Impacts of biodiversity on the emergence and transmission of infectious diseases.

Current unprecedented declines in biodiversity reduce the ability of ecological communities to provide many fundamental ecosystem services. Here we evaluate evidence that reduced biodiversity affects the transmission of infectious diseases of humans, other animals and plants. In principle, loss of biodiversity could either increase or decrease disease transmission. However, mounting evidence indicates that biodiversity loss frequently increases disease transmission. In contrast, areas of naturally high biodiversity may serve as a source pool for new pathogens. Overall, despite many remaining q

Emergence and spread of drug resistant Neisseria gonorrhoeae.

PURPOSE: The emergence and spread of Neisseria gonorrhoeae with resistance to oral antibiotics have led to difficulty in treating gonorrhea. We review drug resistance in N. gonorrhoeae with a particular emphasis on resistance to fluoroquinolones, cefixime and azithromycin. MATERIALS AND METHODS: Literature selected from peer reviewed journals listed in MEDLINE(R)/PubMed(R) from 1943 to 2009 and from resources cited in those articles was reviewed comprehensively. RESULTS: Due to the spread of fluoroquinolone resistant N. gonorrhoeae fluoroquinolones are no longer recommended for the treatment

Magnetic toys: the emerging problem in pediatric ingestions.

Acute abdominal pain in children presents a diagnostic dilemma. Although most children with acute abdominal pain have self-limited conditions, the pain may herald a surgical or medical emergency. Timely diagnosis is essential so treatment can be initiated and morbidity is prevented. There are common conditions that cause problems in most age groups (such as appendicitis) and others that are usually confined to a specific age group (such as intussusception). In younger children, in addition to anatomical causes of abdominal pain, foreign body ingestions may cause distress. It is crucial for a c

Identifying transverse myelitis in the emergency care arena.

Transverse myelitis (TM) is a neurological syndrome involving destruction of the anterior and posterior (transverse) segments of the spinal cord. The objective of this article is to educate emergency nurses and nurse practitioners about how to recognize, diagnose, and treat TM. A comparison of the neurological findings of TM and Guillain-Barré syndrome is presented. In addition, this article will include an algorithm and specific diagnostic criteria to assist in the diagnosis of acute idiopathic TM.

Topical antibiotics in dermatology: emerging patterns of resistance.

Resistance to topical antibiotics is increasingly relevant in dermatology. This article discusses emerging patterns of resistance and the implications for clinical practice. Emergence of resistance is complicating decolonization of the skin and nares in patients infected with methicillin-resistant Staphylococcus aureus. In the setting of acne treatment, adding topical benzoyl peroxide has been shown to reduce the emergence of strains resistant to topical antibiotics. Other agents, such as zinc salts, are promising in this regard. This article discusses alternatives to antibiotics and mechanism

Dermatological conditions presenting at an emergency department in Singapore.

INTRODUCTION: In Singapore, a significant proportion of patients receive specialist dermatological services via referrals from points of primary care, such as polyclinics and emergency departments (ED). The study hospital, Tan Tock Seng Hospital, is an acute care general hospital with a large catchment area, and has the busiest ED in Singapore. The aim of this paper was to describe the types of dermatological conditions presented at the ED in the year 2007. This information is useful for the future education of junior doctors working in the department, as well as for the allocation of future r

Iron toxicity: new conditions continue to emerge.

During the past half century, excessive/misplaced iron has been observed to be a risk factor for an increasing number and diversity of disease conditions. An extensive list of conditions and of the types of iron association were published in early 2008. Within the subsequent year, four additional disorders have been recognized to be enhanced by iron: aging muscle atrophy, viral replication, rosacea and pulmonary alveolar proteinosis. This paper adds new data and emphasis on these disorders as entities associated with increased iron load and toxicity.

[EEG examination in emergency medicine and analysis of unconciousness].

For a long time, the EEG examination has been used as a laboratory examination especially in the diagnostics of epilepsy, but both technical difficulties and problems in interpretation have limited its use in emergency medicine outside special neurological intensive care units. Development of monitoring technology and easier data transfer is making EEG monitoring possible also in the emergency area. The greatest need focuses on the diagnostics and treatment of status epilepticus underlying an unclear unconsciousness. In other indications the usually required information can be obtained by dayt

Fascioliasis: can Cuba conquer this emerging parasitosis?

Fascioliasis, an emerging parasitic infection, impacts significantly on both veterinary and human health worldwide. Endemic foci are not limited only to areas of extensive livestock farming, but owing to the parasite's abilities to colonise new intermediate hosts and adapt to new environments, also occur in other places, including Cuba. In Cuba, despite a high prevalence of fascioliasis in livestock, and the widespread occurrence of two potential intermediate hosts, human infection has decreased steadily over the past 10 years. In other parts of the world, human fascioliasis is apparently beco

Anabolic-androgenic steroid dependence: an emerging disorder.

AIMS: Anabolic-androgenic steroids (AAS) are widely used illicitly to gain muscle and lose body fat. Here we review the accumulating human and animal evidence showing that AAS may cause a distinct dependence syndrome, often associated with adverse psychiatric and medical effects. METHOD: We present an illustrative case of AAS dependence, followed by a summary of the human and animal literature on this topic, based on publications known to us or obtained by searching the PubMed database. RESULTS: About 30% of AAS users appear to develop a dependence syndrome, characterized by chronic AAS use

Orbital compartment syndrome: the ophthalmic surgical emergency.

Orbital compartment syndrome is an uncommon, ophthalmic surgical emergency characterized by an acute rise in orbital pressure. When intraorbital tension rises, damage to ocular and other intraorbital structures, including irreversible blindness, may occur if not promptly treated. The diagnosis of orbital compartment syndrome is completely clinical and early recognition and emergent orbital decompression (even prior to imaging) is essential in preventing permanent vision loss. Lateral canthotomy and inferior cantholysis remain the mainstays of management. More extensive incision of the orbital

Emerging roles of TLR7 and TLR9 in murine SLE.

Systemic lupus erythematosus (SLE) is an autoimmune disorder characterized by B cell hyperactivity leading to the production of various autoantibodies and subsequent development of glomerulonephritis, i.e. lupus nephritis. Among the principal targets of autoantibodies produced in murine SLE are nucleic acid-protein complexes, such as chromatin and ribonucleoproteins, and the envelope glycoprotein gp70 of endogenous retroviruses. The preferential production of these autoantibodies is apparently promoted by the presence of genetic abnormalities leading to defects in the elimination of apoptotic

Predicting the complicated neutropenic fever in the emergency department.

ObjectivesThe purpose of this study was to identify independent factors that can be used to predict whether febrile neutropenic patients who appear healthy at presentation will develop subsequent complications, using variables that are readily available in the emergency department (ED).MethodThe medical records of 192 episodes in which the patients presented to the ED with neutropenic fever resulting from chemotherapy, with an alert mental state and haemodynamic stability were retrospectively reviewed. Endpoints examined were fever response to administered antibiotics, death or severe medical

Emerging drugs for small-cell lung cancer.

Small-cell lung cancer (SCLC) is a rapidly progressing tumor in which chemotherapy has a limited impact on survival. Unfortunately, little progress has been made in the medical management of SCLC during the last 30 years, which is best exemplified by the fact that standard first-line chemotherapy has remained platinum-based over time. On the other hand, improvements in survival have been obtained only with the introduction of innovative radiation strategies such as accelerated hyperfractionation to the thorax for limited-stage disease and prophylactic cranial irradiation for both limited- and

Emerging antiviral drugs for hepatitis C virus.

Infection with hepatitis C virus (HCV) is a global health problem that affects approximately 170 million people worldwide. The current standard therapy with peginterferon alpha plus ribavirin for 48 weeks results in a sustained virologic response in less than 50% of patients with chronic hepatitis C genotype 1--the most prevalent type of HCV in North America and Europe. Development of new antiviral medicines has been hampered by the lack of an effective cell culture system and small-animal model. Herein we review recent progress in the development of new treatments under investigation in clini

Etiological characterization of acute poisonings in the emergency department.

INTRODUCTION: An investigation of emergency department (ED) poisonings was conducted to characterize poisoning demographics and evaluate correlations with select co-morbidities. METHODS: The study population evaluated consisted of 649 poisoning cases admitted between 2004 and 2007 to an inner-city, level 1 emergency department. RESULTS: Ethnicity, age, and gender had a substantial impact on the population distribution as poisoning cases were predominantly African Americans (79.9%) between 36 and 45 years old with a 1:3 male to female ratio. Intentional illicit drug overdose was the most prev
