[Treatment of social anxiety disorder (SAD)].

Social anxiety disorder(SAD) is the most common anxiety disorder and it is a risk factor for subsequent substance abuse. The efficacy of selective serotonin reuptake inhibitors (SSRIs) and serotonin noradrenaline reuptake inhibitor(SNRI) for the treatment of SAD has been reported. Cognitive behavioral therapy(CBT) was also effective for the treatment of SAD in many studies. High rate of co-occurring SAD and alcohol use disorder (AUD) can be found in both clinical samples and in the general population. With regard to treatments for those suffering from SAD and AUD, little is known about the mos…

A self psychological and relational approach to group therapy for university students with bulimia.

Bulimia is a complex disorder that can significantly interfere with psychosocial functioning. It is a particularly significant problem among adolescents and young women in college and university settings. The preponderance of literature on group therapeutic approaches for this disorder is from a cognitive-behavioral perspective. In this paper, we present a psychodynamic group therapy approach for female university students, informed specifically by self psychological and relational frameworks. Through clinical illustrations, we demonstrate how the group provides selfobject experiences, includi

A naturalistic study of psychotherapy for bulimia nervosa, part 1: comorbidity and therapeutic outcome.

Data from naturalistic samples provide an important complement to findings from randomized trials of psychotherapy. A random national sample of US clinicians provided data on 145 completed treatments of patients with bulimic symptoms. Treatment in the community was substantially longer than treatment prescribed in manuals, with a mean length of cognitive-behavioral therapy of 69 sessions and significantly longer for eclectic and psychodynamic therapies. Most patients treated in the community had substantial comorbidity, and this comorbidity was associated with longer treatments and poorer outc

[Post-traumatic stress disorders: diagnostic and therapeutic principles ].

Over 50 percent of the general population are exposed to a traumatic event at least once in their lives. Some but not all trauma survivors go on to develop posttraumatic stress disorder (PTSD). The diagnostic criteria of acute stress reaction, posttraumatic stress disorder, and enduring personality change after catastrophic experience are presented. PTSD should primarily be treated by means of psychotherapy. Cognitive-behavioral approaches, exposure therapy in particular, currently show the best evidence as effective treatments for PTSD. In addition to psychotherapy, psychopharmacological trea

Premature ejaculation: a psychophysiological approach for assessment and management.

This article distinguishes several subtypes of biogenic and psychogenic premature ejaculation (PE) according to their etiologic features: the physiological PE types of (a) neurologic constitution, (b) acute physical illness, (c) physical injury, and (d) pharmacologic side effect; and the psychological PE types of (a) psychological constitution, (b) acute psychological distress, (c) relationship distress, and (d) psychosexual skills deficit. Attention is given to assessment and differential diagnosis, and to specific treatment of the types of PE, such as the pharmacologic management of difficul

Clobazam therapy of refractory epilepsy in tuberous sclerosis complex.

Clobazam (CLB) was recently approved by the FDA, but has not been evaluated in tuberous sclerosis complex (TSC). We retrospectively reviewed a cohort of patients with TSC and refractory epilepsy who started CLB over a 5-year period. Clinical characteristics and number of tubers on MRI were assessed. Duration of therapy, therapeutic response and adverse events were recorded. CLB was prescribed in 29 adults and children of whom 72% were cognitively impaired, with a median age at seizure onset of 5 months. Mean duration of CLB therapy was 17.3 months with a 12 and 24-month estimated retention rat

Brain activity pattern changes after adaptive working memory training in multiple sclerosis.

Cognitive impairment and related abnormal brain activity are common in people with multiple sclerosis (PwMS). Adaptive training based on working memory (WM) has been shown to ameliorate cognitive symptoms, although the effects at a neural level are unclear. The aim of this study was to expand the existing research on the effects of an adaptive WM rehabilitative intervention on brain functional activity in PwMS. A sample of eighteen PwMS performed an 8-week home-based cognitive rehabilitation treatment based on adaptive WM training. PwMS were assessed before and after treatment using a validate

Validation of the ED-15-NL, a Brief Session-by-Session Questionnaire to Measure Changes in Eating Disorder Cognitions and Behaviors.

OBJECTIVE: The Eating Disorder-15 (ED-15) questionnaire measures core symptoms of eating disorders (EDs) and is used to track session-by-session symptom change during treatment. The ED-15 was found to have good validity and reliability with a two-factor structure in the original English version, as well as in the Portuguese, Spanish, and Turkish versions. The aim of the current study was to examine the psychometric qualities and factor structure of the Dutch version of the ED-15 (ED-15-NL). METHOD: Fifty-nine patients with an ED (currently receiving CBT-T treatment) and 513 non-clinical contr

Feasibility and effects of cognitive training on cognition and psychosocial function in Huntington's disease: a randomised pilot trial.

BACKGROUND: Huntington's disease (HD) is a rare neurodegenerative disease that causes progressive cognitive, physical, and psychiatric symptoms. Computerised cognitive training (CCT) is a novel intervention that aims to improve and maintain cognitive functions through repeated practice. The effects of CCT have yet to be established in HD. This randomised pilot trial examined the feasibility of a large scale trial to assess efficacy of multidomain CCT in pre-manifest and early-stage HD. METHODS: 28 participants were randomised to either at-home CCT (2 × 60 min sessions per week for 12 weeks; n

Neuropsychological management of multiple sclerosis: evaluation of a supervised and customized cognitive rehabilitation program for self-used at home (SEPIA): protocol for a randomized controlled trial.

BACKGROUND: Cognitive and mood disorders negatively impact daily life in patients with multiple sclerosis (MS). Pharmacological treatments did not demonstrate any effect on cognition compared with cognitive rehabilitation (CR). However, if CR programs offer promising results on cognition, they are less consistent concerning mood and quality of life (QoL). In this context, we designed a randomized controlled trial to evaluate the efficacy of an innovative computerized CR program, conducted at home, on QoL. Secondary objectives will estimate the improvement, or the stabilization over time, of pa

Impact of breast cancer on prospective memory functioning assessed by virtual reality and influence of sleep quality and hormonal therapy: PROSOM-K study.

BACKGROUND: Breast cancer (BC) is the most frequent cancer in women with more than 70% of BC patients being treated with hormonal therapy (HT). Among these patients, some report difficulties in remembering what they are supposed to do at the right moment, referring to prospective memory (PM). PM is essential for autonomy and medical adherence of patients, and requires an ecological assessment. Virtual reality, that recreates naturalistic environment, seems to be a promising method to evaluate PM. Several BC patients also report sleep disturbances. Given the role of sleep on memory consolidatio

Strong subjective recovery as a protective factor against the effects of positive symptoms on quality of life outcomes in schizophrenia.

BACKGROUND: Interest in recovery from schizophrenia has been growing steadily, with much of the focus on remission from psychotic symptoms and a return to functioning. Less is known about the experience of subjective recovery and its relationships with other important outcomes, such as quality of life and the formation and sustenance of social connections. This study sought to address this gap in knowledge by examining the links between self perceived recovery, symptoms, and the social components of quality of life. METHODS: Sixty eight veterans with schizophrenia-spectrum disorders who were

Pharmacology of sexually compulsive behavior.

In a meta-analysis on controlled outcomes evaluations of 22,000 sex offenders, Losel and Schmucker found 80 comparisons between treatment and control groups. The recidivism rate averaged 19% in treated groups, and 27% in controls. Most other reviews reported a lower rate of sexual recidivism in treated sexual offenders. Of 2039 citations in this study (including literature in five languages), 60 studies held independent comparisons. Problematic issues included the control groups; various hormonal, surgical, cognitive behavioral, and psychotherapeutic treatments; and sample sizes. In the 80 stu

Psychosocial and behavioral correlates of persistent pain post disease-modifying treatment change in rheumatoid arthritis: a 12-month cohort study.

BACKGROUND: Pain is a debilitating and persistent symptom of rheumatoid arthritis (RA), associated with impaired functional capacity and reduced quality of life. Despite targeted disease-modifying treatments, many RA patients experience persistent pain, suggesting mechanisms operating independently of classic inflammatory pathways. Psychological factors, such as depression and anxiety, can impact patterns of appraisal and behavioral coping strategies. Understanding how these associations underpin pain symptoms in RA is key for developing targeted symptom management interventions. Using data fr

Adjunctive intermittent theta-burst stimulation for first-episode schizophrenia: A randomized clinical trial.

BACKGROUND: The efficacy of intermittent theta-burst stimulation (iTBS) combined with pharmacotherapy and psychotherapy in first-episode schizophrenia remains unclear. This study evaluated adjunctive iTBS with risperidone and cognitive behavioral therapy (CBT) and explored serum biomarkers indicating treatment response. METHODS: In this randomized, assessor-blind trial, 100 first-episode schizophrenia patients received either iTBS plus risperidone and CBT (iTBS group, n = 50) or risperidone and CBT alone (control, n = 50) for 3 months. The primary outcome was change in PANSS total score at 4 …

Electroacupuncture enhances cerebral blood perfusion by inhibiting HIF-1α in rat subarachnoid hemorrhage.

OBJECTIVE: Cerebral blood perfusion (CBP) reduction is a prevalent complication following subarachnoid hemorrhage (SAH) in clinical practice, often associated with long-term cognitive impairment and prognosis. Electroacupuncture (EA), a widely utilized traditional Chinese therapy for central nervous system disorders, has demonstrated promising therapeutic effects. This study aims to investigate the therapeutic potential of EA in restoring CBP in SAH rats and to explore the mechanisms involving HIF-1α in this process. METHODS: Rats were randomly assigned to one of five groups, including Sham,

Changes in Decentering and Reappraisal Temporally Precede Symptom Reduction During Emotion Regulation Therapy for Generalized Anxiety Disorder With and Without Co-Occurring Depression.

Emotion Regulation Therapy (ERT) has demonstrated efficacy in both open and randomized trials for generalized anxiety disorder (GAD) with and without co-occurring depression. An important goal in ERT is to teach clients adaptive emotion regulation, including healthier metacognitive abilities such as decentering and cognitive reappraisal. A few studies thus far have demonstrated a mediating role for these metacognitive abilities in other cognitive behavioral therapies (CBTs) for GAD. However, a drawback to most of these has been the ability to demonstrate a causal role for improved metacognitiv

Neurophysiological markers that predict and track treatment outcomes in childhood anxiety.

The present study examined the cortical processes that mediate cognitive regulation in response to emotion-eliciting stimuli, before and after anxious children participated in a cognitive behavioral therapy program. Electroencephalographic activity was recorded from anxious children (n = 24, 8 males) and comparison children (n = 16, 7 males) at pre-and post-treatment sessions. The change in anxiety T-scores from pre- to post-treatment was used to signify clinical improvement among anxious children (Improvers: n = 11 vs. Non-improvers: n = 13). Event-related potential components were recorded w…
