2 patients, aged 8 mth and 48 yr, referred for a clinical diagnosis of ventricular septal defect, were studied by ultrasound. In both a fluttering and an anterior bowing of the systolic segment of the tricuspid valve was found, consistent with a left ventricular--right atrial communication. Diagnosis was confirmed by angiography and open-heart surgery, showing in 1 patient a supravalvular defect, and an infravalvular one in the other. Metoclopramide, a procainamide derivative with a documented effect on the gastro-intestinal motility, was given to four patients with hydroureter. It induced strong peristalsis in the ureter, manifested clinically as relief of pain and fluoroscopically as emptying of the ureter and the renal pelvis.

Ceforanide (BL-S 786) is a new long-acting parenteral cephalosporin which has the major pharmacologic advantage of requiring only twice a day dosage. We treated 28 adult patients with community-acquired bacterial pneumonia using doses of 500 or 1000 mg every 12 hours. Twenty-four of 28 infections were due to Streptococcus pneumoniae and/or Hemophilus influenzae, and all pathogens were susceptible in vitro to both cephalothin and ceforanide. In three adjacent Newfoundland communities comprising some 1500 people, 589 people have been HLA typed. Forty-six of the typed people gave a history of previous clinical tuberculosis which required treatment. Fifty-six percent of the TB patients carried HLA B8 compared with 20% of the remainder of the population. This is a highly significant difference (P less than 0.01). The intensity and prevalence of Schistosoma haematobium infection has been measured in a community in the laterite plateau area of the McCarthy Island Division, The Gambia. The prevalence reaches 100% by the age of 10 years and counts of over 1000 ova in 10 ml of noon urine are common. During the second and third decade prevalence and intensity of infection decline and the mean egg output of a cohort falls by approximately 50% every three years. In subjects aged over 30 sex differences in exposure to infection and egg output suggest the possible effect of protective immunity.

Over a ten year period of a man 40 had a progressively increasing atrioventricular conduction defect, resulting from a ventricular septal defect with eventual cardlac failure due to persistant bradycardia. In a study of 106 predominantly young, lower-social-class men participating in a methadone maintenance program, the authors found that about one-third were moderately to severely depressed as assessed on standard rating scales of depression. The depressive symptoms were associated with a decrease in social functioning, increase in stress in the past 6 months, and a history of alcohol abuse. Fecal specimens from 32 champanzees living in Gombe National Park, Tanzania, were examined. Six species of helminths and 2 species of ciliates were found: Probstmayris gombensis File (in press), Strongyloides fuelleborni von Linstow 1905, Necator sp., Oesophagostomum sp., Abbreviata caucasica von Linstow 1902, Trichuris sp., Troglodytella abrassarti Brumpt and Joyeux 1921, and an unidentified ciliate. None of the parasitic infections were heavy. This is the first such survey of the chimpanzee in its natural habitat.

All (151) adult patients in north-east Scotland newly diagnosed in one year (1967) as having a supposedly benign definite gastric ulcer crater were studied after a minimum of four years, with a 98 per cent follow-up.
