Topical treatment of a number of out-patients with psoriasis vulgaris resulted in reduced induration and diminished thickness of the layer of scales but complete involution was not obtained. A sequential photographic study of the macroscopic appearance of the lesions over a 3-month period of treatment revealed that artificially produced excoriations and a horny layer without scales were prominent features. The view is put forward that the cutaneous reaction to the injury probably elicited by the excoriations, together with an unsatisfactory treatment, may give rise to tissue changes which coun

Discoid lupus erythematosus. Is systemic treatment necessary?

An evaluation of fluocinolone acetonide applied topically in the treatment of discoid lupus erythematosus is presented. Bilateral comparison of 0.025% fluocinolone acetonide cream to the cream base alone demonstrated the effectiveness of this program. In 59 patients an attempt was made to control this disease with topical fluocinolone acetonide as their only treatment. Forty three patients could be controlled in this fashion. Eleven patients responded partially, but for optimal response required a systemic supplement. Five patients did not show any improvement following topical fluocinolone ac

Vaginal fungi are associated with treatment-induced shifts in the vaginal microbiota and with a distinct genital immune profile.

Vaginal colonization by fungi may elicit genital inflammation and enhance the risk of adverse reproductive health outcomes, such as HIV acquisition. Cross-sectional studies have linked fungi with an absence of bacterial vaginosis (BV), but it is unclear whether shifts in vaginal bacteria alter the abundance of vaginal fungi. Vaginal swabs collected following topical metronidazole treatment for BV during the phase 2b, placebo-controlled trial of LACTIN-V, a Lactobacillus crispatus-based live biotherapeutic, were assayed with semi-quantitative PCR for the relative quantitation of fungi and key b

Focal Epithelial Hyperplasia in a Somali Child: A Case Report and Review of Management with Topical Imiquimod.

BACKGROUND: Focal epithelial hyperplasia (FEH), also known as Heck's disease, is a rare, benign, contagious condition of the oral mucosa associated primarily with human papillomavirus (HPV) types 13 and 32. It predominantly affects children and adolescents and is often underreported in many regions. CASE PRESENTATION: We report the case of an 8-year-old Somali girl who presented with a 3.5-year history of asymptomatic, progressively increasing oral mucosal lesions. Clinical examination revealed multiple soft, flat-topped, verrucous papules involving the lower labial, buccal, lingual mucosa, a

Successful Treatment of Proliferative Verrucous Leukoplakia With Topical Imiquimod.

Proliferative verrucous leukoplakia (PVL) is a rare and clinically aggressive form of oral leukoplakia with a high rate of malignant transformation and recurrence. Given its aggressive clinical behavior, surgical excision has traditionally been the primary treatment modality. The effectiveness of topical chemotherapeutics in treating potentially malignant oral lesions has recently become a topic of considerable debate. Herein, we report the treatment outcome of a middle-aged female patient who declined surgical intervention and was instead managed solely with topical imiquimod, a Toll-like rec

Modernizing the Regulatory Framework for Dietary Supplements: A Position Paper From the American College of Physicians.

In the United States, dietary supplements are regulated as a subcategory of food and therefore are not required to undergo review and approval by the U.S. Food and Drug Administration (FDA). This inadequate regulatory framework has allowed adulterated and mislabeled products to repeatedly enter the market and poses a health threat to the public. In this position paper, the American College of Physicians offers policy recommendations to bolster the FDA's pre- and postmarket regulatory authority for dietary supplements, standardize dietary supplement terminology and data sharing, and implement c

Factor XI inhibitors versus direct oral anticoagulants for the prevention of thromboembolic events and safety in patients with atrial fibrillation: A systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: While DOACs - Direct Oral Anticoagulants are the current standard for stroke prevention, nearly 30-50% of eligible patients remain untreated or undertreated due to perceived or actual bleeding risk. Therefore, evaluating agents that potentially uncouple antithrombotic efficacy from hemostasis is a clinical priority. Factor XI/XIa (FXI/XIa) inhibitors are proposed to reduce bleeding while maintaining antithrombotic efficacy. OBJECTIVES: We aimed to compare the efficacy and safety of FXI/XIa inhibitors versus DOACs in patients with AF. METHODS: We conducted a systematic review and

Licuri Oil (Syagrus coronata) as a Natural Oily Core for Cationic Polymeric Nanocapsules for Topical Formulation: Development, Characterization, and Incorporation into Hydrogels.

Solar ultraviolet (UV) radiation is a major contributor to skin damage, making the regular use of broad-spectrum sunscreens essential for effective photoprotection. However, the long-term efficacy of sunscreen formulations is limited by the photoinstability of some organic UV filters, particularly avobenzone. Licuri oil (Syagrus coronata) is a naturally derived Brazilian palm oil that represents a promising alternative to medium-chain triglycerides (MCTs) as an oily core for polymeric nanocapsules. Therefore, this study aimed to develop Eudragit® RS 100-based cationic nanocapsules using licuri

Induction treatment of lupus nephritis: to pulse or not to pulse? Whether 'tis nobler to universally or selectively prescribe.

OBJECTIVE: Although treatment guidelines recommend pulse steroids, induction treatment of lupus nephritis (LN) varies significantly among providers. This paper aims to explore evidence that intravenous pulse provides pharmacological benefits along with improved clinical efficacy without greater toxicity compared with high-dose oral glucocorticoids justifying inclusion for all active LN. METHODS: We conducted a systematic literature review (SLR) using the term 'pulse glucocorticoids in LN' in order to identify studies that summarise the pharmacologic mechanisms of glucocorticoids, reviewed the

Efficacy of Oral Dextrose Gel for Neonatal Hypoglycaemia: An Updated Systematic Review and Meta-Analysis of Randomized Controlled Trials With 4368 Patients.

BACKGROUND: Neonatal hypoglycaemia is a common metabolic disorder affecting up to 15% of healthy neonates and a higher proportion of at-risk infants. Oral 40% dextrose gel has emerged as a promising, low-cost intervention, but recent evidence remains inconsistent. This updated meta-analysis evaluated the efficacy of 40% oral dextrose gel for preventing and treating neonatal hypoglycaemia. METHODS: We systematically searched PubMed, Embase, Web of Science, Scopus and the Cochrane Library from inception to March 2026 for randomized controlled trials evaluating 40% oral dextrose gel in neonates
