[Lumbar spinal surgery in elderly patients].

INTRODUCTION: In the geriatric population, pain with sciatic irradiation requires a differential diagnosis to enable a distinction to be made mainly between a herniated disc, lateral recess stenosis or lumbar stenosis. In addition, in many cases the degenerative problems are often associated with lumbar listhesis or instability. Furthermore, these patients present very diverse associated cardiovascular, pulmonary or metabolic pathologies which can make surgery complicated and, above all, prolong post-operative recovery, as well as increasing morbidity and mortality. PATIENTS AND METHODS: We r

Scaphoid Non-union Arthroscopic Treatment without Bone Graft: Is it Possible?

Scaphoid fractures are the most frequent fractures of carpal bones, accounting for 70 to 80% of all carpal fractures. Appropriate and early diagnosis of scaphoid fractures is imperative. To relieve symptoms and prevent degenerative changes, scaphoid non-union surgery should aim at healing and restoration of the normal anatomy, and correction of carpal deformities. Since the scaphoid non-unions are a heterogeneous group, and categorization is not easy, the management remains controversial with a reported failure rate as high as 40% and the absence of a "gold standard" treatment. Based on (a) Qu

Assessment of Transserosal Microcirculation with Visible Light Spectroscopy and Laser Doppler Flowmetry in Patients with Median Arcuate Ligament Syndrome and Chronic Mesenteric Ischemia.

INTRODUCTION: Previous studies with visible light spectroscopy (VLS) and laser Doppler flowmetry (LDF) have shown reduced mucosal circulation of the stomach and duodenal wall in patients with median arcuate ligament syndrome (MALS) and chronic mesenteric ischemia (CMI). However, transserosal microcirculatory assessment during the operative treatment of patients with these conditions has not yet been performed. We aimed to investigate if laparoscopic decompression for MALS and aortomesenteric bypass in CMI can result in immediate measurable increase in the microcirculation in stomach and duoden

Optimal Timing of Cranioplasty After Decompressive Craniectomy: Timing or Collapse Ratio.

BACKGROUND AND OBJECTIVES: Although cranioplasty (CP) is a relatively straightforward surgical procedure, it is associated with a high complication rate. The optimal timing for this surgery remains undetermined. This study aimed to identify the most suitable timing for CP to minimize postoperative complications. METHODS: We conducted a retrospective analysis of all CP cases performed in our department from August 2015 to March 2022. Data were gathered through case statistics and categorized based on the occurrence of complications. The collapse ratio was determined using 3-dimensional Slicer

Transcervical, retropharyngeal odontoidectomy - Anatomical considerations.

CONTEXT: Anterior craniocervical junction lesions have always been a challenge for neurosurgeons. Presenting with lower cranial nerve dysfunction and symptoms of brainstem compression, decompression is often required. While posterior approaches offer indirect ventral brainstem decompression, direct decompression via odontoidectomy is necessary when they fail. The transoral and endoscopic endonasal approaches have been explored but come with their own limitations and risks. A novel retropharyngeal approach to the cervical spine has shown promising results with reduced complications. AIMS: This

Effectiveness of Multicentric Therapy Post Posterior Cervical Decompression and Fusion for Cervical Disc Disease: A Case Report.

In clinical practice, cervical disc abnormalities such as herniated nucleus pulposus (HNP), degenerative disc disease (DDD), and internal disc disturbance are seen. DDD includes degeneration causing annular tears, a decrease in disc height, and nuclear degradation. Cervical stenosis with myelopathy can be caused by anything that narrows the spinal canal and compresses the spinal cord, such as bone spurs, herniated discs, or bulging ligaments. Posterior cervical decompression and fusion (PCDF) is a standard surgical treatment for treating numerous cervical spine diseases. It has been suggested

Risk of posterior circulation stroke in patients with cervical spondylosis: A nationwide, population-based study.

BACKGROUND AND AIMS: Cervical spondylosis (CS) is reported to be associated with vertebrobasilar insufficiency. However, few cohort studies have investigated the association between CS and posterior circulation ischemic stroke. METHODS: The study cohort comprised 27,990 patients aged ≥18 years with a first diagnosis of CS. The controls consisted of patients with propensity score matched for age, sex, and comorbidities at a ratio of 1:1. We investigated the relationships of CS with ischemic stroke and all-cause mortality. Cox regression was used to estimate hazard ratios (HRs) and 95% confiden

Clinical outcomes of the LORDS strategy for thoracolumbar spinal metastases: a retrospective cohort study.

OBJECTIVE: To compare surgical outcomes between patients with spinal metastases who received surgical treatment concordant with the LORDS (life expectancy, operability, responsible segment, decompression, and stabilization) strategy and those who did not. BACKGROUND: The Neurologic, Oncologic, Mechanical, and Systemic (NOMS) framework is widely used to guide decision-making in spinal metastases. With advances in targeted therapy, immunotherapy, and surgical techniques, more patients are now candidates for anterior column resection. However, a balanced surgical strategy that is compatible with

Defining the associations between weight, obesity and outcomes of posterior fossa decompression for Chiari malformation in adolescents.

BACKGROUND: Posterior fossa decompression in the setting of Chiari malformation is a common neurosurgical procedure in adolescents; however, the effect of obesity on its surgical outcomes is understudied. This is partly due to the lower incidence of obesity in adolescents requiring higher statistical power. Correspondingly, we sought to interrogate a national surgical database to aggregate a significant cohort size to elucidate possible relationships between obesity and surgical outcomes in this niche. METHODS: The American College of Surgeons National Surgical Quality Improvement Program-Ped
