Contact dermatitis caused by airborne agents. A review and case reports.

A general review is given of airborne-induced contact dermatoses, particularly of the irritant and allergenic types. Because the reports in the literature often omit the term airborne, 12 volumes of Contact Dermatitis (January 1975-July 1985) were screened, and the cases cited were classified in function of the anamnesis, lesion locations, causative irritants and allergens, and other factors. The present article also discusses differential diagnoses, in particular with regard to contact dermatitis of the face, ears, and neck. Finally, seven case reports of occupational and nonoccupational cont…

The effect of grenz rays on the expression of allergic contact dermatitis in man.

To investigate the effect of grenz rays on the expression of allergic contact dermatitis, six patients hypersensitive to nickel were studied. They were given 3 Gy of grenz rays, once a week for 3 weeks, to a defined area of the back. Twenty-four hours after the last treatment serial dilution nickel patch tests were applied on the grenz-ray-treated area and on untreated control skin. There was an almost total suppression of the allergic contact dermatitis at the site of grenz ray treatment. Biopsy specimens were taken from nickel-challenged and unchallenged skin, both from the grenz-ray-treated…

Concurrence of lupus erythematosus and dermatitis herpetiformis. A report of nine cases.

Nine cases of lupus erythematosus (LE) associated with dermatitis herpetiformis (DH), seen at the Mayo Clinic, Rochester, Minn, were reviewed. The female to male ratio was seven to two. In seven patients DH developed first, followed by systemic LE in five and by discoid LE in two. In two patients systemic LE developed first, followed by DH. Since multiple autoimmune diseases have been reported in association with LE, and since similar HLA antigens have been found in patients with DH or LE, there is very likely an immunologic relationship between the two diseases.

Evidence for histamine-mediated inhibition of monocyte chemotaxis in atopic dermatitis.

Leukocyte chemotaxis was studied in 11 patients with severe childhood onset atopic dermatitis at a time when their disease was relatively quiescent. Pyoderma had been an important complication of the dermatitis in these patients. The chemotactic responsiveness of patient neutrophils and monocytes was on the average not significantly different from that of healthy control subjects, although three patients were identified who had significantly impaired responses. No correlation between IgE levels and leukocyte chemotaxis was observed. Because excessive amounts of histamine have been recovered fr…

Occupational contact dermatitis in the workers employed in production of Texas textiles.

Pathological changes in the skin, of allergic type, have been detected in 35 of 131 weavers, employed in the production of primarily texas textiles. Patch tests yielded in 21 patients positive results with cotton and texas textiles. Approximately 50% of positive results of dermal tests have been found to be induced by naphthol compounds added to dyes in the process of preliminary dying. Moreover, 2 cases of extensive hyperchromatism in persons concerned with the above mentioned textiles have been observed which, due to the treatment procedures and removed from work in exposure have partly subs

Dermatitis artefacta induced in a patient by one of her multiple personalities.

A young woman suffered for 5 weeks with an inexplicable, severe dermatitis limited to her left arm. During hospitalization it was found that she had multiple personalities, one of whom had been applying the leaves of the poison ivy plant each night to her left arm, using 'his' gloved right hand. The factitial nature of the dermatitis was further documented by the fact that when the 'responsible personality' was cut off from supplies of poison ivy in the hospital, 'he' produced factitious haematomas by trauma to her left hand. Although this appears to be the first report of dermatitis artefacta…

A case of contact dermatitis, erythema multiforme, and toxic epidermal necrolysis.

A young adult female patient, on prolonged corticosteroid therapy for nephrotic syndrome, developed erythema multiforme and toxic epidermal necrolysis following a double exposure to a locally applied perfume. We believe that this route of exposure should be re-emphasized. Toxic epidermal necrolysis, or the scalded skin syndrome, has become a well-recognized entity which may be divided into three distinct subgroups: the staphylococcal scalded skin syndrome, nonstaphylococcal or drug-induced scalded skin syndrome, and idiopathic. We present a patient who developed a dermatitis on her chest and a…

Measurement and differentiation of the cellular infiltrate in experimental toxic contact dermatitis.

Toxic (irritant) contact dermatitis was elicited by epicutaneous application of dinitrochlorobenzene (DNCB) and croton oil in unsensitized guinea pigs. The course of the skin reaction was studied with the naked eye, low-power microscopy, and a method based on counting of infiltrating cells in the upper corium. A weakly toxic dose of DNCB (100 microgram/cm2) gave marked erythema and moderate swelling, more pronounced 6 h after application of the DNCB than at 24 h or 48 h. A significant increase in mononuclear cells in particular but also in neutrophil and eosinophil granulocytes was noted in th…

Brilliant Lake Red R as a cause of pigmented contact dermatitis.

Twenty-three patients suffering from pigmented contact dermatitis caused by cosmetics containing Brilliant Lake Red R were observed. Commercial samples of Brilliant Lake Red R proved to contain many ethyl acetate extractable impurities; 1-phenylazo-2-naphthol and azobenzene were isolated and identified. To determine the responsible allergens, five patients were examined by patch tests with purified samples of azo-dyes and the unidentified fractions of ethyl acetate extractable impurities. Three out of five showed weaker reactions to purified samples of Brilliant Lake Red R and the other two sh…

Identification of IgA binding structures in skin of patients with dermatitis herpetiformis.

Immunoelectronmicroscopic and ultrastructural identification of the structures which bind IgA in skin of patients with dermatitis herpetiformis (DH) reveals them to be, in part, a complex of fibrillar components which are covered with amorphous substances. One of the fibers has a diameter of 80--130 A and appears to be tubular, resembling dermal microfibrillar bundles or microtubular elements of elastic fibers. The structures, at times, are associated with the dermal microfibrillar bundles and are found within an elastic fiber system which is in close proximity to the dermal-epidermal junction…

Immunoglobulins in jejunal mucosa and serum from patients with dermatitis herpetiformis.

The quantitative distribution of immunoglobulin-(Ig)-producing cells of the major classes was determined by paired immunohistochemistry in the proximal jejunal mucosa of 29 patients with dermatitis herpetiformis (DH). The specimens were categorized stereomicroscopically in three groups. Compared with controls, the total number of Ig-producing cells in a defined "mucosal tissue unit" was for group I (normal or minor abnormality) found to be raised by a factor of 1.4, but the class ratios remained normal. In group II (major abnormality) the total immunocyte number was raised 2.6 times; IgA, IgM,…

Adult coeliac disease. A case with ulceration, dermatitis herpetiformis and reticulosarcoma.

A 64-year-old woman with adult coeliac disease developed recurrent ulceration and perforation of the small intestine, dermatitis herpetiformis and a reticulosarcoma of the small intestine. She died following the third perforation. The possible importance of strict adherence to a gluten-free diet in the hope of preventing these complications in patients with adult coeliac disease is discussed.

A comparison of histocompatibility antigens in dermatitis herpetiformis and adult coeliac disease.

The incidence of histocompatibility antigens HL-A, 4a and 4b was studied in thirty-eight patients with dermatitis herpetiformis (DH) and thirty-six patients with adult coeliac disease (ACD). The 4b antigen was found in all the DH and ACD patients. HL-A 8 was found in 89% of patients with ACD--similar to the incidence reported in previous studies--and in 79% of patients with DH, a higher incidence than in previous studies which may be due to stricter criteria being used here to diagnose DH. There was no significant difference in the incidence of HL-A 8 between those patients with DH whose small…
