Cognitive impairment and McArdle disease: Is there a link?

McArdle disease is caused by deficiency of myophosphorylase, the muscle isoform of glycogen phosphorylase. This isoform is also expressed in astrocytes, where it seems to have a key role in neural energy metabolism. Whereas in other glycogen storage diseases cognitive impairment has been rarely reported, it has not been previously observed in McArdle disease. Here we report the case of an Italian 55-year-old woman with McArdle disease and cognitive impairment with bilateral dysfunction of prefrontal and frontal cortex (shown by neuropsychological assessment and fluorodeoxyglucose PET). Further…

Neuronal and glial inclusions in frontotemporal dementia with or without motor neuron disease are immunopositive for p62.

We examined the immunoreactivity of p62 in five cases of frontotemporal dementia (FTD) with ubiquitin-positive, tau-negative inclusions. Only one case had clinical features suggestive of motor neuron disease (MND). In all cases, ubiquitin-positive neuronal inclusions and neurites in the hippocampal region and cerebral neocortex were immunohistochemically positive for p62. Moreover, in the temporal region of a case of FTD with MND, many oligodendrocytes and some astrocytes were positive for p62. These results suggest that the degenerative process involves p62 in FTD and that the process takes p…

Chimeric brains generated by intraventricular transplantation of fetal human brain cells into embryonic rats.

Limited experimental access to the central nervous system (CNS) is a key problem in the study of human neural development, disease, and regeneration. We have addressed this problem by generating neural chimeras composed of human and rodent cells. Fetal human brain cells implanted into the cerebral ventricles of embryonic rats incorporate individually into all major compartments of the brain, generating widespread CNS chimerism. The human cells differentiate into neurons, astrocytes, and oligodendrocytes, which populate the host fore-, mid-, and hindbrain. These chimeras provide a unique model …

Ethanol-induced disturbances of gliogenesis and neuronogenesis in the developing murine brain: an in vitro and in vivo immunohistochemical and ultrastructural study.

The effects of ethanol on murine brain development were studied in a model of whole embryo culture and in vivo. Ethanol locally enhances cell death in the primitive neuroepithelium. During neuronal migration, ethanol induces a premature transformation of the radial glial guides into astrocytes. Ethanol also inhibits the late gliogenesis. The resulting postmigratory neocortex displays an abnormal neuronal pattern almost completely deprived of vertical columnization. These glial-neuronal disturbances can explain neuropathological and clinical features of the fetal alcohol syndrome. Our study pro…

Ultrastructural pathology of axons and myelin in experimental scrapie in hamsters and bovine spongiform encephalopathy in cattle and a comparison with the panencephalopathic type of Creutzfeldt-Jakob disease.

We report the ultrastructural pathology of axons and myelin sheaths in bovine spongiform encephalopathy (BSE) and experimental scrapie in hamsters and compare it with that found in a panencephalopathic model of Creutzfeldt-Jakob disease (CJD). Intramyelinic vacuoles (myelin ballooning), dystrophic axons, phagocytic astrocytes and macrophages were found in all three models but to different degrees, while axons containing numerous cellular processes and concentric cisterns were observed only in experimental scrapie and CJD. We conclude that axonal and myelin pathology is a widespread phenomenon …

Subependymoma and unexpected death.

A previously healthy 53-year-old man died suddenly and unexpected. The main finding at the post mortem examination was a large 4th ventricle subependymoma, a rare brain tumour related to the ventricles of the brain. The salient features of subependymoma are described with emphasis on the microscopic picture and distinction from subependymal giant cell astrocytoma, as the tumour harboured large, bizarre astrocytes. Coronary atherosclerosis and mild cardiac hypertrophy were present as the only pathologic changes outside the central nervous system. The cause of death is discussed in light of earl…

Glial outgrowth and central-type myelination of regenerating axons in spinal nerve roots following transection and suture: light and electron microscopic study in the pig.

Glial bundles growing out of the spinal cord were observed in spinal nerve roots in the pig after their section and surgical repair. The heterotopic occurrence of glial cells, most of which were identified as astrocytes, was more frequent and more pronounced in dorsal than in ventral roots. Several of the glial bundles contained regenerated myelinated axons. These were probably myelinated by cells which showed the typical cytological criteria for oligodendrocytes and the mode of myelination was clearly of central type. It was concluded that the glial outgrowth is induced by degeneration and re…

Juvenile astrocytomas with subarachnoid spread.

The surgical and necropsy findings in five cases of extracerebellar astrocytoma showing subarachnoid spread and invasion of surrounding structures are presented. The patients varied in age from 6 to 23 yr, and all died of the effects of the tumour within 20 mth of the onset of the symptoms. In each case histological examination reveals areas of piloid astrocytes, but these show varying degrees of nuclear abnormalities. Oligodendrocytomatous foci are also seen in some cases. It is suggested that nuclear pleomorphism or oligodendrocytomatous areas indicate a poor prognosis in an otherwise typica…

Short Circuit: Understanding and Treating Neuropathic Pain.

Toll-like receptors (TLRs) play a significant role in the development and maintenance of neuropathic pain and are expressed widely within the central nervous system on neurons, microglia, and astrocytes. TLRs detect both external (pathogen-associated molecular patterns) and internal (damage-associated molecular patterns) signals of inflammation. Chronic neuropathic pain has a prevalence of 6.9% and 10% of the general population. The diagnosis of neuropathic pain in the clinical setting commonly relies on a thorough history and physical examination. A multimodal and multidisciplinary approach i…

Short communication: Evaluating roles of plasma glial fibrillary acidic protein as Alzheimer's disease biomarker in real-world multi-center memory clinics in Thailand.

The roles of reactive astrocytes in Alzheimer's disease (AD) and the correlation between plasma glial fibrillary acidic protein (GFAP) and amyloid-β are emerging. Among 133 patients with cognitive complaints from multi-center memory clinics in Thailand, 73 had AD as defined either by cerebrospinal fluid core biomarkers or amyloid PET. Plasma GFAP demonstrated an AUC of 0.74 (95%CI: 0.65-0.83) for detecting AD and showed large effects on identifying AD status with Cohen's d = 0.81 (95%CI 0.44-1.18). LOESS regression illustrated that plasma GFAP increased from the early stages of AD. Plasma GFAP…

Exploring JC Polyomavirus Sequences and Human Gene Expression in Brain Tissue of Patients With Progressive Multifocal Leukoencephalopathy.

Progressive multifocal leukoencephalopathy (PML) is a rare neurological condition associated with reactivation of dormant JC polyomavirus (JCPyV). In this study, we characterized gene expression and JCPyV rearrangements in PML brain tissue. Infection of white matter astrocytes and oligodendrocytes as well as occasional brain cortex neurons was shown. PML brain harbored exclusively rearranged JCPyV variants. Viral transcripts covered the whole genome on both strands. Strong differential expression of human genes associated with neuroinflammation, blood-brain barrier permeability, and neurodegen…

A 36-year-old Man with Limb Weakness and Dimness of Vision (NMO).

Neuromyelitis Optica (NMO) previously known as Devic disease is inflammatory disorder of central nervous system characterized by severe immune mediated demyelination and axonal damage predominating targeting optic nerves and spinal cord leading to blindness and paralysis. The spectrum of the disease has expanded based on the specificity of the autoimmune response to the aquaporin-4 (AQP-4) water channel expressed on the end feet of astrocytes in the central nervous system. The coordinated immunological attack against aquaporin-4 is mediated by B & T cells, innate cells including neutrophils an…

Roles of Pericytes in Stroke Pathogenesis.

Stroke is a cerebrovascular disorder that affects many people worldwide. In addition to the well-established functions of astrocytes and microglia in stroke pathogenesis, pericytes also play an important role in stroke progression and recovery. As perivascular multi-potent cells and an important component of the blood-brain barrier (BBB), pericytes have been shown to exert a large variety of functions, including serving as stem/progenitor cells and maintaining BBB integrity. Here in this review, we summarize the roles of pericytes in stroke pathogenesis, with a focus on their effects in cerebr…

Septins in the glial cells of the nervous system.

The capacity of cytoskeletal septins to mediate diverse cellular processes is related to their ability to assemble as distinct heterooligomers and higher order structures. However, in many cell types the functional relevance of septins is not well understood. This minireview provides a brief overview of our current knowledge about septins in the non-neuronal cells of the vertebrate nervous system, collectively termed 'glial cells', i.e., astrocytes, microglia, oligodendrocytes, and Schwann cells. The dysregulation of septins observed in various models of myelin pathology is discussed with resp…

Discovery of peptoid ligands for anti-aquaporin 4 antibodies.

Neuromyelitis optica (NMO) is an autoimmune inflammatory disorder of the central nervous system. In most NMO patients, autoantibodies to the water channel protein Aquaporin 4 (AQP4) are present at high levels and are thought to drive pathology by mediating complement-dependent destruction of astrocytes. Here, we apply recently developed chemical library screening technology to identify a synthetic peptoid that binds anti-AQP4 antibodies in the serum of NMO patients. This finding validates, in a well-defined human disease, that synthetic, unnatural ligands for the antigen-binding site of a dise…

Visualizing chemokine-dependent T cell activation and migration in response to central nervous system infection.

In response to central nervous system (CNS) injury and infection, astrocytes, neurons, and CNS vasculature express several chemokines, including CCL21. Quantitative polymerase chain reaction (qPCR), western blot, and immunohistochemical methods can quantify mRNA and protein expression. However, these methods do not quantify chemokine bioavailability and bioactivity, variables modified by many environmental factors including composition of the extracellular matrix (ECM). Here we illustrate how two-photon microscopy and carboxyfluorescein succinimidyl ester (CFSE or CFDA SE) labeling of T cells …

mRNA distribution of the thalidomide binding protein cereblon in adult mouse brain.

Cereblon is implicated in mild mental retardation and proved to bind to a teratogenic hypnotic, thalidomide. Here, we determined cereblon mRNA distributions in adult mouse brain. Almost all neurons expressed cereblon mRNA with various intensities whereas the signals in astrocytes and oligodendrocytes were modest or negligible. Intense mRNA signals were found in the hippocampus and cerebellum, especially in hippocampal pyramidal cells and Purkinje cells. Higher levels of cereblon mRNA were also detected in serotonergic and noradrenergic neurons in raphe nuclei and locus ceruleus, respectively. …
