Prospective cohort study of early biosignatures of response to lithium in bipolar-I-disorders: overview of the H2020-funded R-LiNK initiative.

BACKGROUND: Lithium is recommended as a first line treatment for bipolar disorders. However, only 30% of patients show an optimal outcome and variability in lithium response and tolerability is poorly understood. It remains difficult for clinicians to reliably predict which patients will benefit without recourse to a lengthy treatment trial. Greater precision in the early identification of individuals who are likely to respond to lithium is a significant unmet clinical need. STRUCTURE: The H2020-funded Response to Lithium Network (R-LiNK; http://www.r-link.eu.com/ ) will undertake a prospecti…

Differences in the circuitry-based association of copy numbers and gene expression between the hippocampi of patients with schizophrenia and the hippocampi of patients with bipolar disorder.

CONTEXT: GAD67 regulation involves a network of genes implicated in schizophrenia and bipolar disorder. We have studied the copy number intensities of these genes in specific hippocampal subregions to clarify whether abnormalities of genomic integrity covary with gene expression in a circuitry-based manner. OBJECTIVE: To compare the copy number intensities of genes associated with GAD67 regulation in the stratum oriens of sectors CA3/2 and CA1 in patients with schizophrenia, patients with bipolar disorder, and healthy controls. DESIGN: Samples of sectors CA3/2 and CA1 were obtained from pati…

Association between polymorphism in intron 1 of cocaine- and amphetamine-regulated transcript gene with alcoholism, but not with bipolar disorder and schizophrenia in Korean population.

Cocaine- and amphetamine-regulated transcript (CART) peptide is a novel neuropeptide involved in feeding, drug reward, and stress. We hypothesized that the polymorphism of CART gene might be related with susceptibility to neuropsychiatric diseases such as alcoholism, bipolar disorder, and schizophrenia. The polymorphism (rs2239670) in intron 1 was selected for study among other single nucleotide polymorphisms (SNPs) located at the area of CART gene, because it had not been tested until to date. The study included patients of alcoholism (100), bipolar disorder (76) and schizophrenia (169) from …

On the nature of depressive and anxious states in a family practice setting: the high prevalence of bipolar II and related disorders in a cohort followed longitudinally.

Much of the scientific literature on affective states in primary care settings is derived from instrument-based diagnoses, typically without the benefit of clinical in-depth examination. In a naturalistic family practice setting, we prospectively evaluated 108 consecutive anxious and/or depressed patients. All diagnoses derived from semistructured interviews conducted by a family physician with enhanced training in mood disorders. Nonbipolar depressions were found in 60 of 108 patients (55.6%), nearly half of whom were in the depression not otherwise specified (DNOS) category; yet on careful h

The effect of intranasal (R,S)-ketamine on symptoms of fatigue in severe major depressive disorder or bipolar depression with and without comorbid alcohol use disorder: Results from a randomized, double-blind, placebo-controlled trial.

BACKGROUND: Fatigue is a multidimensional condition that may overlap with depression. Initial studies found that fatigue responds in only a limited way to standard monoaminergic antidepressants and mood stabilizers but does respond positively to intravenous (IV) racemic (R,S)-ketamine (ketamine). However, IV ketamine's use is limited by cost and access barriers. To date, no study has evaluated intranasal (IN) ketamine in individuals with fatigue. This study sought to evaluate the anti-fatigue effects of a single 50 mg dose of IN ketamine in individuals with major depressive disorder (MDD) or b

Protocol for a randomised controlled trial on smartphone- and web-based app for precision dosing and monitoring of lithium («PharmOracle») for improving adherence to therapy in patients with bipolar affective disorder or schizoaffective disorder.

Lithium remains a first-line treatment for bipolar disorder but its use is declining owing to the need for intensive monitoring and concerns about tolerability. Digital decision-support tools that combine model-informed precision dosing, Bayesian therapeutic-drug-monitoring, and adherence support may facilitate safer, more effective lithium therapy and improve long-term adherence. We aimed to evaluate whether adding a mobile application with precision-dosing, monitoring reminders and medication-support functions («PharmOracle») to standard care reduces a composite measure of non-adherence and …

Age and Sex as Moderators of Sleep Architecture in Schizophrenia, Bipolar Disorder, and Unipolar Depression: A Case-Control Polysomnographic Systematic Review and Meta-Regression.

Sleep architecture alterations have been shown in major psychiatric disorders, but the contribution of demographic factors to between-study heterogeneity remains insufficiently characterized. This study reports a transdiagnostic systematic review and meta-regression of case-control polysomnographic studies comparing schizophrenia, bipolar disorder, and unipolar depression with healthy controls. The factors age and sex as possible moderators across harmonized case-control polysomnographic studies have been analyzed. Eligible studies reported diagnostic criteria, treatment status, age, and sex. …

Adjunctive bilateral vs. unilateral or sham repetitive transcranial magnetic stimulation for major depressive disorder or bipolar depression: a meta-analysis of randomized controlled studies.

OBJECTIVES: Bilateral repetitive transcranial magnetic stimulation (brTMS) has shown promise as a therapeutic approach for depression; however, evidence from existing studies remains inconsistent. To address the concern, a meta-analysis was conducted to examine the efficacy and safety of brTMS in comparison with unilateral repetitive transcranial magnetic stimulation (urTMS) or sham repetitive transcranial magnetic stimulation (srTMS) in treating major depressive disorder (MDD) and bipolar depression (BD). METHODS: A systematic search covering publications up to May 22, 2024, was performed ac
