Asbestos and the lung in the 21st century: an update.

The asbestos-related disorders (ARDs) are currently of significant occupational and public health concern. Asbestos usage has been banned in most developed countries, but asbestos is still used in many developing countries and the number of cases of ARDs worldwide is rising. Many countries are now experiencing an epidemic of ARDs that is the legacy of occupational exposure in the 1960s-1980s because of the long latency period between asbestos exposure and manifestation of disease. It is likely that asbestos-related mortality and morbidity will continue to increase. Although the most feared com…

Epidemiology of hepatitis E in low- and middle-income countries of Asia and Africa.

Hepatitis E is an acute, viral hepatitis primarily transmitted through the fecal-oral route. The first major epidemic of hepatitis E virus (HEV) was reported in 1955 in Delhi, India. Since that time, numerous epidemics have been reported across the low- and middle-income countries in Asia and Africa. Even in the absence of large-scale outbreaks, hepatitis E is an important cause of clinical hepatitis. Serologic studies across Asia and Africa show a high prevalence of anti-HEV antibodies. Interest in hepatitis E has increased over the last two decades. However, there are many unanswered questio

Key harm reduction interventions and their impact on the reduction of risky behavior and HIV incidence among people who inject drugs in low-income and middle-income countries.

PURPOSE OF REVIEW: HIV infection rates continue to rise among people who inject drugs (PWID) in many lower- and middle-income countries (LMICs). Although progress is being made in prevention and care for PWID in some settings, coverage of essential services remains low. This article reviews the evidence for the benefits of scaling up key interventions as a combination prevention and treatment package for PWID. RECENT FINDINGS: WHO defined a comprehensive package of nine interventions for PWID, of which the following four have evidence for effectiveness in reducing HIV incidence: needle and sy

Pattern of head growth and nutritional status of microcephalic infants at early postnatal assessment in a low-income country.

OBJECTIVE: To determine the pattern of head growth and the early postnatal nutritional status of microcephalic infants in a low-income country. MATERIALS AND METHODS: A cohort study in Lagos, Nigeria in which the head growth of full-term singletons within the first postnatal check-up at 6-8 weeks was evaluated using the latest World Health Organization (WHO)'s Child Growth Standards (WHO-CGS) for head circumference. Nutritional status of microcephalic infants at follow-up was also determined after adjustments for potential confounders. RESULTS: Of the 452 infants (male: 227) enrolled, microc

A critical review of interventions for clubfoot in low and middle-income countries: effectiveness and contextual influences.

Inadequate treatment provision for clubfoot in many low and middle-income countries results in a high prevalence of neglected clubfoot, a condition causing severe impairment. This study critically reviewed evidence on surgical, conservative and mixed (Ponseti) treatment interventions for clubfoot in low and middle-income countries. Intervention effectiveness was analysed by comparing outcomes within International Classification of Functioning groupings. Contextual factors were qualitatively analysed for effect on intervention outcomes. The Ponseti method appeared to be more effective than cons

A collaborative public health approach to clubfoot intervention in 10 low-income and middle-income countries: 2-year outcomes and lessons learnt.

Untreated clubfoot has been acknowledged as a public health problem in low-income countries. In 2007-2009, a 10-country initiative was implemented by a collaboration of nongovernmental organizations and Ministries of Health to establish and strengthen national programmes for management of clubfoot using the Ponseti technique. Independent evaluation used quantitative data on programme outcomes and qualitative data from service providers and users. Overall, 110 clubfoot clinics were established, 634 practitioners were trained and 7705 children were enrolled for treatment. The public health model

Cholera vaccine: new preventive tool for endemic countries.

Cholera is a major global public health problem and remains an important threat in almost every developing country, especially in areas where population overcrowding and poor sanitation are common, such as slums and refugee camps. Cholera is one of the most dreaded diseases in the world, in some cases leading to death within 24 h if left untreated. Without treatment, severe infection has a mortality rate of 30-50%. In 2007, WHO recorded 177,963 cholera cases and 4,031 deaths worldwide. However, the estimated actual burden of cholera is in the vicinity of 3 to 5 million cases and 100,000 to 130

[Microscopic diagnosis of amebiasis: an obsolete method but necessary in the developing world].

Molecular biology-based diagnosis offers the best approach to detect amebiasis, but remains impractical in the clinical laboratories from the developing world. In these areas, the microscopic diagnosis remains the routine method. It is imperative that a series of fresh stool specimens be examined. The use of a concentration method should become a routine procedure. Permanent stained smears is the most critical and reliable diagnostic method for the microscopic detection of intestinal protozoa. If the direct or concentrate wet mounts were considered as a preliminary examination; and the use of

Vitamin A supplementation and neonatal mortality in the developing world: a meta-regression of cluster-randomized trials.

OBJECTIVE: To assess the relationship between the prevalence of vitamin A deficiency among pregnant women and the effect of neonatal vitamin A supplementation on infant mortality. METHODS: Studies of neonatal supplementation with vitamin A have yielded contradictory findings with regard to its effect on the risk of infant death, possibly owing to heterogeneity between studies. One source of that heterogeneity is the prevalence of vitamin A deficiency among pregnant women, which we examined using meta-regression techniques on eligible individual and cluster-randomized trials. Adapting standard

Obesity in emerging nations: evolutionary origins and the impact of a rapid nutrition transition.

Here we explore whether there is any evidence that the rapid development of the obesity epidemic in emerging nations, and its unusual coexistence with malnutrition, may have evolutionary origins that make such populations especially vulnerable to the obesogenic conditions accompanying the nutrition transition. It is concluded that any selection of so-called 'thrifty genes' is likely to have affected most races due to the frequency and ubiquity of famines and seasonal food shortages in ancient populations. Although it remains a useful stimulus for research, the thrifty gene hypothesis remains a
