Bone Formation Between 2.5 and 25% Interfragmentary Strain Induced by Immediate and Delayed Loading in a Bone Healing Model with a Monotonic Strain Gradient.

PURPOSE: This study investigated the formation of fracture repair tissue in response to 2.5-25% strain magnitudes under immediate and delayed loading in a large animal model with monotonically increasing interfragmentary strain. METHODS: Experimental osteotomies were created in ten sheep and were instrumented with an active fixator that generated a gradient (2.5-25%) of interfragmentary strain across the osteotomy. Sheep were randomly assigned to an immediate-loading (from day 1 post-surgery) group or a delayed-loading (from day 22 post-surgery) group. Five weeks post-surgery, the tibiae were

Surgical management of a blast-induced ankle injury complicated by myiasis using cross-leg flap reconstruction: A case report.

INTRODUCTION: Blast injuries to the lower extremities often result in extensive soft tissue damage and are prone to complications such as infection and, rarely, maggot infestation. These challenges can jeopardize limb salvage, especially in resource-limited settings. We report a successfully managed case of a neglected anterior ankle blast wound complicated by infection and myiasis, treated with a multidisciplinary approach incorporating sunlight exposure and a cross-leg flap. CASE PRESENTATION: A 38-year-old woman presented with a 20-day-old anterior ankle blast injury complicated by infecti

Gastropleurocutaneous fistula secondary to cement prosthesis migration: A rare late complication of chest wall sarcoma reconstruction.

BACKGROUND: Gastropleurocutaneous fistula (GPCF) is a rare, potentially life-threatening condition defined by an abnormal connection between the stomach and the pleural cavity with an accompanying cutaneous tract. It is distinct from a gastropleural fistula, which lacks an external cutaneous extension. To our knowledge, no prior reports have described GPCF caused by migration and erosion of a cement chest-wall prosthesis. CASE PRESENTATION: We present the case of a 40-year-old female with a history of anterior chest wall sarcoma resection and cement prosthesis reconstruction performed 23 year

Pelvic support osteotomy with subsequent femoral lengthening: a reliable treatment for late tuberculous coxitis in skeletally immature patients.

INTRODUCTION: Tuberculous (TB) coxitis, constituting 15-20% of musculoskeletal tuberculosis cases, often leads to late-stage complications. Joint replacement yields unsatisfactory results. This study aims to know about the efficacy of pelvic support osteotomy (PSO) with subsequent femoral lengthening in treating late TB coxitis in skeletally immature patients. METHODS: Thirteen pediatric patients with late TB coxitis underwent retrospective analysis from 2016 to 2024. PSO with femoral lengthening, utilizing limb reconstruction systems (LRS) external fixator, was performed. Outcomes were asses

Development and validation of a tool for ambulatory monitoring of peripherally inserted central catheter-associated complications.

INTRODUCTION: Peripherally inserted central catheters (PICC) are the preferred vascular access for Outpatient Parenteral Antimicrobial Therapy (OPAT) due to user-friendliness and high safety level. Nevertheless, the lack of a validated tool hampers the yet ill-charted epidemiology and monitoring of PICC related complications in ambulatory care. METHODS: A sequential exploratory mixed-method three phase research design was used to develop and validate a monitoring tool for PICC complications in OPAT. A literature review and co-design approach allowed its development, followed by Delphi panel c

An undiscovered circadian clock to regulate phytoplankton photosynthesis.

Circadian clocks exist in all types of organisms and coordinate key biological processes, e.g. photosynthesis in phytoplankton (microalgae) and land plants. We asked whether a circadian rhythm sustains in phytoplankton when living under constant illumination without environmental cues. Here, we report the first transcriptomic architecture of persistent oscillatory gene expression in the model marine diatom, Phaeodactylum tricornutum living under constant illumination and temperature without environmental cues. We show that cyclic expression of a considerable number of genes involved in light h

[Single-cut derotational osteotomy of the distal femur for correction of torsion and frontal axis].

OBJECTIVE: A rotational osteotomy requires a complete cut of the bone in order to correct maltorsion. An additional correction of the frontal axis can be achieved via an oblique cut of the bone. The osteotomy with bone to bone contact is fixed with an angle stable plate. INDICATIONS: Symptoms such as anterior knee pain, inwardly pointing knee syndrome, lateral patellar subluxation or dislocation, lateral patellar hypercompression syndrome are a common indication for derivational osteotomy if clinically increased femoral internal rotation and radiologically increased femoral antetorsion is det

Arthroscopic Management of an Extremely Rare Wrist Fracture-Dislocation Pattern.

BACKGROUND: Axial dislocation involves the displacement of carpal bones along the longitudinal axis of the wrist joint. There are three major groups: (1) axial ulnar, (2) axial radial, and (3) combined. On the other hand, radiocarpal dislocation entails disruption of the articulation between the radius and carpal bones. There are two groups: (group 1) it is either a pure radiocarpal dislocation or a minor avulsion of the radial styloid and (group 2) a fracture-dislocations characterized by a significant radial styloid fragment. CASE DESCRIPTION: The patient, a 66-year-old female involved in a

Anterior cervical discectomy and fusion for the treatment of pediatric Hirayama disease.

PURPOSE: Hirayama disease, a rare cervical myelopathy in children and young adults, leads to progressive upper limb weakness and muscle loss. Non-invasive external cervical orthosis has been shown to prevent further neurologic decline; however, this treatment modality has not been successful at restoring neurologic and motor function, especially in long standing cases with significant weakness. The pathophysiology remains not entirely understood, complicating standardized operative guidelines; however, some studies report favorable outcomes with internal fixation. We report a successful surgic

Femoral neck fracture in sickle cell anemia: A comprehensive case study.

KEY CLINICAL MESSAGE: The significance of taking femoral neck stress fractures into account as a possible consequence in sickle cell anemia patients is underscored by this case report. In this high-risk group, early identification, timely diagnosis, and suitable care are crucial for averting major problems and improving results. When sickle cell anemia patients complain with hip or groin discomfort, healthcare practitioners should keep a high index of suspicion for femoral neck stress fractures to assure prompt management and prevent long-term impairment. ABSTRACT: Sickle cell disease (SCD) i

A case of early-onset congenital syphilitic osteomyelitis of the calcaneus and literature review.

BACKGROUND: Congenital syphilis (CS) is a sexually transmitted disease caused by Treponema pallidum (TP). When the skeletal system is involved, it often results in multiple, symmetrical bone destruction at the epiphyses of long tubular bones such as the humerus and radius, rarely involving the calcaneus. This article reports a case of calcaneal osteomyelitis caused by TP in a child with no other bone damage and subtle clinical manifestations, No similar cases have been reported. CASE PRESENTATION: A 4-month-old male infant presented with right foot swelling without any obvious cause and no hi

Virtual reality as a countermeasure for astronaut motion sickness during simulated post-flight water landings.

Entry motion sickness (EMS) affects crewmembers upon return to Earth following extended adaptation to microgravity. Anticholinergic pharmaceuticals (e.g., Meclizine) are often taken prior to landing; however, they have operationally adverse side effects (e.g., drowsiness). There is a need to develop non-pharmaceutical countermeasures to EMS. We assessed the efficacy of a technological countermeasure providing external visual cues following splashdown, where otherwise only nauseogenic internal cabin visual references are available. Our countermeasure provided motion-congruent visual cues of an
