S-allylcysteine Improves Blood Flow Recovery and Prevents Ischemic Injury by Augmenting Neovasculogenesis.

Studies suggest that a low level of circulating human endothelial progenitor cells (EPCs) is a risk factor for ischemic injury and coronary artery disease (CAD). Consumption of S-allylcysteine (SAC) is known to prevent CAD. However, the protective effects of SAC on the ischemic injury are not yet clear. In this study, we examined whether SAC could improve blood flow recovery in ischemic tissues through EPC-mediated neovasculogenesis. The results demonstrate that SAC significantly enhances the neovasculogenesis of EPCs in vitro. The molecular mechanisms for SAC enhancement of neovasculogenesis

Use of prostacyclin and its analogues in the treatment of cardiovascular disease.

Prostacyclin is found in all body tissues and body fluids, and is the major metabolite of arachidonic acid in the vasculature. It is a potent vasodilator that affects both the systemic and pulmonary circulations. Prostacyclin also prevents vascular smooth muscle proliferation and inhibits platelet adhesion and aggregation. These features have made it a very attractive substance for treatment of many different cardiovascular diseases. Epoprostenol, a synthetic prostacyclin, is currently available in parenteral form for the treatment of primary pulmonary hypertension, and has been shown to be a

Heart rate and metabolic response to competitive squash in veteran players: identification of risk factors for sudden cardiac death.

We studied 10 older males during a competitive game and the early post-exercise period to define the metabolic response to squash in veteran players. For comparison, all subjects were also studied during exhaustive treadmill exercise. Squash caused a dramatic increase in heart rate (150%), and circulating levels of noradrenaline (164%), adrenaline (93%), lactate (202%) and free fatty acids (67%). These effects were independent of haemoconcentration. The early post-exercise period (5 min) was characterized by persistent elevation of plasma catecholamines, lactate, and free fatty acids, hypokala

[The lung, producer of prostacyclin].

Under physiological conditions prostacyclin (PGI2) is the main metabolite of arachidonic acid that is generated and released by lungs. The generation of PGI2 by lungs seems to be a continuous process and, therefore, PGI2 can be considered as a circulating hormone whose concentration is by 100--200 pg/ml higher in arterial blood than in mixed venous blood. The generation of PGI2 by lungs can be augmented by angiotensin ll, bradykinin and arachidonic acid provided that low concentrations of these substances are infused into pulmonary artery. Also air pulmonary emboli and mechanical hyperventilat

Progression of Coronary Artery Disease in Patients Undergoing Transcatheter Aortic Valve Replacement: A Quantitative Coronary Angiography and Quantitative Flow Ratio Analysis.

BACKGROUND: The progression of coronary artery disease (CAD) following transcatheter aortic valve replacement (TAVR) is poorly understood. Changes in coronary hemodynamics after TAVR and their relationship with atherosclerosis remain an active area of investigation. Quantitative coronary angiography (QCA) and quantitative flow ratio (QFR) offer objective, reproducible assessments of coronary disease burden, hemodynamics, and progression. AIMS: To evaluate anatomic and hemodynamic progression of CAD in select patients undergoing TAVR using QCA and QFR, and to compare outcomes by valve type-sel

Effect of State-of-the-Art Treatment on Symptoms, Absolute Coronary Artery Flow, and Resistance Among Patients With Ischemia and Non-Obstructive Coronary Arteries (STAR-INOCA Study): Study Design and Rationale.

A significant proportion of patients referred to the cardiac catheterization laboratory for ischemia and/or angina do not have obstructive coronary arteries. This condition (INOCA and/or ANOCA, respectively) is frequently associated with coronary microvascular dysfunction (CMD). CMD causes myocardial ischemia, persistent symptoms, impaired quality of life, and increased healthcare utilization. Despite its clinical relevance, evidence-based therapeutic strategies remain limited, as prior studies regarding the treatment have been small and heterogeneous. Recently, an expert consensus proposed a

Could Pan-Immune-Inflammation Value be a Marker for the Diagnosis of Coronary Slow Flow Phenomenon?

Inflammation plays a key role in the pathogenesis of the coronary slow flow phenomenon (CSFP). The newly developed inflammatory marker, pan-immune-inflammation value (PIV), is associated with adverse cardiovascular events. This study investigated the predictive value of PIV for diagnosing CSFP in comparison to other inflammation-based markers. A total of 214 patients, 109 in the CSFP group and 105 in the normal coronary flow (NCF) group, were retrospectively included in the study. Coronary flow was calculated using the Thrombolysis in Myocardial Infarction frame count method. In addition to PI

Haptoglobin Phenotype Is Associated With High-Density Lipoprotein-Bound Hemoglobin Content and Coronary Endothelial Dysfunction in Patients With Mild Nonobstructive Coronary Artery Disease.

Objective- Coronary endothelial dysfunction (ED) is an early stage of atherosclerosis and is associated with impaired high-density lipoprotein (HDL) function. A functional polymorphism at the haptoglobin (Hp) gene locus (rs72294371) has been associated with marked differences in HDL structure and function. We sought to determine whether Hp phenotype was associated with coronary ED and whether the amount of hemoglobin (Hb) tethered to HDL via Hp was Hp-type dependent and associated with ED. Approach and Results- Microvascular and epicardial coronary endothelial function was assessed in 338 indi

A Novel Intra-aortic Device Designed for Coronary Blood Flow Amplification in Unrevascularizable Patients.

Patients with unrevascularizable coronary artery disease represent a substantial number of all patients with coronary disease. However, their therapeutic options are limited; they endure recurrent hospitalizations, a poor quality of life and prognosis. We aim to investigate a novel alternative approach to the treatment of this common medical condition by using a specialized intra-aortic device with coiling properties capable of enhancing diastolic coronary flow. Both a mathematical analysis and in vitro study presented in the current study have yielded enhanced coronary diastolic blood flow an
