Hydatidiform moles and teratomas confirm the human identity of the preimplantation embryo.

Results of recent research on hydatidiform moles and teratomas show that during pregnancy the embryo does not receive any message or information from the mother able to control the mechanisms of development or to produce the type of cellular differentiation necessary for building the tissues of the new human adult. Thus, the biological identity of the new human being does not depend on the sojourn in the uterus; the preimplantation embryo is the same individual of the human species as the adult, into whom the embryo can in principle develop.

Roles and applications of artificial intelligence in fetal and placental MRI: a literature review.

BACKGROUND: Artificial intelligence (AI) has been increasingly integrated with fetal and placental magnetic resonance imaging (MRI) to enhance the detection of abnormalities and streamline diagnostic processes. MRI, known for its superior soft tissue contrast and multiplanar imaging capabilities, is a critical tool for evaluating complex fetal and placental conditions. However, the large volume of MRI data often poses challenges for clinicians in providing timely and accurate diagnoses. Fetal and placental MRI are subject to several inherent limitations, including fetal motion artifacts, low s

In vivo physiological ramifications of placental SLC2A3-RNA interference throughout gestation in sheep.

RNA interference (RNAi) of the placental glucose transporter SLC2A3 resulted in smaller hypoglycemic fetuses with reduced umbilical artery insulin and glucagon concentrations at mid-gestation (75 d of gestation [dGA]) in sheep. Our current objective was to determine the ramifications of SLC2A3-RNAi throughout gestation, and to assess fetal insulin secretion in response to glucose and arginine challenges. We successfully generated SLC2A3-RNAi (n = 6) and NTS-RNAi (non-targeting sequence) control sheep pregnancies (n = 6). Near-term they underwent surgical catheterization followed by in vivo met

SegQC: a segmentation network-based framework for multi-metric segmentation quality control and segmentation error detection in volumetric medical images.

Quality control (QC) of structures segmentation in volumetric medical images is important for identifying segmentation errors in clinical practice and for facilitating model development by enhancing network performance in semi-supervised and active learning scenarios. This paper introduces SegQC, a novel framework for segmentation quality estimation and segmentation error detection. SegQC computes an estimate measure of the quality of a segmentation in volumetric scans and in their individual slices and identifies possible segmentation error regions within a slice. The key components of SegQC

Prenatal diagnosis of tracheo-oesophageal fistula/oesophageal atresia: is MRI helpful?

BACKGROUND: Oesophageal atresia (OA) with or without tracheo-oesophageal fistula (TOF) affects 2.75 per 10,000 births within the UK. It is most frequently suspected on antenatal imaging when the stomach is absent or appears small. Studies have shown fetal magnetic resonance imaging (MRI) has greater diagnostic accuracy than ultrasound; however, there remains uncertainty over what size constitutes a small stomach and how frequently this correlates with a diagnosis of TOF/OA. METHODS: A retrospective study of patients referred for fetal MRI due to suspicions of TOF/OA on antenatal ultrasound fr

Intrauterine Gardnerella vaginalis Infection Results in Fetal Growth Restriction and Alveolar Septal Hypertrophy in a Rabbit Model.

Background: Gardnerella vaginalis (GV) is most frequently associated with bacterial vaginosis and is the second most common etiology causing intrauterine infection after Ureaplasma urealyticum. Intrauterine GV infection adversely affects pregnancy outcomes, resulting in preterm birth, fetal growth restriction, and neonatal pneumonia. The knowledge of how GV exerts its effects is limited. We developed an in vivo animal model to study its effects on fetal development. Materials and Methods: A survival mini-laparotomy was conducted on New Zealand rabbits on gestational day 21 (28 weeks of human p…

Human placental biotin transport: normal characteristics and effect of ethanol.

Biotin, a vitamin essential for many metabolic reactions, is supplied to the fetus exclusively from the mother. Deficiency of biotin in pregnancy leads to impaired fetal growth and development. Alcohol taken in pregnancy likewise may cause fetal growth abnormalities. Normal biotin transport via the placenta and the effects of ethanol on this transport apparently have not been studied. Our aims were to characterize these phenomena for the normal human-term placenta. Using maternal-facing placental membrane vesicles, biotin uptake was sodium- and temperature-dependent, saturable, and inhibited b

Fetal Lymphoid Organ Immune Responses to Transient and Persistent Infection with Bovine Viral Diarrhea Virus.

Bovine Viral Diarrhea Virus (BVDV) fetal infections occur in two forms; persistent infection (PI) or transient infection (TI), depending on what stage of gestation the fetus is infected. Examination of lymphoid organs from both PI and TI fetuses reveals drastically different fetal responses, dependent upon the developmental stage of the fetal immune system. Total RNA was extracted from the thymuses and spleens of uninfected control, PI, and TI fetuses collected on day 190 of gestation to test the hypothesis that BVDV infection impairs the innate and adaptive immune response in the fetal thymus

Fetoscopic surgery: encouraged by clinical experience and boosted by instrument innovation.

Today, modern ultrasound equipment and the wide implementation of screening programmes allow the timely diagnosis of many congenital anomalies. For some of these, fetal surgery may be a life-saving option. In Europe, open fetal surgery became poorly accepted because of its invasiveness and the high incidence of postoperative premature labour and rupture of the fetal membranes. In the 1990s, the merger of fetoscopy and advanced video-endoscopic surgery formed the basis for endoscopic fetal surgery. We review the current applications of fetal surgery via both methods of access. The first clinica

Sexual dimorphism in lung transcriptomic adaptations in fetal alcohol spectrum disorders.

Current fetal alcohol spectrum disorders (FASD) studies primarily focus on alcohol's actions on the fetal brain although respiratory infections are a leading cause of morbidity/mortality in newborns. The limited studies examining the pulmonary adaptations in FASD demonstrate decreased surfactant protein A and alveolar macrophage phagocytosis, impaired differentiation, and increased risk of Group B streptococcal pneumonia with no study examining sexual dimorphism in adaptations. We hypothesized that developmental alcohol exposure in pregnancy will lead to sexually dimorphic fetal lung morpholog

Functional maturation of the primate fetal adrenal in vivo: 3. Specific zonal localization and developmental regulation of CYP21A2 (P450c21) and CYP11B1/CYP11B2 (P450c11/aldosterone synthase) lead to integrated concept of zonal and temporal steroid biosynthesis.

Previous studies in the primate fetal adrenal gland have indicated that the gland is comprised of three functional zones: 1) the inner fetal zone (FZ), which has the enzymes necessary for dehydroepiandrosterone sulfate (DHEAS) production beginning early in gestation; 2) the transitional zone (TZ), which possesses enzymes necessary for cortisol production; and 3) the outer, definitive zone (DZ), which appears to function as a reservoir of progenitor cells that may populate the remainder of the gland and does not acquire a steroidogenic phenotype with the capacity to produce mineralocorticoids u

Good prognosis for psychomotor development in survivors with nonimmune hydrops fetalis.

We examined the psychomotor development of 33 of 61 surviving children, from a series of 107 consecutive live-born cases with nonimmune hydrops fetalis. The majority had a normal outcome. Three had a (simultaneous) serious underlying disease (e.g. fetal herpes infection) and had either severe psychomotor retardation or blindness. Two showed clumsiness and were considered to have minor neurological dysfunction. We conclude that survivors, especially those with transient benign intrauterine conditions, such as lymphatic aetiology have no additional risk to their psychomotor development.

Oxygen and glucose supplementation increases rates of linear growth and cell proliferation in growth-restricted fetal sheep.

Placental insufficiency lowers oxygen and glucose concentrations in the fetus, which causes fetal growth restriction (FGR). Previously we showed that FGR fetuses chronically supplemented with oxygen and glucose (OG) have improved glucose tolerance. However growth was not evaluated. Here we test the hypothesis that sustained OG supplementation to an FGR fetus will increase linear growth rates, raise anabolic hormone concentrations and promote proliferation rates in pancreatic β-cells and skeletal muscle satellite cells, which are two tissues involved in glucose regulation. FGR was induced in sh

Safety and feasibility pilot study of continuous low-dose maternal supplemental oxygen in fetal single ventricle heart disease.

OBJECTIVE: Fetuses with single ventricle physiology (SVP) exhibit reductions in fetal cerebral oxygenation, with associated delays in fetal brain growth and neurodevelopmental outcomes. Maternal supplemental oxygen (MSO) has been proposed to improve fetal brain growth, but current evidence on dosing, candidacy and outcomes is limited. In this pilot study, we evaluated the safety and feasibility of continuous low-dose MSO in the setting of SVP. METHODS: This single-center, open-label, pilot phase-1 safety and feasibility clinical trial included 25 pregnant individuals with a diagnosis of fetal

Maternal AMPK pathway activation with uterine artery blood flow and fetal growth maintenance during hypoxia.

High-altitude (HA) hypoxia lowers uterine artery (UtA) blood flow during pregnancy and birth weight. Adenosine monophosphate kinase (AMPK) activation has selective, uteroplacental vasodilator effects that lessen hypoxia-associated birth weight reductions. In this study, we determined the relationship between AMPK-pathway gene expression and metabolites in the maternal circulation during HA pregnancy as well as with the maintenance of UtA blood flow and birth weight at HA. Residents at HA (2,793 m) versus low altitude (LA; 1,640 m) had smaller UtA diameters at weeks 20 and 34, lower UtA blood f
