Severe coronary artery spasm induced by epidural injection of bupivacaine hydrochloride: a case report.

This report describes a case of variant angina induced by epidural infusion of bupivacaine hydrochloride for the treatment of intractable low back pain in a 66-year-old male patient with lumbar discopathy. Severe reversible coronary artery spasm of right coronary artery was demonstrated by coronary angiography. Withdrawal of epidural anesthesia and treatment with nitrates and calcium channel antagonists resulted in cessation of variant angina.

Ropivacaine in peribulbar anesthesia - vasoconstrictive properties.

BACKGROUND AND OBJECTIVES: Peribulbar anesthesia can reduce ocular blood flow (OBF) by increasing intraocular pressure (IOP) or due to the action of drugs. Ropivacaine has low toxicity and intrinsic vasoconstrictive properties, yet to be proven on the ocular vasculature. Measurements of ocular pulse amplitude (OPA) allow the indirect evaluation of the OBF. The objective of the present study was to evaluate through the OBF the vasoconstrictive properties of ropivacaine in peribulbar anesthesia. METHODS: Forty eyes undergoing peribulbar anesthesia with 7 mL of anesthetic solution without vasoco

Successful resuscitation following ropivacaine-induced systemic toxicity in a neonate.

A 5-week-old preterm infant was scheduled for inguinal hernia repair. Following induction of general anaesthesia, 10 mg.kg(-1) ropivacaine was injected, accidently, into the caudal space. The infant developed cardiac depression with bradycardia (minimum heart rate 50 beats.min(-1) ), elevated T waves and widening of QRS complexes. Resuscitation by means of external chest compression, intravenous adrenaline and fluid administration was successful. Ropivacaine serum concentrations were obtained at three time points yielding a peak level of 6 μg.ml(-1) 20 min after caudal injection.

Injection treatment and back pain associated with degenerative lumbar spinal stenosis in older adults.

BACKGROUND: Lower back pain is one of the most common health-related complaints in the adult population. Thirty percent of Americans 65 years and older reported symptoms of lower back pain in 2004 (NCHS, 2006). Injection treatment is a commonly used non-surgical procedure to alleviate lower back pain in older adults. However, the effectiveness of injection treatment, particularly in older adults, has not been well documented. OBJECTIVE: This study quantified the effectiveness of injection treatment on pain relief among adults 60 years and over who were diagnosed with degenerative lumbar spina

Encapsulation and sustained release from biodegradable microcapsules made by emulsification/freeze drying and spray/freeze drying.

Hollow biodegradable poly(DL-lactide) (PLA) particles with porous shell walls were prepared by freeze drying small droplets of PLA solution formed by emulsification or spraying. The hollow freeze-dried particles were dispersed in water, and the resulting aqueous suspensions were exposed to plasticizing solvents, either dichloromethane or compressed carbon dioxide. The plasticizing solvent causes the pores in the shell wall to close, forming microcapsules surrounding an aqueous core. A water soluble drug, procaine hydrochloride, was successfully encapsulated in the microcapsule core. The encaps

Oblique sub-costal transversus abdominis plane (TAP) catheters: an alternative to epidural analgesia after upper abdominal surgery.

The authors present three cases where catheters placed in the oblique sub-costal transversus abdominis plane provided prolonged analgesia after upper abdominal surgery. Patient 1 was admitted with severe sepsis following major hepatobiliary surgery. Bilateral catheters facilitated weaning from mechanical ventilation and provided adequate analgesia for 4 days. Patient 2 underwent emergency laparotomy for intestinal obstruction having refused consent for epidural analgesia. The transversus abdominis plane catheters provided a significant opioid sparing effect. A unilateral catheter offered rescu

[Cardiac arrest after axillary plexic anaesthesia with ropivacaine in a chronic kidney failure dialysis patient].

The local anaesthetics are responsible for an important part of morbidity and mortality during conduction anaesthesia. We describe fatal accident plexic anaesthesia with ropivacaine in spite of a classical reanimation measure in a dialysis patient.

Intraurethral fluorouracil and lidocaine for intraurethral condyloma acuminata.

PURPOSE: A new and convenient means of administering fluorouracil and lidocaine for the treatment of intraurethral condyloma acuminata is discussed. SUMMARY: Condyloma acuminata are warts of the genital and perianal region caused by various types of human papillomavirus (HPV). Intraurethral condylomas are associated with complications such as urinary burning, frequency, urgency, urethral bleeding, obstruction, fistula formation, and dyspareunia. A 55-year-old white man had a chief complaint of profuse, but painless, hematuria when he urinated. Cystourethroscopy confirmed extensive intraurethr

Cross-reactions among parabens, para-phenylenediamine, and benzocaine: a retrospective analysis of patch testing.

BACKGROUND: Parabens are alkyl ester derivatives of para-hydroxybenzoic acid and are the most commonly used preservatives in the cosmetic industry. Cross-reactions with para-amino compounds, namely, benzocaine and para-phenylenediamine (PPD), have been reported but are thought to be extremely rare. OBJECTIVE: To evaluate the rate of cross-reactivity between parabens, PPD, and benzocaine in a population of patients patch-tested in a hospital-based contact dermatitis clinic. METHODS: A retrospective analysis of 4,368 patients consecutively patch-tested between July 1989 and June 2005. RESULTS

Thoracic cross-over pathways of the rat vagal trunks.

It is very difficult to study the independent contributions of the afferent and efferent pathways of the subdiaphragmatic vagus to physiology and behavior. Total subdiaphragmatic vagotomy can confound the interpretation of experimental results because it destroys both afferent and efferent vagal fibers. One approach to address this problem involves producing a total ablation of afferent (or efferent) vagal fibers while retaining half of the efferent (or afferent) vagal fibers by making a unilateral rhizotomy plus contralateral subdiaphragmatic vagotomy. However, the completeness of this affere

Repetitive low-energy shock wave application without local anesthesia is more efficient than repetitive low-energy shock wave application with local anesthesia in the treatment of chronic plantar fasciitis.

BACKGROUND: It remains unclear whether application of local anesthesia (LA) interferes with clinical efficacy of extracorporeal shock wave therapy (ESWT) for chronic plantar fasciitis. AIMS: To evaluate the effect of local anesthesia on the clinical outcome after repetitive low-energy ESWT for chronic plantar fasciitis. METHODS: Eighty-six patients with chronic plantar fasciitis were randomly assigned to receive either low-energy ESWT without LA, given weekly for three weeks (Group I, n=45; 3 x 2000 pulses, total energy flux density per shock 0.09 mJ/mm2) or identical ESWT with LA (Group II,
